Job Description
Location: Mainly at Women and Babies Hospital. Will need to occasionally be asked to travel to other OBGYN locations in Parkesburg and/or Lancaster Health Beginnings offices as needed.
Schedule: Monday through Friday, 40 hours/week, Day Shift. Hours from 8:00am-5:00pm.
A Brief Overview
To provide nursing services to patients at a provider's office. Patient care includes care to patients in following age group: pediatric (infant to 12 years), adolescent (13 to 17 years), adult (18 to 64 years) and geriatric (over 64 years). Provides patient care under the direction of the Registered Nurse and within the auspices of the licensure
What you will do
+ Provides clinical advice by phone/electronic correspondence based on clinical judgments and/or triage protocols in accordance with regulatory requirements and standards of practice. Consults with provider as necessary.
+ Provides patient specific education including but not limited to preventive care, chronic disease management, acute diagnosis, medications and other areas pertinent to patient outcomes.
+ Prepares patient for visit by using appropriate rooming standards tool. Gathers information from patient and directs to the RN or Provider when necessary.
+ Administers injections and medications as prescribed by the provider and in accordance with department protocol, regulatory requirements and standards of practice.
+ Prepares and assists providers with various procedures, minor surgeries and other exams as designated by the provider.
+ Performs Point of Care Testing, including but not limited to hearing, flu, urine and strep tests.
+ Collects blood or other laboratory specimens and prepares them for further testing.
+ Authorizes prescription refills as instructed by the provider.
+ Assists with test tracking and monitoring overdue results.
+ Removes sutures and changes dressings on wounds as necessary.
+ Participates in daily huddles with the provider.
+ Serves as a patient experience advocate by being a positive influence at all times when interacting with patients, visitors and colleagues. Communicates with all patients, visitors and colleagues in a professional manner while keeping customer service a top priority.
+ Handles all patient information in a confidential manner according to Lancaster General Health privacy and confidentiality policies and procedures.
+ Performs various front office responsibilities as necessary.
+ May require some regional travel - valid driver's license is required if traveling.
+ Participates in the on-call responsibilities for this position as required by practice location.
Qualifications
+ Req Other Completion of a LPN educational program
+ Pref 1+ years One (1) to Two (2) years of nursing experience in a primary or specialty care physician's office.
+ Pref Prior triage, emergency room, cardiac care, intensive care or other related specialty nursing experience.
+ Current licensure as a LicensedPracticalNurse (LPN), issued by the PA State Board of Nursing
+ American Heart Association CPR certification

MINIMUM REQUIRED QUALIFICATIONS:

+ Bachelor of Science in Nursing (BSN) or higher level nursing degree.
+ A graduate registered nurse may only practice professional nursing under supervision and if the graduate registered nurse holds a current temporary practice permit. Supervision means that a licensed registered nurse is physically present in the area or unit where the graduate registered nurse is practicing.
+ All Clinical Nurse 1 RN's will be required to successfully complete the Nurse Residency Program.

Job Summary

Utilizes standard precautions. LOCATION: East Petersburg, PA. Opportunity to work remotely _after_ onsite onboarding and training for a minimum of 6+ months and demonstration of proficiency in role.
HOURS: Part Time (32 hours per week). Monday-Friday (8am-4:30pm).

POSITION SUMMARY: This position will provide support to providers, staff and patients who fall under the auspices of the LGHP (Lancaster General Health Physicians) and/or partnering practices. Responsible for the telephonic/electronic intake, assessment, planning, coordination, and implementation of standardized protocols relating to patient symptoms and dispensation of medical advice. The triage nurse will assist with triage in the EMR inbasket and nurse telephone line, nurse case management, collaboration with clinical teams, clinicians, and other departments as necessary, and provide quality patient care in compliance with local, state, and federal regulations and accreditation standards. This patient-centered position delivers services with focus on customer service and symptom management. This nurse works with all primary care practices to assist with triage duties and related tasks.
ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
+ Monitors and responds to triage calls in person, through the nurse phone line and/or via an EPIC inbasket.
+ Talks directly to patients on the telephone and directs them to emergency rooms/urgent care centers, dispenses home care advice, or schedules appointments with health care clinicians during office hours.
+ Determines urgency of seeing patient based on brief assessment and familiarity with patient's condition and history.
+ Uses computerized, evidence-based clinical decision-making including algorithms.
+ Sends patients with high-risk chief complaints such as chest pain, abdominal pain, or severe headaches to emergency departments immediately or arranges for an ambulance.
+ Provides appropriate home health advice to patients who do not need to go directly to the emergency room, and schedules appointment with provider as necessary.
+ Ensures accurate documentation for all conversations and treatments are recorded in patient's electronic medical record.
+ Arranges appointments for patients who do not need to go to the emergency room but need to see a physician. Consults with clinicians as needed.
+ Provides generalized health information as appropriate when patients have questions, i.e. "Do I need a flu shot?"
+ Serves as a clinical resource and professional role model for other nursing staff.
+ Prioritizes work tasks and services and completes them in a timely manner.
+ Assists in medical chart reviews and compiling data for audits and reports.
+ Maintains accurate, clear and concise progress notes, problem lists, and medication lists.
+ Provides patient education regarding disease processes, therapies, and healthful behaviors. Communicates appropriately and tactfully with all staff, patients, families, consultants, and community members in accordance with the Penn Medicine Experience Standards.
+ Provides timely response to calls and patient/staff inquiries.
+ Participates in staff, planning, in-service, and other meetings as needed.
+ Other duties as assigned as related to triage workflows.
MINIMUM REQUIRED QUALIFICATIONS:
+ Current licensure as a LicensedPracticalNurse, or Registered Nurse, issued by the PA State Board of Nursing required.
+ Healthcare BLS
+ Minimum of Two (2) years of clinical experience in direct patient care within a Family Medicine office setting or minimum of Two (2) years' experience working in an emergency department.
+ CPR Certification - American Heart Association AHA Basic Life Support, formerly Healthcare Provider AHA Heart Saver courses for CPR are not accepted required within 30 days of hire.
+ Strong customer service skills
