    The OBGYN Staff Physician is responsible for comprehensive care to patients under the direction of the CMO or Medical Director per medical protocols. The position is responsible for providing comprehensive obstetric and gynecologic care to patients, including preventive health services, prenatal and postnatal care, labor and delivery, and the diagnosis and treatment of reproductive health conditions. This role involves performing clinical assessments, procedures, and surgeries based on role, as well as collaborating with other healthcare professionals to ensure the highest standard of patient care. This position will collaborate with Nurse Practitioners, PA's and Teaching Assistants (TA). Responsibilities: · Interview patients, obtain medical history, update all information in EHR according to standards. · Provide patient medication information for all new prescriptions according to standards. · Provide comprehensive medical care to women across their lifespan, including adolescents, adults, and seniors. · Diagnose, treat, and manage conditions related to female reproductive health, such as reproductive system disorders, menstrual conditions, infertility, and menopause-related issues. · Perform routine gynecological exams (Pap smears, pelvic exams, breast exams). · Provide prenatal, intrapartum (labor & delivery), and postpartum care. · Manage both low-risk and high-risk pregnancies. · Counsel patients on reproductive health, contraception, family planning, and sexual health. · Perform obstetric procedures. · Educate patients on preventive care, healthy lifestyle choices, and disease risk reduction. · Provide guidance on breastfeeding, newborn care, and maternal mental health. · Stay current with advancements in women's health through continuing medical education. · Maintain accurate medical records and documentation. · Participate in quality improvement initiatives and peer reviews. · Conduct gynecological exams, screenings, and wellness visits. · Maintain accurate and timely medical documentation in compliance with regulatory and organizational standards. · Stay current with continuing medical education, licensing, and board certification requirements. · Establish and maintain collaborative relationships with healthcare colleagues. · Manage and maintain a comprehensive health program for patients including preventative medicine and acute chronic conditions. · Prepare and review medical histories and obtain data through interviews. · Examine patients, formulate diagnostic plans, and order required dialogistic testing. · Interpret examination findings and test results and implement treatment plans. · Prescribe and address effectiveness of pharmaceuticals, other medications, and treatment regimens as appropriate for patient medical conditions. · Determine need for consultation and assist in medical care and treatment provided at the direction of other specialists. · Participate in other care procedures according to training and demonstrated ability. · Actively participate in patient care by supervising assigned medical staff, nurses, and other medical technicians as needed. · Attend and actively participate in departmental meetings. · Determine needs for and advise on the type and quality of necessary medical supplies and equipment if applicable. · Care management of high-risk patients to reduce risk, decrease hospital, and ER utilization, and improve outcomes. · Maintenance of a care management registry for documentation of high-risk patients, care management interventions and care plan. · Timely and ongoing communication with the healthcare team to identify high risk patients and to maximize the management of patient needs and related risk reduction. Experience 4+ years of practicing medicine in a clinic, hospital, or private practice preferred. Adept at diagnosing problems and creating treatment plans for patients. Excels in communicating sensitive and adverse information. Sound judgment and expert decision-making abilities. Analytical thinker and complex problem solver. Working knowledge of FH clinical specific EMR - Centricity or EPIC. Technological competency skills are mandatory.

The Primary Care Physician (PCP) in our organization demonstrates: • Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients. They understand that they can strongly influence the patient's outcomes by building a trusting relationship and helping them change behaviors. • Coaching for health: The PCP acts as a health coach, rather than just a consultant for sickness, by helping patients set short and long-term health goals, partners with the patient to work toward the goals, and frequently follows up on those goals on the path to improved health for their patients. • Simplifying for action: The PCP simplifies and prioritizes appropriately so that behavior change is more actionable, both for the patient in helping them achieve their goals, and when leading their care teams towards their performance goals. We are an outcomes-focused, value-based organization and for their panel of patients, the following metrics are regularly measured to help PCP's become successful and reach partnership status: patient admissions/thousand, using between 18-21 appointment slots per day (each new patient count for 2 slots, follow-up patients 1 slot), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures. Each PCP will have goals for these metrics and will be expected to work towards those targets with their center and market leadership as well as their care teams. Culture is very important in the medical centers and because PCP's are leaders in our organization and centers, they are expected to help champion a positive culture of love, accountability, and passion along with center leadership. The PCP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The PCP will work closely with the applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals. The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: Independently assesses acute and non-acute clinical problems. Performs and documents physical assessments and patient histories, analyzes trends in patient conditions, and develops, documents and implements a patient management plan in response to the data obtained. This also includes assisting in the development of the nursing plan of care in addition to providing appropriate patient/ family/significant other counseling and education. Plans patient care based on in-depth knowledge of the specific patient population and/ or protocol, anticipating and identifying physiological and/ or psychological problems commonly encountered including the consideration of the patient's cultural background, level of understanding, personality and support systems. Serves as patient advocate. Patient management includes the following: Writes admission, transfers and discharges orders. Orders and interprets appropriate laboratory and diagnostic studies. Orders of appropriate medication and treatments. Refers patients for consultation when indicated i.e. dermatology, neurology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, psychiatry, social service, dietary, etc. Documentation through in-depth progress notes and summaries. May perform invasive procedures independently upon the completion of documented competency. Participates in patient care rounds and conferences. Communicates patient management strategies to members of the patient care team. Collaborates with members of the multidisciplinary team to ensure that patient management strategies are successful in meeting patient care needs. Recognizes situations which require the immediate attention of a physician, and initiates life-saving procedures when necessary. Uses advanced communication skills to problem solve complex situations and to improve processes and service to patients. Other Responsibilities may include: Collaborates with other multidisciplinary team members to analyze and evaluate current systems of health care delivery to identify and implement new practice patterns as appropriate. Participates in outside activities that enhance personal and professional growth and development. Initiates arrangements and writes orders for SNF discharges and completes appropriate paperwork. Works collaboratively with physicians, nurses, PT, social workers, family and key caregivers to transition the patient to a lower level of care as soon as medically appropriate. Introduces self to patient/family and explain nurse practitioner role. Facilitates patient/family conferences to review treatment goals, optimize resource utilization, provide family education and identify post-SNF needs. Enhances a collaborative relationship to maximize the patient's/family's ability to make informed decisions re: goals of care, palliative care and hospice. Facilitates discharge to appropriate level of care and uses preferred providers when additional services are required. Prescribes medication to patients based on State of practice. Other duties as assigned and modified at manager's discretion.
