As an LPN, you will play an essential role in the delivery of quality healthcare at PHC working as a part of our compassionate care teams. In this role, you will be the shepherd of the care team, ensuring that the clinical day flows seamlessly for your patients and providers. Your day-to-day duties will include clinical care and administrative support as delegated by licensed clinical staff. This position typically works at PHC Marshalltown Clinic and may be 32 - 40 hours per week with the schedule to be determined. Clinic hours are Monday - Thursday 8 AM - 8 PM, Friday 8 AM - 5 PM, and Saturday 9 AM - 1 PM. This position will participate in the Saturday rotation working once approximately every other month. Our Medical Assistants are occasionally needed to cover other shifts or to work at other PHC clinics in Ames, Des Moines, and Marshalltown. Mileage reimbursement and/or an out of town travel bonus may apply.
What You Will Do
Documents patient care accurately and timely in the electronic health record. (Clinical documentation)
Facilitates the patient rooming and intake process and obtains accurate patient vital signs per established workflow. (Patient rooming, intake, and vital signs)
Performs point-of-care testing per CLIA guidelines and other established workflows. Ensures service orders are completed timely. (Point-of-care testing)
Administers medications, immunizations, and treatments per written or standing orders. (Medication administration)
Performs lab and diagnostic testing per written orders, explains testing procedure and preparation, and reports test results per procedure. Ensures test orders are completed timely. (Lab & diagnostic testing)
Completes timely prior authorizations and referrals per established guidelines. (Prior authorizations and referral management)
Answers and returns phone calls timely.
Prepares for exam/procedure and assists provider as requested.
Provides patient education appropriate to the patient's age and identified learning needs.
Collaborates with assigned Care Team by participating in team huddles and pre-visit planning for scheduled patients.
Completes and accurately documents quality controls per established process.
Performs pre-cleaning and transport of contaminated instruments per infection control policies and procedures.
Routinely evaluates supply levels in exam rooms, removing expired supplies and restocking as appropriate.

Position Purpose
Supervises the day-to-day nursing activities of the facility in accordance with current federal, state, and local standards governing the facility, and as may be directed by the Nurse Manager, Assistant Director of Nursing and Senior Director of Nursing, to ensure that the highest degree of quality care is maintained at all times. Required Qualifications
Graduate of an accredited school for LicensedPracticalNursing and currently licensed as an LPN in Georgia.
Three to five years of recent LPN experience in GA supervisory experience required.

Directs the day-to-day functions of nursing activities to include supervising CNAs to ensure residents needs are met

Will work with developmental disability or intellectual disability or an acquired brain injury residents

Communicates medical information, change in resident health status, and health related concerns to the resident's family/guardian.
Performs and/or supervises the medication administration processes, administration of prescribed treatments and other therapies. Assumes this responsibility for both units during occasional staffing shortages.
Monitors, evaluates and documents residents prescribed blood sugar levels, vital signs and weights alerting the physician of any findings outside of established normal ranges.
Collaborates with the Dietician and the Culinary Operations Director in planning for those residents requiring special diets.
Collaborate with therapy regarding plan of care
Receives Physician's orders, updates MAR's with new medication orders and change orders and communicates same to other staff assisting with medications.
Performs admission, transfer and discharge processes.
Facilitates the development of the resident's preliminary and comprehensive assessment and written care plan.

The Nuclear Medicine technologist assists with providing safe, age appropriate care to the patient by performing all exams provided by the department per department procedures. Maintains documentation accuracy and completeness in EMR: assessments, vitals, symptoms, treatments, medications, education, charges, lab results, histories, and consults.
Maintains and monitors supplies, equipment, medication stock, and office machines; follows maintenance and quality control protocols; reports malfunctions.
Recognizes and responds appropriately to urgent/emergency patient needs and initiates emergency procedures.
Other duties as needed and assigned by the manager.
Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health's Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.
Education:
Must be comfortable operating in a collaborative, shared leadership environment.
Active RN License in the state of Iowa; Licensure from other states may be required based on location. Compact State License acceptable where applicable.
Basic Life Support (BLS) certification required within 3 months of hire.
Proof of completion of Mandatory Reporter abuse training specific to the population served within 3 months of hire.
Proficient in basic computer skills (Windows environment, web-based navigation, email, MS Office).
Ability to work with diverse groups of people in a collaborative, shared leadership environment.
Requires the ability to read, write and understand the English language, and communicate effectively with patient, visitors, and colleagues while performing their job duties.
Completion of specialty classes or proof of other certifications required for some roles.

POSITION PURPOSE
A Patient Logistics RN serves as the central coordinator for complex patient transitions across the healthcare continuum, expertly managing inter-facility transfers while balancing clinical priorities, bed capacity constraints, and transportation logistics. This role serves as the first point of contact for referring hospitals, healthcare facilities, and community-based physicians/providers to coordinate emergent transfers and acute admissions to MercyOne Health Ministries. The specialized role requires exceptional clinical judgment to assess patient acuity, determine appropriate levels of care, and anticipate potential complications during transport. The Patient Logistics RN functions as a critical communication hub between referring physicians, accepting facilities, transport teams, and family members, ensuring that comprehensive clinical information is accurately conveyed to maintain continuity of care. Through meticulous attention to regulatory compliance, resource optimization, and patient advocacy, these professionals significantly impact patient outcomes, facility throughput, and healthcare system efficiency.
ESSENTIAL FUNCTIONS
1. Facilitate and coordinate patient transfers between healthcare facilities, departments, or units
2. Work in a fast-paced, ever changing working environment.
3. Conduct clinical assessments of transfer requests to determine appropriate level of care needed
4. Communicate effectively with physicians, nurses, and staff at both sending and receiving facilities
5. Manages multi-phone call activity to minimize any delays experienced by customers
6. Document all transfer information accurately in electronic health records and transfer systems
7. Coordinate transportation arrangements including ground and air ambulances when necessary
8. Monitor bed availability and collaborate with bed management teams
9. Serve as a clinical resource for transfer-related questions and concerns
10. Participate in quality improvement initiatives related to transfer processes 11. Demonstrates knowledge of the use of emergency notification system (PULSARA) and takes initiative to activate system for emergency transfer of patients with STEMI, stroke, trauma, ECMO.
12. Takes the initiative to anticipate problems, uses creative thinking and generates reports as requested.
MINIMUM QUALIFICATIONS
1. Current Registered Nurselicense or equivalent licensure (i.e. Physician's Assistant)
2. Minimum of 2-3 years of clinical nursing experience
3. Critical care, emergency, or medical-surgical experience preferred
4. Strong communication and interpersonal skills 5. Excellent organizational and multitasking abilities 6

Job Summary / Purpose
Provides individualized, high-quality nursing care to patients of all ages. Uses a team approach to delegate and coordinate patient care at admission, transfer, and discharge.
