**Summary:**
The NP will work closely with our complex medical dermatology team caring for a panel of patients with complex diagnoses to help enhance continuity of care. Additionally, the NP will see established patients for routine skin cancer screenings as well as for common dermatologic diagnoses such as atopic dermatitis, psoriasis, acne, warts, and rosacea. .The NP is a member of a multidisciplinary care team that requires effective communication and appropriate collaboration with co-workers and staff locally and other collaborating institutions and agencies. The role requires advanced clinical practice skills, strong self-directed learning skills, critical thinking, an understanding of research principles and practices, and leadership that improves the quality of care and maximizes the practice of medical care throughout the work setting and community. Dermatology NursePractitioner Certification is highly preferred but not required. The ideal candidate will also have an interest in participating in educational initiatives, lectures and presentations locally and regionally. The NP demonstrates clinical expertise and leadership skills and uses clinical and academic skills in the evaluation, diagnosis, and treatment of patients with diseases and adverse health conditions. The NP also functions within a team to help manage the care of complex medical dermatology patients. The NP has prescriptive authority, which includes Schedule II through VI controlled substances in the Commonwealth of Pennsylvania.
Responsibilities:
+ Provides direct clinical care for patient population according to established protocols and utilizes evidence based practice standards in collaboration with team.
+ Maintains accurate and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations consistent with organizational standards in the electronic health record (EHR).
+ Identifies self properly to the patient and family, and clarifies the role of the nursepractitioner or physician assistant if necessary.
+ The APP should function within their scope and refer to the collaborating/supervising physician when outside of scope, and/or directed by the physician or when requested by the patient and/or family. Performs interventions specific to assigned patient population and collaborative/written agreement.
+ Participates in the performance improvement processes to meet quality of care and service standards.
+ Demonstrates awareness of professional responsibility as role model to peers and members of the health care team.
+ Perform approved therapeutic or diagnostic procedures based upon patient's clinical status.
+ Addresses patient and family concerns and creates a therapeutic relationship to provide support.
+ Provide patient education, counseling and instruction regarding patient diagnosis, medical treatments, medications, and preventative care.
+ Participate in and support accreditation, compliance and regulatory activities of the organization.
+ Precept students and other health care learners.
+ Identifies patient/family needs for community resources and refers appropriately.
Education or Equivalent Experience:
+ Master of Science (Required)
+ CRNP: Master's Degree or Doctor of Nursing Practice(DNP) in Nursing required and > 2.5 years of experience.
+ PA-C: Master's Degreee requires and > 2.5 years of experience.
+ The APP must be reviewed by the Credentials Committee and approved by the Medical Board. National Board Certification required.

+ BLS/CPR, as a healthcare provider as per the American Heart Association (For providers in the ambulatory setting) required

Position Purpose:

* Work with Medical Director and Clinic Supervisor to formulate clinical policies, procedures and standards of care. Assists with the development of standards of care; performs on-going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues.
* Advise the Medical Director and Primary Care Provider in ways and means to establish better accountability of the PO services to participants and referral sources. Acts as resource during intake of new participants and in the day-to-day operation of the center programs.
* Participate in provider on-call rotation as required.
Minimum Qualifications:
* Master's degree in Nursing
* Five (5) to seven (7) years of experience required preferably in long-term care.
* Must possess a current, unencumbered, active license to practice in the state of PA.
* CDS Certification and DEA Registration required.
* Board Certification by ANCC or AANP.
* Certification as a NursePractitioner with prescriptive authority required.
* Current BLS required.

The Outpatient Advanced Practice Nurse will:
The Advanced Practice Nurse will be an integral part of the health care team, working with physicians, nurses and nursing or medical assistants to provide comprehensive high-quality ambulatory care, as appropriate to the ages of the patient's growth and development through the life cycle factors (Neonates, Pediatrics, Adolescents, Adults, Geriatrics).
Provide direct patient care, within the limits prescribed by training, and in consultation with a supervising physician.
Assist with evaluating and managing follow up patients
Maintain a panel of continuity care patients
Assist with patient flow by seeing patients as requested by the attending physician.
Assist with procedures if triage nurses are unavailable.
Perform urinary catheterization, venipuncture, and other nursing tasks as requested attending physician - i.e. NBS
Assist physicians with other specialized procedures - i.e. skin biopsy
Coordinate on-call telephone coverage with physician
Assist with patient follow-up.
Review routine laboratory reports for abnormalities, notify responsible clinician according to degree of urgency.
When appropriate, contact families to return for further evaluation or treatment of conditions detected by screening.
Document all aspects of patient care promptly.
Complete in accordance with practice guidelines, within 24 hours of patient care.
Assist in developing APN role in Genetics, to teach other department personnel.
Teach medical students the elements of well child care, one formal session a week, and informally at other pertinent times.
Provide in-service education for nurses and nursing and medical assistants about common problems and their management.
Share the nursepractitioner perspective of health care with residents and other physician staff.
Assist with coordination of emergency policy and procedure including organizing mock training exercises for staff
Assist with planning, developing and implementing of patient education activities, as appropriate to the ages of the patient's growth and development through the life cycle factors (Neonates, Adolescents, Pediatrics, Adults, Geriatrics).
Evaluate educational level and understanding of caregivers.
Explain the nature of the child's illness, the reason for all tests and procedures, and the care necessary in the home, including administration of medicines, ensure understanding, using printed information to supplement oral teaching.
Develop new, innovative methods of patient education, including group waiting room teaching, geared to the educational level of the clientele.
Locate available simple pictorial education materials or develop new ones as necessary, to assist with the teaching of young parents and those with limited education.
Demonstrate competency in knowledge and skill necessary to provide care to provide care to specific age-related populations (i.e. Neonates, Pediatrics, Adolescents, Adults, Geriatrics).
Maintain CPR review certification on an annual basis as evidenced by a valid card. Submit to Chair and Practice Coordinator.
Attend all mandatory in-service updates as required.
Attend at least the minimum required continuing education programs per year, needed for maintenance of certification and licensure.
Assist with setting and revising practice policies.
Review current policies and make suggestions for revision.
Be alert for the need of new policies to improve the efficiency and patient friendliness of the clinic, and, after discussion with the Practice Coordinator and Chair, assist with writing these.
Assist with routine paperwork.
Call in routine medication and vitamin refills, consulting with physician as necessary.
Assist with sharing patient information, either in writing or by telephone, with other health care providers. Complete school and daycare Supplemental Social Security Income forms.
Requirements:
Current Registered Nurse licensure in the State of New Jersey
Current Advance Practice Nursing licensure in the State of New Jersey
Current CDS and DEA required
Master's degree in nursing required
Two (2) years' experience in specialty strongly preferred
Certified as a nursepractitioner in the State of New Jersey within area of specialty
BLS certification, as approved by the American Heart Association
We offer competitive base rates that are determined by many factors, including job-related work experience, internal equity, and industry-specific market data. In addition to base salary, some positions may be eligible for clinical certification pay and shift differentials.
The salary range listed for exempt positions reflects full-time compensation and will be prorated based on employment status.

Summary: The Certified NursePractitioner-Mental Health will work in both the general outpatient clinic and the new multidisciplinary TEAM Clinic. Both models are focused on increasing patient access to treatment. The TEAM Clinic offers an innovative clinical care model using an evidence-based and collaborative approach (medication management and psychotherapy) to mental health treatment.
All CRNPs function under the guidelines as designed by the University of Pennsylvania Medical Center medical staff guidelines and are credentialed through that body. The CRNP will be held accountable through a formal job description, delineation of privileges, and through their agreement with their collaborating physician and a Collaborative Practice Agreement. All clinical work hours and associated requirements will be defined and managed under the direction of the collaborating physician/division designee, and within the Human Resource guidelines. The NursePractitioner contributes to excellence in education, leadership, professional contribution, and practice.
Responsibilities:
+ Provides direct clinical care for patient population according to established protocols and utilizes evidence-based practice standards in collaboration with team.
+ Maintains accurate and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations consistent with organizational standards in the electronic health record (EHR).
+ Identifies self properly to the patient and family and clarifies the role of the nursepractitioner if necessary.
+ The nursepractitioner should function within their scope and refer to the collaborating physician when outside of scope, and/or directed by the collaborating physician or when requested by the patient and/or family.
+ Performs interventions specific to assigned patient population and collaborative agreement.
+ Participates in the performance improvement processes to meet quality of care and service standards.
+ Demonstrates awareness of professional responsibility as role model to peers and members of the health care team.
+ Perform approved therapeutic or diagnostic procedures based upon patient's clinical status.
+ Addresses patient and family concerns and creates a therapeutic relationship to provide support.
+ Provide patient education, counseling and instruction regarding patient diagnosis, medical treatments, medications, and preventative care.
+ Participate in and support accreditation, compliance and regulatory activities of the organization.
+ Precept NP students and other health care learners.
+ Identifies patient/family needs for community resources and refers appropriately.
+ Performs duties in accordance with Penn Medicine and entity values, policies, and procedures.
