Internal Medicine - Geriatrics Physician
Signify Health 3.5
Primary care physician job in Illinois
Are you interested in working flexible hours, connecting with people one-on-one, and helping them re-engage with their health? At Signify Health, part of CVS Health , our mission is to build trusted relationships to make people healthier.
We re hiring physician (MD/DO) contractors to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.
These visits include a medication and medical history review, as well as a physical evaluation performed in a health plan member s home. Our physician contractors can earn a competitive wage (or supplement their current income) while managing other personal and clinical commitments. You will be provided a pre-populated, patient-specific form for each evaluation, and do not prescribe medicine, order lab tests, or alter people s current treatment regimen.
This is an excellent opportunity for resident physicians to earn additional income or for more seasoned physicians seeking a change of pace.
Making a Difference - what physicians say about working with Signify Health
Many of the people I see are shocked a physician is coming to their home to visit with them one-on-one. When you sit down and have an unhurried conversation about their health, they are impressed, happy, and understand that their health plan and care team cares about them. - Marcie L., M.D., Arizona
The primary carephysician gets a snapshot of a person in an office setting, but with Signify Health, we see things like the number of steps to enter their home, the state of disrepair those steps may be in, and the person s ability to function comfortably and with ease in their own home. We can make referrals to the individual s health plan, encourage the participation of their loved ones, and ensure their PCPs are aware of their health and overall quality of life. - Cathy H., M.D., Kentucky
Incentive rates are subject to change. Area exclusions apply.

About Medicus Healthcare Solutions
We serve as a vital connector between physicians, advanced practitioners, healthcare facilities, and the patients they serve. In healthcare, seconds and minutes can be critical, and it's imperative that we get the right people in the right place at the right time. We are creative problem solvers, and we are committed to continuous improvement. We believe potential is all about your perspective. We make the impossible, possible. Since 2004, our mission has been to provide innovative solutions for the gap that exists between patient demand and healthcare services. As the healthcare needs of our population increase, the availability of medical providers continues to decline. Our clients, physicians, and advanced practitioners rely on us so that they can care for the communities that rely on them. There is no greater responsibility or reward.
