Were currently seeking a Family Practice Physician to provide comprehensive care to both pediatric and adult patients in a clinic setting. This role carries full authority for all preventive, diagnostic, treatment, and referral services provided under the care team model, and plays a vital part in mentoring and supporting fellow providers and staff.
This position may include remote work requirements and is subject to all current CMS vaccine mandates.
What Youll Do
Perform physical exams and deliver preventive healthcare in accordance with evidence-based protocols
Order, interpret, and evaluate diagnostic tests
Develop and communicate treatment plans based on patients individual needs
Prescribe medications or recommend therapies and procedures
Coordinate care with other providers and specialists
Participate in employee training, community education, and outreach
Maintain current credentials and licensure through continuing education
Support and model the principles of the Patient-Centered Medical Home
Engage in quality improvement initiatives and peer review processes
Document patient visits accurately and timely
Supervise mid-level providers and clinical staff on your team

Medical Degree from an accredited college of medicine
Completion of a residency in family practice or internal medicine
Board certified or board eligible in a primary care specialty

Position Summary:
Primary Healthcare Centers is seeking a Licensed Behavioral Health Specialist (LPC, LMFT, or LCSW) to join our integrated care team. This role plays a vital part in improving patient wellness by providing counseling, crisis intervention, and brief behavioral consultations in a primary care setting. The ideal candidate is patient-centered, collaborative, and passionate about making a difference in the lives of others.
Key Responsibilities:
Deliver individual therapy, behavioral consultations, and psychoeducation using evidence-based practices.
Collaborate with providers and clinical staff to create and support patient-centered care plans.
Assess and address mental, emotional, and social needs, including crisis intervention when necessary.
Provide referrals, coordinate care, and support patients with chronic conditions.
Maintain accurate and timely documentation in the EHR.
Participate in outreach events and community engagement activities.
Support continuous improvement of integrated behavioral health practices.
Provide leadership and mentorship to other behavioral health staff if serving in a lead role.
Qualifications:
Masters degree in a human services field (required).
Active LPC, LMFT, or LCSW licensure in the state of Georgia (required).
Minimum of 5 years' experience in behavioral health; experience in primary care preferred.
Proficient in motivational interviewing and evidence-based therapeutic interventions.
Strong documentation, communication, and collaboration skills.
Ability to assess and respond to crisis situations effectively.
Experience with EHR systems and Microsoft Office Suite.
Valid drivers license required.
