Wound Care Physician - South Sacramento
The Permanente Medical Group is actively seeking a Board Certified/Eligible physician who is experienced in wound care for our South Sacramento facility. The ideal candidate will have experience within an outpatient wound care clinic or academic setting and preferably, but not mandatory, with surgical training, hyperbaric medicine and/or vascular medicine training.
Procedures are a part of daily work and include debridement, hematoma evacuation, application of biologic dressings, biopsies, etc. Based on skill set it may also include operative debridement and skin grafting. You will also provide supervision of the wound and ostomy nurses. There is potential for administrative work if desired.
This position will be based out of the Kaiser South Sacramento Medical Facility but will be part of our Greater Sacramento Valley Wound and Ostomy service. The provider will spend time at our Sacramento and Roseville campuses as well. You will work with a team of certified wound and ostomy nurses who function in an extended role at each site and will have medical assistant support at each site as well.
Hours will be Monday-Friday 8:00am to 5:00pm with an hour lunch daily. Currently, there is no call required and there is potential for academic affiliation and education of Family Practice residents through our new Family Practice residency program. Research opportunities are available if interested.
As a member of one of the largest medical groups in the country you will collaborate closely with our Podiatric wound care providers, vascular surgery, endocrinology, orthopedics, plastics, nutrition, and social services. This position is a shareholder track position with the opportunity for full partnership after three years of service.
Full-time annual salary range is $320,040 to $330,000 plus additional potential incentives up to $44,350*. Reduced schedules with pro-rated compensation may be available. *Some incentive opportunities are estimates based on potential premium pay.

Role Scope:
We are seeking an outstanding physician who is highly motivated and passionate about improving the quality of care for vulnerable adults. Under the supervision of the Medical Director, this physician will provide primary medical care to participants in the Habitat Health program, in collaboration with an Interdisciplinary Team (“IDT”).
Core Responsibilities & Expectations for the Role
Creates a center experience that Participants want to spend time in, and team members love.
Establishes and honors Habitat values within the team.
Performs an initial comprehensive history and physical assessment on new enrollees, interval assessments on established participants (semi-annually, as condition necessitates or as requested), and documents appropriately in the medical record as required.
Provides ongoing and longitudinal primary care using evidence-based practice; is an active member of the IDT, communicates participant changes to team members, and participates in IDT meetings. Provides updates to the IDT on a regular basis.
Assists in the development, implementation, and evaluation of the participant plan of care as described in PACE policy. Develops care planning goals based on the expressed wishes and preferences of participants within the context of their medical and psychosocial conditions.
Evaluates and treats participants with episodic illnesses and documents in the medical record as required by policy.
Provides, promotes, and educates participants and families on health maintenance of their chronic medical conditions and actively participates in family conferences.
Collaborates with other providers and facilitates consultations with medical specialists as indicated and oversees care rendered.
Communicates with attending physicians when participants are hospitalized or admitted to a skilled nursing facility (“SNF”). and advocates for care that is consistent with the participant's expressed wishes and values.
Coordinates discharge planning with the IDT and hospital discharge planners.
Participates in committees, work groups, and Quality Improvement activities as needed.
Participates in the coordination of the participant's 24-hour delivery of care services across settings.
Maintains current accurate documentation of diagnoses, health care services provided, coordinated, or contracted.
Promptly addresses inquiries from medical records staff and coding specialists.
Participates in a 24-hour on-call rotation with other Primary Care Providers.
Evaluates the effectiveness of the participant's plan of care and revises as appropriate.
Performs functional status evaluation and/or mental status examinations using standardized procedures.
Works as a supervising physician with the nurse practitioner or clinical nurse specialist.
Prepares and submits reports including services provided as required.
Protects privacy and maintains confidentiality of employee, participant, and other sensitive information.
Performs related duties as assigned.
Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
