AgeWell Medical Associates is a primary care practice that specializes in providing comprehensive medical care to seniors aged 60 and above. AgeWell Medical Associates consists of two physicians, four APP's and a psychologist providing on site geriatric behavioral health support. The practice caters to seniors with varying health conditions, ranging from those who are well to those with multiple chronic conditions. Patient-centered care is the practice's north star, with the goal of helping patients achieve their health goals. Our practice is a member of the independent practice association (IPA), PHPprime. PHPprime provides wrap-around services to help make our practice successful, including care coordination, practice transformation coaching, risk adjustment coding education, population health management, data and analytics, specialist network management, and large group contracting.
