Case Manager Dept: Preble County- SBHI Shift: Part Time - Monday-Thursday 9:00 AM- 2:00 PM Case Manager functions as a member of a team to assist in the planning, development, and delivery of outcome-effective treatment services in various types of settings. This position maintains an ongoing working knowledge of agencies and community/emergency resources for, adult, and geriatric clients and uses that knowledge to coordinate services and advocate for identified clients. Case Manager acts as a resource consultant to other team members and collects resource materials regarding other agencies and services in the community. Patient Care Coordinator- Infusion/ Oncology Department: Outpatient Infusion SVCS Shift: 7AM-5:30PM/ Rotating hours/ 8 hour shifts Status: Part-time / 48 hours per pay period/ 24 hours per week Facility: Miami Valley South The Infusion Center Patient Care Coordinator (PCC) is responsible for tasks that are integral to complex, interdisciplinary patient care coordination and is a multi-skilled individual able to perform administrative and clinical duties under the supervision of the Registered Nurse and delegated in accordance with the Ohio Board of Nursing. The PCC is the initial point of contact for all patients and visitors to the department. This individual is responsible for administrative duties that may include coordinating all incoming calls, collecting a variety of patient data, entering patient information and charges into the hospital system, scheduling appointments, completion of registration by collecting and entering all pertinent financial and demographic information into the ADT system, verifying insurance benefit information, generation of the ABN, reviewing orders for compliancy, completion of MSP, obtaining financial and treatment consents, scheduling ordered medical tests and procedures, obtaining a pre-certification when applicable, preparation of patient hybrid charts, and providing clerical support while maintaining compliance with regulatory requirements. The Critical Care Transport Nurse utilizes the nursing process in conjunction with evidence-based principles to provide safe and rapid transport of critically ill or injured patients by a mobile intensive care to a higher-level medical treatment facility. The Critical Care Transport Nurse provides advanced emergency/critical care during medical transport and may also provide nursing care for patients throughout Premier Health. The YCATS therapist works with young children ages birth to 8 years and their families providing strengths-based mental health therapy services. The YCATS Program is looking for therapists with experience working with children and families or a strong desire to learn to work with young children and families. The YCATS therapist works as part of a team and may share clients with other therapists when clients are engaged in multiple services. Our 90-day orientation period offers job shadowing and observation opportunities, multiple trainings offering CEU's, and gradual building of therapist caseload. Part-time, full-time, and associate therapist positions are available.
