Staff Nurse Under the supervision of the Nursing Supervisor, this position is responsible for conducting assigned health assessments, developing service plans, performing health monitoring and personal care programs of the facility. Functions
1. Conduct assigned initial assessment on prospective residents, including off-site visits. These may include, but not be limited to home visits, hospital visits and nursing home visits. The assessment should include input from pertinent health care providers, family and friends on an as needed basis.
2. Clarifies initial or hospital discharge medication orders and contacts the Physician to review the resident's status and medication plans.
3. Develops assigned service plans for residents based upon the resident assessment(s). The service plan should be designed to meet the personal care needs of the resident and to monitor significant health conditions, while also promoting resident independence and dignity. This includes:
ensuring appropriate medical follow-up
providing special instructions to resident and staff related to medical diagnosis
coordinating and reviewing lab work
coordinating care with clinics, outside physicians and hospitals
health teaching
4. Completes standardized assessment and assessment tools.
5. Promotes resident empowerment and self-care activities including self-medication.
6. Performs assigned quarterly and annual reassessments on residents often times utilizing computer and software program. Also performs such assigned reassessments when the resident's health condition warrants.
7. Communicates with resident's physician(s), other pertinent health care providers and family as to the ongoing health status of the residents.
8. Refers residents to appropriate medical care when needed. Conducts follow-up care as needed.
9. Assigns the various personal care tasks contained in the resident's service plans to the certified nursing assistants on an ongoing basis.
10. Assists, as assigned, in the facility's resident medication monitoring and set-up program.

Physical Requirements

Qualifications
The Staff Nurse must have a valid IL LPN or RN license and shall have had at least 1 year of experience in a geriatric care environment. He/she must be able to perform each of the essential position functions and meet the facility's health assessment requirements - including a two-step Mantoux skin test. Valid driver's license with driving record in good standing per insurance vendor.
