JOB DESCRIPTION JOB TITLE: Nurse Practitioner LOCATION: Poughkeepsie, New York JOB STATUS: Full-time DATE: January 2025 JOB SUMMARY Premier is seeking a Nurse Practitioner who aspires to practice urology. Nurse Practitioner will be responsible for seeing patients independently for office visits as well as under the guidance of a supervising physician. This role requires a high level of clinical expertise, the ability to manage a fast-paced environment, and the flexibility to take both weekday and weekend call shifts to support patient needs. Key Responsibilities:
Conduct thorough patient assessments, including history taking, physical exams, and diagnostic evaluations related to urological conditions (e.g., urinary tract infections, prostate issues, kidney stones, bladder disorders, etc.).
Develop and implement individualized treatment plans in collaboration with supervising urologists.
Prescribe medications, perform minor procedures, and provide follow-up care for patients.
Counsel and educate patients on urological health, preventive measures, and treatment options, ensuring they understand their conditions and treatment plans.
Provide comprehensive care by taking weekday and weekend on-call shifts to address urgent and emergent urological issues.
Respond promptly to calls for advice, triaging cases, and providing necessary interventions.
Provide call coverage remotely 1 weekday evening (Monday-Thursday) 5pm to 8am, and 1 weekend per month, Friday 5pm to Monday 8am.
Communicate clearly and effectively with patients, families, and the healthcare team during on-call duties, offering remote consultations when appropriate or directing patients to the practice or emergency care if needed.
Work closely with the urologists and healthcare team to manage patient care across a variety of conditions.
Ensure accurate and timely documentation of patient interactions, treatment plans, and follow-up actions in the electronic health record (EHR).
Communicate effectively with patients and their families, offering guidance, reassurance, and clear instructions.
Educate patients on urological health, lifestyle modifications, preventive care, and managing chronic conditions (e.g., incontinence, erectile dysfunction, etc.).
Provide counseling and support for patients undergoing urological surgeries or treatments, including pre- and post-operative care.
Adhere to all practice protocols, clinical guidelines, and legal requirements related to patient care, privacy, and documentation.
Participate in quality improvement initiatives to enhance patient care and the operational efficiency of the practice.
Maintain accurate records for billing, coding, and insurance purposes, ensuring compliance with all healthcare regulations.
EDUCATION & EXPERIENCE
Graduate of an accredited Nurse Practitioner Program; NYS licensed
1 - 5 years related experience; or equivalent combination of education and experience.
QUALIFICATIONS & REQUIREMENTS
Prior experience in managing call coverage, including handling urgent and emergent patient concerns, is preferred.
BLS/ACLS certification.
Excellent clinical assessment, diagnostic, and procedural skills.
Strong communication and interpersonal skills, with the ability to manage complex patient interactions.
Ability to work independently and make critical decisions in a fast-paced, high-pressure environment.
Proficient in using electronic health records (EMR) and other medical technologies.

* Rounding- Surgical PA's round on general surgery and surgical oncology patients daily. This is done as a team-based approach with attendings, surgical residents, medical students and PA students

* Floor Procedures- Surgical PA's are responsible for floor procedures including NG tube and difficult foley insertion, wound vac changes, bedside I&D
This position is for three 12-hour shifts per week with one weekend shift per month. There is an extensive onboarding and training period as well as a supportive environment for collaboration and learning.

* The qualified candidate will be a graduate of an accredited Physician Assistant program and be NCCPA certified as well as possess strong surgical and patient management experience.
* Candidates with at least one to three years of surgical experience and/or surgical residency trained candidates preferred, but new graduates will be considered as will new nurse practitioners with RNF certification and surgical experience.

Summary of position
The Senior Psychiatrist is the psychiatric leader of the facility or program and works in close collaboration with the Mental Health Unit Chief, Clinical Supervisors, and Supervising Psychiatrists (if applicable) to ensure that safe, holistic, and evidence-based psychiatric care is provided to incarcerated patients with varying levels of mental health needs. A team-based approach to care is critical; one that incorporates all relevant mental health and medical disciplines as well as security staff. A major component of this position will be training, recruiting and retaining high quality staff and providing strong leadership and mentorship for existing staff.
Responsibilities Include (depending on location):
Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management.
In coordination with the facility-specific Mental Health Unit Chief, design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals.
Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. This supervision must include both
in vivo
reviews of clinical skills as well as chart review.

Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatrist will be matched with the population that most closely meets their clinical expertise, desired work location and schedule and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical.
Responsibilities Include:
Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs.
Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information.
Evaluate patients for the need for a higher level of care, including hospitalization, and refer to a higher level of care as clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care.
Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Document all activities in the electronic medical record in a timely, concise and clear manner.
Integrate nursing and medical concepts in planning care for medically complicated psychiatric patients.
Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication.
Counsel patients using advanced psychotherapeutic techniques, including in group settings if needed.

The Senior CL Psychiatrist will also consult on the care of medically complex patients in other jail facilities. Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatrist will be matched with the population that most closely meets their clinical expertise, desired work location and schedule and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical.
Responsibilities Include:
Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs.
Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information.
Evaluate patients for the need for a higher level of care, including hospitalization, and refer to a higher level of care as clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care.
Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Document all activities in the electronic medical record in a timely, concise and clear manner.
Integrate nursing and medical concepts in planning care for medically complicated psychiatric patients.
Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication.
Counsel patients using advanced psychotherapeutic techniques, including in group settings if needed.
Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics.
Engage in clinical supervision, as indicated, with the Supervising or Senior psychiatrist
Other duties as assigned by the Senior Psychiatrist or Mental Health Manager for the facility, Medical Director or Chief of Service for Psychiatry, or their respective designees.
Minimum Qualifications
1. Graduation from an approved medical school.
2. Completion of approved residency or fellowship in the specialty or sub-specialty and Board eligible or certified or Subboard eligible or certified.
3. Five years experience in field of specialty or subspecialty acceptable to the Medical Board of the Hospital.
4. Licensed to practice medicine in the State of New York.
Department Preferences
Possession of a valid license to practice medicine in the State of New York; and,
Completion of three years of approved residency training in psychiatry.
Additional Preferences
Demonstrate advanced knowledge of psychopathology in assessing patients in need of mental health treatment, including mental status examination and diagnosis.
Demonstrate advanced knowledge of psychopharmacology as it pertains to the patient population being treated and writing prescriptions.
Experience and ability working within a multidisciplinary team and in a variety of treatment settings.

Nurse Practitioner will be responsible for seeing patients independently for office visits as well as under the guidance of a supervising physician. This role requires a high level of clinical expertise, the ability to manage a fast-paced environment, and the flexibility to take both weekday and weekend call shifts to support patient needs. Key Responsibilities:
Conduct thorough patient assessments, including history taking, physical exams, and diagnostic evaluations related to urological conditions (e.g., urinary tract infections, prostate issues, kidney stones, bladder disorders, etc.).
Develop and implement individualized treatment plans in collaboration with supervising urologists.
Prescribe medications, perform minor procedures, and provide follow-up care for patients.
