Responsible for following the Prestige Care dress code, wearing a nametag at all times and identifies self to passengers routinely, and carrying a working cell phone at all times. 2. Responsible for carrying a current State Driver's License and know the location of insurance documentation, as well as fire extinguisher, road flares, emergency road equipment and cellular phone and demonstrate the proper usage of this equipment. 3. Responsible for familiarity with transportation of disabled persons and adhering to the Prestige policies and procedures regarding the transportation of resident in a safe manner including the use of appropriate seating restraints. 4. Responsible for immediate notification to Executive Director of any accident, damage, personal injury or behavioral incidents taking place during any trip. 5. Responsible for adhering to all local or state motor vehicle laws, rules and regulations. 6. Will greet residents, visitors, and staff courteously, respecting dignity and confidentiality standards and promote resident rights. 7. Responsible for completing the Prestige Vehicle Trip Log at time of each trip and for completing a weekly Vehicle Inspection form, with identified repairs or adjustments reported to the Executive Director for immediate resolution. Education & Experience you need to qualify: * High School diploma or equivalent. * A valid, current driver's license in the state. * Valid commercial Class C driver's license (CDL) w/passenger endorsement required. * Knowledgeable of the area streets and locations. * Must be able to meet insurance qualifications to operate company vehicles including passenger vans and bus. * Previous experience transporting residents/patients preferred. Responsibilities: Participate in the development of a written plan of care for each resident and review resident's medical and nursing treatments to ensure they are in accordance with the resident's care plan and wishes. Complete the planning, scheduling and revising of the MDS, including the implementation of CAA's and Triggers. Make daily rounds to ensure that all nursing personnel are performing their work assignments. Review nurses' notes to ensure that they are informative and descriptive of the nursing care being provided and the resident's response to care and wishes. Develop and maintain a good working rapport with all facility personnel to ensure that the needs of the resident are met. Participate in reviews of the discharge plans and prepare reports for the Care Plan Committee as directed. Delegate, train, evaluate and support RN, LPN and CNA personnel. Provide direct nursing care as necessary. Assist the Director of Nursing Services and fill in as needed. Participate in facility surveys by authorized government agencies. Make recommendations for new or changes in procedures, policies, methods, education, reference material and general nursing practices to ensure the highest level of quality patient care is given. Attend continuing education programs designed to keep you abreast of changes in your profession, and participate in/provide leadership for in-service training for nursing personnel. Review patient care plans for appropriate goals, problems, approaches, and revisions based on nursing needs. Ensure all personnel involved in providing care to the patients are aware of their care plans. Schedule and facilitate care plan conferences. Monitor and guide the completion of PPS and OBRA assessments. Manage the Utilization Review (UR) process. Oversee MAR's treatment, flow sheets and physicians orders. Participate in facility surveys by authorized government agencies. Qualifications: Must possess a nursing degree from an accredited college or university. Must possess a current, unencumbered, active license to practice as a registered nurse in this state. Must have a minimum of 1 year of experience as a supervisor in a healthcare setting. Must have training in rehabilitative and restorative nursing practices. MDS Certification is preferred. Knowledgeable and experienced in the RAI process, CAAs, RUG categories and MDS 3.0. Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to nursing care facilities. Must chart nurses' notes in an informative and descriptive manner that reflects the care provided to the resident and resident's response to care. Experience with Electronic Medical Records and computer documentation systems. Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
