Develops strong partnerships with agencies and key stakeholders in the community that could serve as referral sources and experts to grow the programs. * Provides strong, mission-driven leadership to program staff ensuring high-quality client care and consistent achievement of performance goals outlined by grants, contracts, and agency standards. * Builds a collaborative, effective team to achieve both immediate programmatic needs and long-term departmental growth. Management Duties * Oversees recruitment, hiring, onboarding training, performance evaluation, and professional development for program staff across the Crisis & Disaster Care Department. * Provides clinical supervision and consultation for clinicians seeing cases in the Suicide Prevention Counseling Center or disaster care program. Identifies training needs and ensures the development, delivery, and evaluation of relevant trainings in collaboration with the Training Division; facilitates or co-leads training as needed to support program quality and staff growth. * Ensures staff have clear work plans, defined responsibilities, and appropriate support to succeed in their roles; fosters a culture of accountability, collaboration, belonging, and clinical excellence. * Supervises assigned staff to ensure high-quality, trauma-informed client care. * Provides immediate supervision and consultation in the midst of crises. * Ensures timely and accurate completion of all clinical and program documentation, including assessments, treatment plans, progress notes, and required outcome measures or data reports, * Participates in and facilitates team meetings and other department gatherings to support coordination, learning, and team cohesion. * Represents the department in community settings and with external partners (e.g., Department of Mental Health, referral agencies); ensures strong, collaborative relationships that support continuity of care. * Develops and strengthens referral relationships and ensures effective procedures are in place for referral, admission, care coordination, and discharge processes across all department programs. Financial Duties * In collaboration with the Business Development Director, regularly examines metrics that drive business operations and makes changes to optimize program efficiency and growth. * Helps ensure long-term financial sustainability by participating in planning efforts related to new funding opportunities, grant applications, and service expansion, in close collaboration with the Grants Team, Business Development, and Finance. * Assists in development of program budget and ensures expenditures comply with approved budgets; monitors and evaluates budget issues and recommends corrective action. Position Requirements * Current California License as a clinician (LCSW, LMFT, LPCC, PsyD, PhD) and at least 5 years post-licensure. * Experience or interest in strategic planning and/or program development, especially in emerging or growing service areas. * Minimum of 3 years providing clinical service and supervision in a social service/mental health and/or substance use setting. Primary Duties * Assists clients to locate, access and utilize a full range of health care services, community support groups and resources; establish and maintain housing; access mental and medical care, rehabilitation programs and drug/alcohol services; increase social contacts and engage in constructive activities. * Assesses clients' needs and functioning, presents cases in treatment team meetings and monitors progress as needed. * Acts as client advocate interfacing with housing and support agencies including property owners, support groups, Social Security, Medicare/Medi-Cal, etc. * Develops and maintains rapport and cooperative relationships with community agencies for the benefit of the clients. * Provides individual/group supportive counseling and psycho-education to assist clients in coping with mental illness and/or substance use, promote recovery and develop self-sufficiency. * Provides crisis intervention and facilitates conflict resolution among the program clients as needed; assists clients in problem-solving life situations. * Interfaces with clinical and medical staff in coordinating client care. * Meets Agency client service expectations. * Monitors and maintains documentation of client services; ensures compliance with Agency, county, state and federal policies and procedures, standards and guidelines. * Develops and maintains clinical skills through seminars, workshops, or other forms of continuing education/training related to individuals with a history of mental illness and/or substance use disorders. * Attends and participates in Agency and program/team meetings, individual/group supervision, and related community Agency meetings as required/requested. * Provides social rehabilitation services to clients and their families individually and/or group setting. * Conducts outreach services to generate client referrals. * Participates in community resource and/or health fairs. Position Requirements * A minimum of two (2) years of related experience or a bachelor's degree is required. * Registered Alcohol & Drug counselor (RADT) or ability to obtain certification within 90 days upon hiring through CCAPP. Maintain renewal requirements annually. * Previous experience working in a social service/mental health environment, preferably with mental health and/or substance use clients. * Bilingual Spanish preferred but not required. * Current knowledge of all job specific skills including crisis intervention and multicultural and socio-economic issues, and documentation standards. * Knowledge of Agency policies and procedures, HIPAA, policies and documentation guidelines, and county, state and federal regulations relating to service programs for individuals and families with a history of mental illness and/or substance use. * Knowledgeable about and actively supports integrated mental health, physical health and substance use treatment for improving care for the whole person. * Basic advocacy skills to include a holistic approach to integrated health care and case management. * Skills to present ideas, information, and viewpoints clearly, both verbally and in writing. * Able to efficiently use the personal computer to include Microsoft Office Suite and Electronic Health Record. * Ability to travel in the community and/or between agency sites to provide services in a variety of settings, including outpatient and field based locations (home, school, etc.). * Ability to utilize data, outcomes, experience and judgment to make effective business and therapeutic decisions. * Demonstrate commitment to team objectives and Didi Hirsch mission. * Adapt to changing needs by acquiring new skills and knowledge. * Ability to transport clients using personal or Agency vehicle and/or public transportation. * Current California driver's license, car insurance, and a driving record acceptable to the Agency's insurance carrier. Primary Duties Engage eligible ECM Members utilizing various modalities including in-person and tele-health. Build strong relationships with health plan members to educate them about the benefits and eligibility criteria for ECM as a means of increasing ECM participation. Outreach to community members through various channels, including community events, workshops, and outreach programs to raise awareness. Accompany ECM Member to office visits, as needed, and in the most easily accessible setting, within MCP guidelines. Advocate on the member's behalf with health care professionals. Promote health promotion and self-management training that addresses barriers to physical and mental health care, such as providing information or instruction on health topics including but not limited to distributing health promotion materials. Monitor treatment adherence (including medication) and communicate adherence concerns to LCM and other treatment providers. Provide health navigation services, including training, information, referrals, and support members in accessing health care, understanding the health care delivery system, or engaging in their own care. Arrange transportation to medical appointments using the MCP medical transportation system. Call Member to facilitate a visit with LCM and other care providers. Assist with linkage and connect ECM Member to other social services and supports the member may need. Use motivational interviewing, trauma-informed care, and harm-reduction approaches. Monitors and reports member's behavior, schedule, medications, and activities and provides relevant feedback on any behavioral deviations from recovery to the team and relevant stakeholders to support timely responses of care.
