Summary:
The Med Surg Registered Nurse delivers comprehensive high-quality, patient-centered care in medical-surgical and telemetry units. This role involves assessing patient health conditions, monitoring cardiac telemetry data, and collaborating with multidisciplinary teams to manage diverse patient needs. The position requires strong clinical skills in med-surg and telemetry nursing, critical thinking abilities, excellent communication, and a commitment to maintaining the highest standards of patient safety and professional ethics.
Essential Work Functions:
Explain procedures and treatments to patients and families, addressing concerns and ensuring understanding
Administer prescribed medications and treatments in adherence to nursing standards
Conduct comprehensive assessments of patients' conditions, documenting changes and reporting concerns to the care team
Prepare patients for diagnostic procedures, assist physicians during treatments, and monitor patient responses
Maintain a safe and comfortable environment for patients recovering from medical conditions or surgical procedures
Document comprehensive patient information, including vital signs, medications, treatments, and care outcomes, in the medical record
Collaborate with interdisciplinary teams, including physicians, physical therapists, and social workers, to create and implement individualized care plans
Manage multiple patients efficiently, prioritizing care based on acuity and clinical needs
Ensure infection control practices are strictly followed, including hand hygiene and PPE use
Advocate for patients' needs and preferences, ensuring they are respected and integrated into the care plan
Facilitate discharge planning, coordinating with patients, families, and external resources to ensure a smooth transition from hospital to home
Perform other duties as assigned within the scope of practice
Adhere to hospital safety protocols, infection control guidelines, and regulatory standards

The Crisis Recovery Center
The Crisis Recovery Center (CRC) is the critical next step in DuPage County Health Department's decades-long journey to build the infrastructure needed to support a best-in-class behavioral health system and ensures all DuPage County residents will have "someone to contact, someone to respond, and a safe place to get help" when experiencing a mental health or substance use crisis. The CRC will help residents de-escalate, stabilize, and connect to community resources catered to their individual mental health and substance use treatment needs. The CRC serves as an alternative to hospital emergency departments and simplifies access to crisis care for all individuals, families, and first responders. This full-time evening position is scheduled to work Friday - Monday, from 2:00 PM to 12:00 AM. The hiring range for this position is $44,400 to $52,000, depending on experience.

Responsibilities
As a Crisis Support Specialist, you will:
- Assist adult clients by facilitating illness self-management skill building, identification and use of adaptive and compensatory skills, utilization of natural resources, and connection to community resources, while working in the Crisis Residential Program. In addition, provide similar supports in the mental health units at the Crisis Recovery Center.
- Provide case management services
- Act as a care coordinator, facilitating transition from a crisis point of care to ongoing services
- Collaborate with a multidisciplinary team and other community agencies to ensure appropriate levels of service delivery
- Interview and gather information from collateral contacts, clients, and caregivers to provide appropriate resources
- Complete client assessments, treatment plans, and participate in client engagement activities

Requirements
- Bachelor's or Master's degree in Psychology, Social Work, or related field
- 1+ years of experience working with chronically mentally ill populations

In this on-call position, you will be required to work at least three (3) shifts per month, having evening and weekend availability, working no more than 1000 hours per year. We offer a $2.00 per hour shift differential for our second shift and $2.50 for our overnight shift.
Responsibilities
Supports maintenance of a safe, therapeutic environment for clients seeking services through Crisis Recovery Center programs on the Substance Use Stabilization Unit, Adult Mental Health Unit, and Youth Mental Health Unit, as well as in Crisis Residential.
Facilitates client safety and quality care in Crisis Residential and Crisis Recovery Center Programs, through completion of client rounding and belongings checks
Assists with client medication observation in Crisis programming as needed
Performs tasks related to maintaining environmental standards in crisis programming
Assist in de-escalation of clients experiencing a mental health crisis through engagement with client and coordination with team members providing clinical intervention.
Completion of required documentation within established timeframes, through use of an Electronic Client Record

A qualified candidate must possess a valid State of Illinois Driver's license and have a good driving record. New employees are required to attend paid on-site, HR New Hire Orientation at our Central Public Health Center in Wheaton, during our day shift generally, from 8:00 am to 4:30 pm, for one week. Must have flexibility to work on the day shift for two weeks of additional training on site at the CRC.

What You Will Do At Gema
* Troubleshoot and service equipment in customer's facility by scheduling in advance, by unscheduled courtesy visits or emergency service calls. Essential Functions:
Perform all tasks as required in the written plan.
Meet productivity requirements as established by the program director or designee.
Provide client monitoring and linkage to appropriate services both within LSSI and other community agencies.
Complete and submit all necessary reports, billing, and documentation accurately and on time as required by agency and funding sources.
Communicate with supervisor, work group and collateral agencies regarding individual's and /or family's needs.
Position Qualifications:
High school diploma (or GED) required.
AA Degree with two years of college preferred.
Age requirement may apply.
Record of compliance with the law and clearance on the Illinois CANTS
Basic knowledge of child development and parenting or equivalent life experience preferred.
Demonstrate ability to communicate both verbally and in writing in a clear, comprehensible manner.
Valid driver's license in good standing for the state of residency required.
Access to reliable transportation for visits, meetings, and to transport clients as needed.
Valid IL statutory minimum liability insurance coverage, bodily injury, and property damage.
