The physician will provide direct clinical services and structured supervision for up to three APPs, helping to enhance the consistency, safety, and effectiveness of patient care across the team.
Core Responsibilities:
Provide direct patient care in a clinical setting, focusing on evidence-based obesity treatment and chronic weight management. The physician will collaborate with APP colleagues to co-manage a shared panel and not have their own primary patient panel.
Review charts and conduct return visits for APP patients every third visit to ensure quality and continuity of care.
Provide treatment planning and medical management support for complex cases or escalated concerns.
Prescribe non-controlled obesity medications for patients under both direct care and in coordination with APPs.
Conduct weekly 30 minute collaborative meetings and 30 min asynchronous communication time with each assigned APP to review cases, address clinical challenges, and provide ongoing mentorship.
Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team.
Assist in the professional development of APPs by fostering clinical growth, offering feedback, and supporting continuing education.
Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care.
Attend relevant departmental meetings and contribute to quality improvement efforts within the obesity medicine program.
We seek applicants who:
Have an MD or DO degree, are board certified and diplomates of the American Board of Obesity Medicine (ABOM)
Minimum of 5 years as an attending physician in primary specialty

Responsibilities:
Provide full-spectrum family medicine care, including preventive care, chronic disease management, acute care, and women's health
Deliver obstetric care including prenatal, intrapartum, and postpartum care
Offer comprehensive, patient- and family-centered care.
Ensure consistent, cost-effective care.
Provide inpatient obstetric and newborn care as part of a call pool with other physicians.
For surgical obstetric fellowship trained family physicians: perform scheduled Cesarean deliveries and bilateral tubal ligations
Provide patient care in a holistic manner including attention to physical, mental, emotional, and spiritual health.
Maintain electronic medical records efficiently and maximize resources.
Collaborate with interdisciplinary clinical and program teams at Jericho Road Community Health Center.
Create a learning plan that includes familiarity with new research and changes the standard of practice.
Participate in quality improvement/population health activities with a data-driven approach. Participate in regular peer reviews and risk management activities as assigned.
Attend required staff and clinician meetings except when clinical activities prevent such attendance.
Arrange to ensure that mandatory and required training is completed
Serve as a clinical preceptor or mentor to students and residents, as applicable
Qualifications:
Doctor of Medicine or Doctor of Osteopathy, licensed to practice in New York State prior to start date of employment

Job Summary
