Total Compensation Package (base pay + bonus) could exceed $300k, depending on experience and location.
This full-time position will be located at our East Port Orange center.
Responsibilities:
Evaluates and treats center patients in accordance with standards of care.
Follows level of medical care and quality for patients and monitors care using available data and chart reviews.
Assists in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment and home health care.
Acts as anactive participant and key source of medical expertise with the care team through daily huddles.
Helps RegionalMedical Director (RMD) and Center Administrator (CA) in setting a tone of cooperationin practice by displaying a professional and approachable demeanor.
Completes allmedical record documentation in a timely manner working with a quality- based coder to optimize coding specificity.
Follows policy and protocol defined by Clinical Leadership.
Meets with RMD about quality of care,review of outcome data,policy, procedure and records issues.
Participates in potential growth opportunities for new or existing services within the center.
Participates in the local primary care on-call program of Conviva as needed.
Assures personal compliance with licensing, certification, and accrediting bodies.
Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care.
Required Qualifications:
Current and unrestricted medical licenseorwilling to obtain a medical license instate of practice (FL)
Graduate of accredited MD or DO program of accredited university
Board Certification or Board Eligible in Family Medicine or Internal Medicine (Geriatric Medicine preferred)
Active and unrestricted DEA license
Excellent verbal and written communication skills
Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients
Fully engaged in the concept of integrated team-based caremodel
Willingness and ability to learn/adapt to practice in a value-based care setting
Superior patient/customer service
Basic computer skills, including email and EMR
This role is considered patient facing and is a part of our Tuberculosis (TB) screening program. If selected for this role, you will be requested to be screened for TB
Preferred Qualifications:
Minimum of two to five years directly applicable experiencepreferred
Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment
Knowledge of Medicare guidelines and coverage
Knowledge of HEDIS quality indicators
Bilingual (English and Spanish) is a plus

Professional responsibilities for this position include about 40% inpatient care, 30% OR, and 30% clinic. You would provide perioperative care to vascular surgery patients in the ICU, stepdown, and regular surgical floor settings in close collaboration with the surgeons. This includes daily hospital rounds, admission H&P?s, discharges, and consults. PA?s first-assist in all vascular surgical cases. Our surgeons perform a wide range of vascular surgeries, including EVAR (including fenestrated, snorkel, and chimney techniques), TEVAR, open aneurysm repair, CEA, TCAR (transcarotid artery revascularization), lower extremity bypasses, dialysis access, amputations, vascular traumas and emergencies, as well as endovascular procedures for arterial and venous diseases. You would also see outpatients in the clinic one week per month. The Vascular Surgery PA acts as the liaison between the surgeons, consultants, hospital staff, and patients to ensure continuity and highest quality patient care. The PA would need to have a team-focused mindset and excellent communication and collaboration skills.

Physician Assistant
The duties of a Physician Assistant include, but are not limited to: providing accurate assessment, diagnosis, treatment, consultation, education and follow-up care to patients ranging in age from infant to geriatric, proper maintenance of records, and any other reasonable and proper duties assigned within approved Scope of Practice and under the direction of the supervisory Physician.
PRIMARY FUNCTIONS
Examine patients and record history
Adhere to proper charting protocols
Discuss with Physician and other health care professionals to prepare comprehensive plan
Order or execute tests or images as needed
Submit health care plan and goals of individual patients for periodic review by Physician
Analyze results of tests, diagnose and report
Administer or prescribe treatment and medications
Promote health by advising patient of diet, hygiene and prevention of disease
Inoculate and vaccinate as required
Refer to specialist as required for patient's well-being
Perform minor surgery
Report deaths and contagious diseases to governmental authorities
Other responsibilities as assigned
EDUCATION AND EXPERIENCE
Must have and maintain current State of Florida license in good standing
Current National Board Certification
KNOWLEDGE, SKILLS AND ABILITIES
Effective written, verbal and electronic communication skills
Computer competency
