The Primary Care Physician (PCP) works as a lead in our team-based care environment. We are a value-based care provider focused on quality of care for the patients we serve. Our care team consists of doctors, advanced practice professionals, pharm D, care coach nurses, medical assistants, behavioral health, specialists, quality-based coders, referral coordinators and more. Our approach allows us to provide an unmatched experience for seniors. Our model is positioned to provide higher quality care and better outcomes for seniors by providing a concierge experience, multidisciplinary services, coordinated care supported by analytics and tools, and deep community relationships. This robust support allows our PCP to see fewer patients and spend more time with those they do.
Responsibilities
Evaluates and treats center patients in accordance with standards of care.
Follows level of medical care and quality for patients and monitors care using available data and chart reviews.
Assists in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment and home health care.
Acts as an active participant and key source of medical expertise with the care team through daily huddles.
Helps regional medical director and center administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor.
Completes all medical record documentation in a timely manner working with a quality- based coder to optimize coding specificity.
Follows policy and protocol defined by clinical leadership.
Meets with RMD about quality of care, review of outcome data, policy, procedure and records issues.
Participates in potential growth opportunities for new or existing services within the Center.
Participates in the local primary care “on-call” program of CenterWell as needed.
Assures personal compliance with licensing, certification, and accrediting bodies.
Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care.
Required Qualifications

Current and unrestricted medical license or willing to obtain a medical license in state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required
Graduate of accredited MD or DO program of accredited university

Responsibilities
The Medical Pod Leader will be the clinical leader responsible for the clinical performance of clinical pod team members - to include: Accurate diagnosis and documentation, quality, patient experience, affordability, patient safety, clinical team dynamics, and provider productivity. The pod leader will be tasked with developing and maintaining close working relationships with all clinical team members in the clinical pod. The pod leader will also function as the collaborating physician for associated pod advanced practice providers (APPs). This position will have a dyad partnership with the Market Manager and market office staff.
Essential Duties and Responsibilities
Act as on-site supervising Physician for assigned APPs, and Physicians assigned to the POD (collaborating physician). Total number of collaborative Practice agreements should not exceed state regulations.
Collaborative time will correlate to assigned APPs and Physicians
Participate in at least 1 ride along with all collaborative APPs per quarter
Provide a complex patient visit as needed for each APP under supervision
Collaborate with clinical team members associated with pod (Community Health Workers, Medical Social Workers, Nurse Navigators, Care Managers, Patient Care Coordinators, etc.)
Develop and participate in APP clinical development (Evidenced based care, professionalism, etc.)
Lead and participate in high-risk huddles and ensure all patients discussed have a plan of care documented and includes all care management team members
Build team-based culture and work collaboratively with clinical team members
Conducts 10 chart audits per month or per state regulations, whichever is higher
Act as resource for clinical team members for patient care questions/concerns and participate in difficult conversations with patients/family members as requested by team
Routinely conduct panel reviews with APPs
Sign DME, death certificates, and other orders as needed on behalf of APP (as regulated by state)
Frequently review Quality metrics, pod level performance and identify areas for improvement leading to superior Quality (HEDIS) performance
Frequently review clinician capacity and productivity performance. Partner with Site Medical Directors to develop countermeasures and support their implementation when provides are off track
Work collaboratively with Regional Medical Director (RMD) to identify opportunities for care improvement
Assists with service inquiry, resolution of patient complaints and requests for Pod as needed
Assist pod providers to achieve higher EMR efficiency
Participate in patient care to assigned panel and other patients as needed (High risk or complex patients, Physician face to face requirements, cognitive assessments, etc.)
Participate and lead clinical education regarding accurate diagnosis, documentation and management of chronic conditions
Assist with compliance and HR resolutions

The Primary Care Physician (PCP) in our organization demonstrates:
• Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients. They understand that they can strongly influence the patient's outcomes by building a trusting relationship and helping them change behaviors.

We are an outcomes-focused, value-based organization and for their panel of patients, the following metrics are regularly measured to help PCP's become successful and reach partnership status: patient admissions/thousand, using between 18-21 appointment slots per day (each new patient count for 2 slots, follow-up patients 1 slot), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures. Each PCP will have goals for these metrics and will be expected to work towards those targets with their center and market leadership as well as their care teams. Culture is very important in the medical centers and because PCP's are leaders in our organization and centers, they are expected to help champion a positive culture of love, accountability, and passion along with center leadership.
The PCP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The PCP will work closely with the applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals.
The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures. ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.
It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).
The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
For patients that are unable to come to the office-in hospital, SNF, LTC or homebound, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market.
PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company.
Performs other duties as assigned and modified at manager's discretion.

Once Board certified, PCP will maintain board certification in their specialty by doing necessary MOC, CME and/or retaking board exams as required
Must have a current DEA number for schedule II-V controlled substances
Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment.

Qualifications:
MD or DO degree from an accredited medical school
Board-certified or board-eligible in Internal Medicine or Family Medicine
Valid and unrestricted medical license in Texas (or ability to obtain)
Excellent communication and clinical skills
