Job Details:
Performs delivery of non-standard, standard and complicated treatment planning and techniques. Calibrates various therapy machines, measures instruments against national standards, and measures therapy beam data for treatment planning purposes. Leads and facilitates the troubleshooting process, works collaboratively with Medical Engineers. Operates and performs quality assurance (QA) on equipment as required.
Performs radiation surveys of patients exposed to radioactive sources; regularly performs radiation surveys of treatment rooms.
Enters and validates data in treatment planning systems. Assists with writing specifications and commissioning new therapy equipment and accessories; designs new techniques and ancillary equipment as required.
Rapidly adopts and champions implementation of new technology and techniques; serves as a technical resource, role model and/or preceptor for staff, and conducts in-services as assigned.
Assists in completion of competencies for other staff.
Assists with ensuring compliance with all state and federal regulating and accrediting bodies. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
Medical Physicist certification issued by the American Board of Medical Physics (ABMP) needs to be obtained within 4 years,
or
Medical Physicist (CMP-ABR) certification issued by The American Board of Radiology (ABR) needs to be obtained within 4 years,
or
Medical Physicist (CMP-CCPM) certification issued by The Canadian College of Physicists in Medicine (CCPM) needs to be obtained within 4 years.
Education Required:
Master's Degree in Medical Physics
Knowledge, Skills & Abilities Required:
• Completion of a CAMPEP-accredited medical physics residency program is required.
• Knowledge of treatment units and simulators and awareness of treatment unit's limitations in clinical situations.
• Knowledge of record and verify and radiation oncology electronic medical record systems.
• Knowledge and ability to learn and apply complex radiation therapy principles.
• Proficient with general radiation equipment.
• Excellent interpersonal skills and the ability to communicate effectively with physicians, staff, patients and their families.
• Excellent problem solving abilities and analytical skills to compare data.
Physical Requirements and Working Conditions:
Must lift up to 20 lbs. frequently and up to 50 lbs. occasionally. Must be able to sit 50% of workday.
May push up to 500 pounds, 50 to 200 feet.
Exposed to mechanical, electrical, chemical and radiation hazards.
Operates all equipment necessary to perform the job.

The CAP Case Manager provides critical case management services to beneficiaries who are at risk of institutionalization, ensuring their health, safety, and well-being are maintained through person-centered care planning and coordination of essential services.
The Case Manager works closely with families, RNs, and other interdisciplinary team members to assess needs, coordinate care, and provide ongoing support to help beneficiaries achieve the best possible quality of life.
DUTIES AND RESPONSIBILITIES:
Assessment and Care Planning:
Conduct initial pre-screening and assessments of beneficiaries and their families to evaluate medical, psychological, behavioral, financial, social, cultural, environmental, legal, vocational, educational, and other areas.
Consultation with the CAP beneficiary and primary caregiver to educate about waiver services, other Medicaid, and community resources to meet the beneficiaries' needs.
Identify needs to prevent health and safety factors to assist in maintaining community placement.
Develop and maintain individualized, person-centered care plans (emergency and disaster planning) to ensure the health, safety, and well-being of beneficiaries.
Review and update care plans at least every 12 months or when the status of the beneficiary changes.
Assist beneficiaries and families in understanding the plan of care and making informed choices.
Coordination of Services:
Link beneficiaries and their families to necessary services, equipment, and supplies to support care in the home.
Collaborate with community resources, healthcare providers, and other agencies to ensure comprehensive care delivery.
Initiate appropriate referrals and utilize community resources for planning and service coordination.
Monitoring and Follow-Up:
Provide ongoing monitoring of services through monthly phone calls and home visits, documenting observations, and beneficiary progress.
Evaluate the effectiveness of care plans and services, recommending or implementing changes as needed to achieve desired outcomes.
Maintain accurate, up-to-date case management documentation within the system, ensuring compliance with state and agency guidelines.
Counseling and Support:
Provide emotional support and basic counseling to beneficiaries and their families to strengthen their support systems.
Assist families in navigating challenges, including long-term palliative care, behavioral issues, and medical needs.
Documentation and Compliance:
Assist in obtaining documentation from medical staff to confirm the need for specific CAP services.
Maintain medical records for each beneficiary, ensuring documentation of current status, service changes, and referrals.
Ensure compliance with 10A NCAC 27G.0202 and other regulatory guidelines.
Complete all required records per agency policy and the State CAP manual, including discharge summaries when CAP services are completed.
Review and ensure proper billing codes and compliance for case management, in-home aide documentation, paid live-in caregiver, re-certifications, and supply billing.
Participate in NC Medicaid-certified training programs and ensure program compliance within 90 days of employment.
Collaboration and Advocacy:
Work closely with RNs and interdisciplinary team members to ensure a comprehensive approach to beneficiary care.
Participate in case discussions and provide input to ensure quality care and service delivery.
Advocate for program participants and their families to secure necessary resources and services.
Serve as a liaison between beneficiaries, families, and external providers to address care needs effectively.
Provide training and support to families to empower them in managing their child's care.
Continuing Education and Professional Development:
Complete all state-mandated training and agency-required continuing education annually.
Stay current on CAP guidelines, best practices, and new developments to enhance service delivery.
Ensure timely updates to the CAP Business system and other documentation systems as required.

SUPERVISORY RESPONSIBILITIES:
This position has no supervisory responsibilities.
MINIMUM QUALIFICATIONS:
Strong understanding of Medicaid programs and compliance requirements.
Excellent communication and interpersonal skills to work effectively with families and multidisciplinary teams.
Ability to manage multiple priorities and maintain detailed records.
Experience working with medically fragile children or in pediatric healthcare settings and physically disabled adults with complex care needs.
Education and Experience:
Candidates must meet one of the following criteria:
Social Work Background:
Bachelor's degree in social work from an accredited school of social work.
Minimum of one year of directly related community experience, preferably case management, in the health or medical field (homecare, long-term care, or personal care).

Human Services Background:
Bachelor's degree in a human services or equivalent field from an accredited college or university.

Non-Human Services Background:

Performs a variety of administrative, training and personnel functions. DUTIES AND RESPONSIBILITIES:
Develops and maintains a system for assuring integration of basic developmental care and programming services.
Supervises and coordinates activities of Direct Support employees and manages operations, staffing and budget of assigned facilities.
Interviews and hires Direct Support Professionals
Assists with creating employee schedules ensuring that there is always adequate staffing coverage.
Works closely with clinical and professional healthcare staff to maintain communication and provide feedback, standardizing procedures, and expediting the implementation of person-centered plans.
Provides shift coverage in times of acute shortages of direct support staff. Provides on call coverage for people supported they support and services as needed.
Provides after-hours and on-call coverage as part of the direct care rotation
Works closely with Qualified Professional and other clinical/professional staff to maintain communication and provide feedback, standardize procedures, expedite Person Centered Plan implementation and workflow and to improve efficiency of team
Partners with the Worker's Compensation team to ensure prompt reporting and investigation of employee injuries, ensures employee receive needed safety training and coaches employees not working safely and properly
Periodically monitors all shifts and basic developmental care to ensure that legislative standards are met. Constantly monitors activities to ensure that health and sanitary standards are met; corrects any deficiencies noted by internal/external authorities.
Assists in the Interdisciplinary Team Process, as needed. Participates in the Quality Assurance Assessment system which may include; Mealtime Assessment, Environmental Assessment; Interaction Assessment and Third Shift Assessment.
Interviews, hires, coaches and terminates employees as needed.
All other duties as assigned.

This position supervises Group Home Supervisors and Direct Support Professionals
MINIMUM QUALIFICATIONS:
A high school diploma or equivalent
One year of experience working with people with developmental disabilities.
Excellent verbal and written communication skills.

CPR, first aid, NCI and/or MedTech certification, a plus
Must be at least 18 years of age

The CAP Case Manager provides critical case management services to beneficiaries who are at risk of institutionalization, ensuring their health, safety, and well-being are maintained through person-centered care planning and coordination of essential services.
The Case Manager works closely with families, RNs, and other interdisciplinary team members to assess needs, coordinate care, and provide ongoing support to help beneficiaries achieve the best possible quality of life.
DUTIES AND RESPONSIBILITIES:
Assessment and Care Planning:
Conduct initial pre-screening and assessments of beneficiaries and their families to evaluate medical, psychological, behavioral, financial, social, cultural, environmental, legal, vocational, educational, and other areas.
Consultation with the CAP beneficiary and primary caregiver to educate about waiver services, other Medicaid, and community resources to meet the beneficiaries' needs.
Identify needs to prevent health and safety factors to assist in maintaining community placement.
Develop and maintain individualized, person-centered care plans (emergency and disaster planning) to ensure the health, safety, and well-being of beneficiaries.
Review and update care plans at least every 12 months or when the status of the beneficiary changes.
Assist beneficiaries and families in understanding the plan of care and making informed choices.
Coordination of Services:
Link beneficiaries and their families to necessary services, equipment, and supplies to support care in the home.
Collaborate with community resources, healthcare providers, and other agencies to ensure comprehensive care delivery.
Initiate appropriate referrals and utilize community resources for planning and service coordination.
Monitoring and Follow-Up:
Provide ongoing monitoring of services through monthly phone calls and home visits, documenting observations, and beneficiary progress.
Evaluate the effectiveness of care plans and services, recommending or implementing changes as needed to achieve desired outcomes.
Maintain accurate, up-to-date case management documentation within the system, ensuring compliance with state and agency guidelines.
Counseling and Support:
Provide emotional support and basic counseling to beneficiaries and their families to strengthen their support systems.
Assist families in navigating challenges, including long-term palliative care, behavioral issues, and medical needs.
Documentation and Compliance:
Assist in obtaining documentation from medical staff to confirm the need for specific CAP services.
Maintain medical records for each beneficiary, ensuring documentation of current status, service changes, and referrals.
Ensure compliance with 10A NCAC 27G.0202 and other regulatory guidelines.
Complete all required records per agency policy and the State CAP manual, including discharge summaries when CAP services are completed.
Review and ensure proper billing codes and compliance for case management, in-home aide documentation, paid live-in caregiver, re-certifications, and supply billing.
Participate in NC Medicaid-certified training programs and ensure program compliance within 90 days of employment.
Collaboration and Advocacy:
Work closely with RNs and interdisciplinary team members to ensure a comprehensive approach to beneficiary care.
Participate in case discussions and provide input to ensure quality care and service delivery.
Advocate for program participants and their families to secure necessary resources and services.
Serve as a liaison between beneficiaries, families, and external providers to address care needs effectively.
Provide training and support to families to empower them in managing their child's care.
Continuing Education and Professional Development:
Complete all state-mandated training and agency-required continuing education annually.
Stay current on CAP guidelines, best practices, and new developments to enhance service delivery.
Ensure timely updates to the CAP Business system and other documentation systems as required.

MINIMUM QUALIFICATIONS:
Strong understanding of Medicaid programs and compliance requirements.

Job Responsibilities:
Perform procedures in accordance with departmental protocols and standards. Operates MRI Scanners and associated equipment with a high level of proficiency.
This equipment includes scanner, post processing stations/programs, IV contrast injectors, O2 and suction equipment, patient transport and patient monitoring equipment.
Essential Functions: Examines requests and verifies orders on each assigned patient. Refers and questions orders to the Radiologist before performing exam.
Contacts ordering provider when there are discrepancies. Properly identify (Using two patient identifiers) and assist patients while offering a brief explanation of the procedures.
Prepares patients for the procedure with a full explanation and documents on appropriate worksheets.
Interviews patients for a complete medical history. Reviews in-patients, ER, and outpatients EMR's to obtain pertinent clinical history. Records medical history on requisitions.
Correlates other test results and laboratory data with exam being performed. Assumes responsibility for the exam from beginning of exam until completion of dictated results.
Prepares and administers IV contrast according to departmental protocols. On request, records contrast type and amount. Evaluates technical quality of images and consults with a Radiologist if needed. Recognizes abnormal anatomy and processes any additional, reconstructed images, and/or measurements as instructed.
Performs basic patient care functions. Informs appropriate nursing personnel of any changes in the patient's condition. Initiates life support measures for patients when necessary.
Assists in “code blue” procedures during medical emergencies. Performs MRI scanning and assists Physicians/PA during invasive procedures. Practices proper sterile technique in the preparation of necessary supplies.
Performs all invasive procedures in accordance with bloodborne pathogens, isolation, and infection control policies and procedures. Is authorized to obtain medication or contrast material as directed for administration by a licensed practitioner.
Practices principle of radiation safety for self, employees, patients and family members. Follows appropriate radiation safety standards Reports malfunctioning equipment to the department director/manager/supervisor and clinical engineering/radiology engineer.
Maintains knowledge and skills required to work safely in an environment that can create significant exposure to bloodborne pathogens and infectious disease.
Adheres to the hospital fire and safety procedures. Attends all in-service classes and completes all required safety modules. Operates radiology computer system to include entering orders and charging procedures/supplies.
Documents contrast administration in the MAR. Maintains assigned work area in a neat and orderly fashion. Keeps area completely stocked. Cleans all work areas to promote cleanliness in the workplace. Assists other medical personnel, such as anesthesia, when performing exams or services in conjunction with a MRI scan.
Maintains knowledge of all MRI safety requirements for both staff and patients while in and around magnetic fields.
Licensure, Registration, and/or Certification Required:
Graduate of an accredited two-year AMA approved program in Radiologic or Nuclear Medicine Technology or active enrollment in a ARMRIT or ARRT registry eligible program required.
ARRT (MR) required. BLS (AHA or ARC) required.
Physical Requirements and Working Conditions:
Requires moderate to heavy physical efforts in lifting and moving patients, equipment and supplies. Occasional standing, walking, and sitting during normal operation. Frequently may be required to stretch, bend twist, squat or kneel in the daily operation of the MRI department. May be exposed to various physical conditions throughout the day such as walking on hard surfaces, climbing stairs, being near moving equipment. Required to use radiation safety guidelines to prevent exposure to ionizing radiation.

Education, Licensure, and Experience required for the position include: High school diploma or G.E.D. required. Four-year degree in a human service field and one year of experience with the population served preferred.

RHA Health Services is looking for a driven, resourceful and compassionate Residential Habilitation Specialist to join our interdisciplinary healthcare team focused on creating, implementing and supporting programs aimed at helping clients achieve personal outcomes, by mentoring and modeling personal outcome programs for clients and home care staff.
In this dynamic role, you will work closely with the interdisciplinary team writing evaluations, training programs, program notes, and training home care staff in program implementation procedures.

Job Responsibilities
As a Habilitation Specialist, you will be responsible for conducting Quality Assurance assessments for mealtime, active treatment, and formal programs, focusing on areas of adaptive behavior including self-help, daily living, community living, education, socialization, pre-vocational, vocational and recreation.
Additional responsibilities of the Residential Habilitation Specialist include:
Evaluating and assessing each client's current level of adaptive behavior, areas of self-help, daily living, personal safety, community living, academics, pre-vocational and vocational skills upon admission and updates annually
Developing personal outcome programs based on priority objectives identified by the interdisciplinary team
Specifying behavioral objectives, training methods, reinforcers, criteria, data collection method, and task analysis or training steps for each program; writing program and implementing within required timelines
Observing and providing feedback while working closely with home care staff as they carry out personal outcome program procedures; providing encouragement and offering suggestions for fine-tuning intent of programs and overall support plan
Reviewing and documenting formal programs on 30-day cycles to modify as appropriate and conducting in-service training
Conducting Personal Outcome Interviews along with other staff members to assist in identifying outcomes and supports
Additional requirements for the Residential Habilitation Specialist role include:
Bachelor's Degree in Social Work, Social Services, Special Education, or Psychology or related Human Services degree
1 year of experience working with persons with intellectual and developmental disabilities
Valid drivers' license and automotive insurance
Ability to read and interpret professional journals, technical procedures, and governmental regulations
Ability to write reports and business correspondence
Ability to pass background check and drug screen
CPR, first aid, NCI and/or role specific certification, a plus
Successful candidates for the Residential Habilitation Specialist are able to utilize a variety of assessment tools to develop habilitation programs for clients who usually have multiple developmental, physical, or behavior problems. Someone who displays communication skills that foster teamwork would be a good fit for this role
