The VA Sioux Falls Health Care System is recruiting (1) full-time Primary Care (PACT) Physician. The Physician provides total care of the Veterans' medical needs by performing history and physical exam, ordering appropriate lab tests, radiology studies, and screening studies.
The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being.
Duties include:
* Outpatient follow up visits, post discharge follow-up and managing care with specialty and sub-specialty medical providers.
* Assist in the preparation of the patient and/or his/her family to include, but not be limited to, teaching (to include health promotion and disease prevention activities), accumulating data and medical problems, and presenting patient centered discussion of medical problems for appraisal and scheduling patients with the appropriate Primary care department services.
* Responsible for completing patient encounters, notes, and correctly coding visits in accordance with coding guidelines.
* Act as the focal point of the designated outpatient clinic by being the contact person for all other consultant services regarding patient care, and providing consults for Primary Care department to other services.
* Provide assistance to the Primary Care Department with regard to duties to assist the service that may be requested. These may include, but not be limited to, assisting in data assimilation for VA reports, quality assurance programs, or others as determined by the Chief of Primary Care or Chief of Staff.
* Demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served in Sioux Falls V AMC Primary Care Department. The individual must demonstrate the knowledge of the changes associated with aging and possess the ability to provide care based upon physical, psycho/social, cultural, safety and other age related factors as noted in the competencies described in departmental and/or unit specific policies and procedures.
* Maintain skills, proficiencies, and professional knowledge required for licensing and clinical scope of practice.
* Provide instruction/preceptor-ship for physician assistant students, physician assistant residents and Nurse practitioner students from various schools across the region, within the Primary Care Department clinic.
* Provide physical/history and medical instruction to pharmacy students, within the Primary Care Department clinic.
* Provide instruction to new primary Care Department providers in the particulars of routine policies and procedures regarding patient care.
* Operate in a Program Manager role assuming additional duties in the Primary Care Service Line.

The applicant must be able to demonstrate active involvement of inpatient services to be at least 50 patients in the last 12 months. OR If a quality profile is not available from the hospital(s) where the applicant currently has privileges, documentation of the applicant's clinic activity for the last 12 months is required. The applicant must be able to demonstrate active involvement in the care of 50 patients in clinical practice in the last 12 months. More education and experience may be required the more advanced the practice becomes.
Successful completion of an Internal Medicine residency program accredited by the ACGME, the American Osteopathic Association (AOA) or the Royal College of Physician and Surgeons of Canada. Training at residency programs accredited by organizations other than the above, may be considered via the waiver of eligibility process as defined in the Credentialing Policy. Certification in the primary area of practice by the appropriate specialty/ subspecialty board of the American Board of Medical Specialties (ABMS), the American Osteopathic Association (AOA) or the Royal College of Physicians and Surgeons of Canada, as applicable. Those applicants who are not board certified at the time of application but who have completed their residency training within the last five years shall be eligible, however must achieve board certification within five years from the date of completion of their residency training unless the appropriate governing Board waives such requirement. Job Description
Hospitalist privileges include the ability to admit, diagnose, evaluate, diagnose, treat and provide consultation to adolescent and adult patients with common and complex illnesses, diseases, and functional disorders of the circulatory, respiratory, endocrine, metabolic, musculoskeletal, hematopoietic, gastroenteric, and genitourinary systems.

M.D. or D.O. degree.
Experience can be demonstrated in one of the following ways and should be reflective of the privileges being requested: An applicant who has just completed residency shall provide their residency procedure log and a list of completed rotations, as applicable. OR An applicant who is not applying directly out of a residency shall provide a quality profile from hospital(s) where he/she currently has privileges showing his/her clinical activity for the last 12 months, to include number of procedures performed-morbidity, mortality, infection rates and other complications. The applicant must be able to demonstrate active involvement of inpatient services to be at least 40 patients in the last 12 months. OR If a quality profile is not available from the hospital(s) where the applicant currently has privileges, documentation of the applicant's hospital activity for the last 12 months is required. The applicant must be able to demonstrate active involvement in the care of 40 patients in the hospital in the last 12 months. More education and experience may be required the more advanced the practice becomes.
Applicants must be able to demonstrate successful completion of an ACGME- or AOA-accredited postgraduate training program in their specialty. These specialties may include internal medicine, pediatrics, family medicine, critical care medicine, an internal medicine subspecialty, or internal medicine/pediatrics. Certification in the primary area of practice by the appropriate specialty/ subspecialty board of the American Board of Medical Specialties (ABMS), the American Osteopathic Association (AOA) or the Royal College of Physicians and Surgeons of Canada, as applicable. Those applicants who are not board certified at the time of application but who have completed their residency training within the last five years shall be eligible, however must achieve board certification within five years from the date of completion of their residency training unless the appropriate governing Board waives such requirement.
