JOB SUMMARY The Customer Service Representative is responsible for answering a high volume of incoming calls from patients and accurately schedule their appointments or manage their request while ensuring a high level of customer service and confidentiality while maximizing productivity. The representative is also responsible for accurate and timely handling of work assignments received from multiple electronic platforms while establishing and maintaining good interpersonal relationships with patients, their families, the public, and co-workers. This position works collaboratively with interdepartmental staff to assist the patient and facilitate successful patient interaction with the practice team. This position reflects and carries forward the practice's mission and goals internally and throughout the community ESSENTIAL JOB RESPONSIBILITIES: RESPONSIBILITY 1. JOB SUMMARY This member will be a part of the Quality Health care team that will assist in coordinating the management of special populations of patients at LLCHC. DUTIES RESPONSIBILITY 1. Team Based Care Coordination * Work in collaboration with multidisciplinary provider team to provide innovative and evidence-based care. * Assist in the care coordination of chronic care patients as requested by the Quality Manager, Value Based Care Management Director, or CHIEF QUALITY OFFICER. * Carefully coordinate aspects of the patient's care working with Referral coordinators Care managers, Social Workers etc., on referrals to specialists, Post ER/ hospitalizations follow up ancillary testing, and other enabling services tracking. * Responsible for responding to patient calls promptly, including telephone advice per protocol, handle urgent calls and emergent calls, escalating to Quality Manager or Value Based Care Management Director as needed. * Assist with triage of urgent concerns that occur over the phone escalating to Quality Manager or Value Based Care Management Director when needed. * Documents all interactions with patient in the EHR. * Report on self-management tasks and abnormal findings relayed to them by the patient to the appropriate Physician team and/or other members of the Care Team. * Educates patients about in self-management tasks per Protocol, or as directed by Quality Manager or Value Based Care Management Director. * Delivers patient self-management educational resources used by the Practice Providers. RESPONSIBILITY 2. Population Health Care Coordination * Anticipate the needs of the patients seeing that necessary documentation is completed or requested before patient visit. * Responsible for working with patient and patient's care team to implement an individualized treatment care plan - including follow-up, labs, and care coordination. * Use of technology to assist with all aspects of care coordination, electronic medical record documentation, documentation prompts. * Participates in the process of engaging the various sets of individuals when opportunities such as chronic disease management, wellness promotion, disease prevention, practice population management program are available by using EHR a reporting to determine which patients are overdue for care, services, testing, and/or screening as required by the Quality Manager, Value Based Care Management Director or Chief Quality Officer * Participates in the preventive care reminder program for the practice's patients, ensuring that patients receive reminders of the need for preventive or disease management screening and testing, including point of care reminders, as required by the Quality Manager, Value Based Care Management Director or Chief Quality Officer * Assists the Physician care teams in ensuring a smooth transition of care for patients treated in a facility (inpatient or emergency department), by a specialty physician (partialists), or by another health care provider. * Assists patient care teams with pre-visit preparation (obtaining medical records, hospital discharge summaries, advising patients to bring meds with them). * In addition, participating in appointment scheduling per protocols, referral, and diagnostic test tracking. RESPONSIBILITY 3. Administrative Tasks * Aids in developing a community resource list of services available to patients and maintains collegial relationships with community resource agencies used most frequently. * Refers patients to other entities for education and support (Social Worker, Care Manager etc.) as directed by Quality Manager, Value Based Care Management Director or Chief Quality Officer * Assist patient with information and resources for medical supplies within the Organization when necessary. Working hours are Monday - Friday 8:00am to 5:30pm with occasional Saturday morning coverage. JOB DUTIES The Practice Manager provides leadership and direct site management for day-to-day operations within LLCHC locations. This includes ensuring adequate and thoroughly trained staffing, process and procedure adherence, and monitoring of and adherence to quality metrics to support the sites(s). Hands-on engagement with identified improvement initiatives will be critical. Daily attention to the assigned site(s)' exterior and interior appearance is required. Especially important is a focus on creating a culture and environment that support a consistently positive patient experience. The Practice Manager works collaboratively with the Director of Operations, front and back-office staff, Providers, Human Resources, Senior Management, and other Practice Managers to develop and implement training that creates best practice to satisfy patient care, as well as to measure team member performance against benchmark metrics. Use of this data, coupled with observations and feedback from others for progressive development of team members will be key. Also critical for this position is ensuring provider, team member, and patient satisfaction through continual open communication. RESPONSIBILITY 1- Office and Clinic Management * Provides direct supervision of front and back-office staff. Oversees enhancement of these department processes. * Responsible for daily professional appearance of assigned sites, including the exterior of buildings. * Orients new staff at appropriate levels. * Communicates process changes, issues and accolades for staff, providers and management. * Follows an inventory control process for all office and medical supplies, regularly placing orders to correct inventory deficiencies. * Responsible for organizing and conducting regular site meetings. * Maintains site regulatory compliance: HIPAA, OSHA, CLIA, Terminal Distribution; 340B Floor Medications * In collaboration with the Director of Operations, monitors site specific productivity goals and objectives for the site. * Ensures adequate staff coverage for the responsible sites of LLCHC. * Ensures site participation in the Quality Improvement and Compliance plans. * Communicates with Director of Operations and site-based providers on operational and oversight issues. * Participates in administrative meetings as needed. * Oversees proper use, maintenance, problem issues and arranges the repair of office equipment, including computer, fax, copier, and phone. * Oversees opening/closing procedures for sites. * Independently composes, compiles and prepares any department reports, audits and documents as assigned. * Other duties within scope of position as assigned. RESPONSIBILITY 2- Personnel * Evaluates performance of front office and clinical staff, students and volunteers. Maintains performance evaluations schedule. * Creates and delivers team member coaching and improvement plans. * Interviews, hires and trains new team members and ensures proper training to include competency training and testing. * Selects candidates for, assists with interview coordination if needed, and identifies successful candidates to fill open positions. * Approves time sheets and completes all needed HR and payroll paperwork for supervised team members. * Tracks absenteeism and approves time off requests. * Executes LLCHC's progressive discipline process with team members as required. Complete follow-up phone calls to patients for provider, using medical interpretation tools as needed. B. Call in prescriptions and/or refill, per provider's order, to pharmacies. C. Instruct patients on test preparation. D. Confer with providers as necessary regarding patient care, teaching and testing. E. Document completely and clearly all assessment data, patient education, and telephone calls in the medical record. F. Document completely any encounters with patient/family. G. Assure documentation is legible and completed in a timely manner. RESPONSIBILITY 4. Demonstrate ability to maintain quality control standards and knowledge of common safety hazards and precautions to establish a safe work environment. A. Maintain current basic life support (BLS) certification for health care workers. B. Demonstrate proper hand washing technique and frequency. C. Demonstrate thorough knowledge of universal precautions by donning and doffing equipment appropriately when exposed to blood/body fluids. D. Practice fire safety and be knowledgeable of all general emergency procedures. RESPONSIBILITY 5. Prepare and maintain records for compliance with all regulatory agencies in accordance with the guidance set forth in the Clinical Manual. A.
