Hospitalist Program: Scope of Activities Clinical Activities The group's current clinical activities include: Teaching services, direct care services, consultative activities, co-management services, complex care services, , the intermediate medical care and a nocturnist service. While there is dedicated nocturnist support, shifts include nights, weekends and holidays. The division employs support staff including a hospitalist administrator, administrative manager, administrative coordinators, medical office coordinators and program specialists, and also employs moonlighters (faculty, residents and fellows) as needed to support some clinical activities. Key Responsibilities: Patient Care: Provide comprehensive medical care for hospitalized patients, including admission, diagnosis, treatment, and discharge planning. Conduct daily rounds to assess patients' conditions and update their treatment plans. Make timely, evidence-based decisions regarding patient care and treatment plans. Respond to clinical emergencies and ensure the safety and comfort of patients. At all times interact with patients and staff in accordance with CCHS Standards of Behavior and Code of Ethics Complete patients encounter medical record and complete tasks in a timely manner to include dropping charges on day of service, completing tasks within 48 hours, addressing requests for prescription renewal within 72 hours and completing and signing all patient encounters within 72 hours of the date of service. Make appropriate referrals to specialists, subspecialties, hospital emergency rooms, hospital clinics, etc., when good medical judgment dictates such referrals. Provide first-aid to patients but may refer serious injuries or severely acute conditions requiring hospital care to local hospital emergency rooms or in-patient facilities, as appropriate, after stabilizing the patient as clinically necessary at the health center. Ensure that standards of quality evidenced based and best practice medical care are met. Participate in medical audits, performance improvement, and peer review activities, and participate in appropriate CCHS committees, when necessary. Provide in-service training sessions for other CCHS staff, as necessary. Attend CCHS staff meetings unless direct patient duties prevent such attendance. Serve as a resource and consultant to the advanced practice clinicians, and review problem cases with the advanced practice clinicians as needed. When assigned by the Medical Director, audit advanced practice clinicians' charts and help assure that the advanced practice clinicians are providing care within generally accepted current standards. Be available to the advanced practice clinicians for questions and consultations when working in Health Center. Take night calls and weekend calls in accordance with CCHS policies and procedures. Must obtain and maintain appropriate licensing, credentialing, and privileges according to CCHS policies and procedures. Complete required CME yearly. Ensure proper and current charting in the medical records. Participate in educational programs and classes for patients and staff. Participate in "special" physical examination sessions, or extra medical sessions as needed by CCHS patients and arranged by CCHS. Participate in Performance Improvement, Risk Management, and Peer Review activities at CCHS. This includes peer review audits, chart reviews, etc. Coordinated Care/Patient Centered Medical Care Management: Works with all members of the Health Care Team towards achieving coordinated patient centered care. Pay Rate $140.00/hour Call ************ to discuss position details and compensation!Or email your CV for immediate review to: ****************************** Key Responsibilities Perform a full range of physician services in accordance with privileges granted by the MTF, as a Primary Care of Family Physician on an interdisciplinary evaluation team; and provides treatment planning for warriors and beneficiaries with mild TBI and psychological health (PH) health issues. Conducts the initial medical intake evaluation, provides medical oversight for the beneficiaries, and works with referring providers at the patient's home town to coordinate long term and follow-on treatment plan. Coordinates with the care team, discusses matters pertaining to personnel, prioritization of projects and equipment, clinic policies and any phase of the clinical service program connected to TBI or PH evaluation, research and treatment planning. Examine, diagnose, treat or prescribe courses of treatment within the scope of training, experience, and privileges. Provide preventive and health maintenance care, including annual physicals, positive health behaviors, and self-care skills through education and counseling. Order diagnostic tests as applicable as well as request consultation or referral with appropriate physicians, clinics, or other health resources as indicated. Technically proficient in directing and teaching other medical staff, providing educational lectures and participating in the provision of in-service training to clinic staff members. Such direction and interaction will adhere to Government and professional clinical standards and accepted clinical protocols. Serve as physician liaison to a referral management coordination call center, in support of care allocation for clinical referrals. Skills, Knowledge and Expertise Degree/Education: Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.). Graduate from an accredited medical school in the United States or Canada. A Doctor of Medicine or equivalent degree from a foreign medical school must provide education and medical knowledge substantially equivalent to accredited schools in the United States and a CGFNS report to confirm. Residency: Successful completion of a residency program in Primary Care or Family Medicine Internship: Successful completion of program with broadly based clinical practice Board certified in Primary Care or Family Medicine Experience: At least two years of experience providing direct patient care in a clinical setting within the past five years with privileges in primary care or family medicine.
