Ensure timely follow-up on referrals and maintain accurate documentation of outreach activities.
Market Leadership:
Provide formal and informal education opportunities with physicians, physician office staff, hospital social workers/discharge planners/patient unit staff, nursing home staff and staff in community agencies.
Collect and analyze data of referral patterns from all referral sources, to include physicians, social workers, discharge planners, hospital vs community physicians and SNFs.
Maintain a thorough knowledge of the community's health care market, competition, consumer demands and governmental policies which influence the agency's outreach goals and access for patients/families
Act as the face of the organization in the local market, promoting brand awareness and trust.
Monitor market trends and competitor activity to inform strategic planning.
Skills and Experience Required
Proven experience in healthcare marketing, sales, or community outreach-preferably in home health, hospice, or a related field.
Demonstrated ability to meet or exceed performance goals and key performance indicators (KPIs). This includes:
Referral volume targets
Patient admission and conversion rates
Community engagement metrics
Timely follow-up and documentation standards
Relationship development with referral sources

What You'll Do:
The Program Officer, Postsecondary will lead efforts to strengthen college and workforce pathways, ensuring improved access, persistence, and completion for historically under-resourced students from North Chicago. Reporting to the Co-Managing Directors-North Chicago, the Program Officer will be responsible for developing and implementing a comprehensive strategy that advances equitable postsecondary opportunities and success. This role requires both strategic vision and hands-on execution. The Program Officer will partner with high schools, colleges, community-based organizations, and other funders to identify barriers, design interventions, and ensure alignment across initiatives. The Program Officer will manage a grantmaking portfolio, oversee program implementation, monitor outcomes, and use data to drive continuous improvement.
The ideal candidate is a collaborator who brings deep knowledge of the postsecondary landscape, experience with diverse student populations, and the ability to build strong partnerships across sectors. This person will play a key role in shaping and advancing initiatives that support students to persist and complete a degree or credential and access clear pathways from high school into college or the workforce leading to a meaningful career.
Primary Responsibilities (the “What”)
The Program Officer, Postsecondary will be accountable for the following areas of responsibility:
Strategic Leadership and Alignment
Develop and implement a comprehensive strategy to improve college access, persistence, and completion, along with workforce pathways, leading to successful outcomes for North Chicago youth.
Align postsecondary initiatives with community goals and the Foundation's mission, strategic plan and theory of change.
Manage Program and Grants Portfolio
Manage a portfolio of grants focused on postsecondary access, college persistence, completion and workforce readiness, ensuring alignment with the Foundation's mission, strategic priorities and North Chicago Community goals.
Source, evaluate and recommend new funding opportunities through proposal review, due diligence and analysis.
Continuous Improvement through Data Review, Collaboration, and Evaluation
Monitor and evaluate grantee and initiative outcomes through site visits, data reviews, and ongoing communication.
Provide technical assistance and thought partnership to strengthen implementation and impact.
Address data gaps in postsecondary outcomes; use evidence-based practices to refine strategies, interventions, and grantmaking priorities.
Engage Community and Partners in Support of Student Success
Partner with colleges, North Chicago School District 187, community based organizations, peer funders, and other stakeholders to expand supports and drive shared accountability for outcomes.

THE POSITION
The Lloyd A. Fry Foundation's Education program has been a cornerstone of our grantmaking since our inception. We seek an Education Program Officer to develop and lead our education grantmaking (~$2M annually), which is focused on increasing the academic achievement of students in historically disinvested Chicago Public Schools.
We support this goal through work that strengthens the preparation and development of principals to lead high-performing schools, the development of teacher leaders to support professional learning in schools, and programs that provide rigorous academic enrichment opportunities for students.
We look for programs that monitor and demonstrate ongoing effectiveness by assessing improvements in academic achievement, instructional quality, and student experience, particularly the classroom conditions that support academic engagement. The Foundation encourages proposals that include efforts to enhance the quality of programs. These efforts might include incorporating new program elements, professional development for program staff, or the development of evaluation tools, among others.
RESPONSIBILITIES
The Education Program Officer's core duties include the management and execution of four annual education grant cycles; knowledge development (e.g, keeping up with the latest education research or findings related to the Lloyd A. Fry Foundation's priority areas and identifying related opportunities and pathways to pursue); and participating in external networks (e.g., funder collaborations, grantmaker organizations, and professional associations) that allow us to collaborate with others who seek to make a positive impact in Chicago Public Schools.
In addition to these specific duties, the Education Program Officer will use their professional knowledge and insight to help refine and improve how we increase the academic achievement of low-income students in historically disinvested Chicago Public Schools, and provide guidance to the board of directors about challenges or opportunities that may impact our work in education.
A detailed breakdown of the Education Program Officer's responsibilities are provided below:
Manage the Education grants portfolio; work with colleagues and the board of directors to identify programmatic goals and strategies, and manage and plan the program grants budget
Oversee the entire “life cycle” for the Foundation's education grants
Solicit and review proposals and letters of inquiry, arrange and manage site visits, prepare proposals for board review, and present written and verbal recommendations to the board of directors
Build and support a pipeline of grantees whose work has the greatest potential to bring about desired educational outcomes; and identify new organizations and initiatives to help fulfill programmatic goals
Actively identify challenges that can be addressed through the Foundation's grantmaking, and opportunities that have the potential to advance program goals
Write about complex issues and concepts in education in clear, accessible, and jargon-free language for lay audiences. Writing, in general, and this type of writing specifically, is a major responsibility of this role.
Partner with grantees to support collaborative work, and help organizations advance knowledge and practice in the field
Cultivate and maintain new and existing relationships with education or adjacent leaders and collaboratives to stay abreast of new developments; generate new ideas and resources and foster partnerships
QUALIFICATIONS

The salary range is $120,000 - $150,000, commensurate with experience and qualifications. Employee benefits include three weeks of vacation, one-week paid office closure during winter holidays, paid sick days, employer-paid health insurance (85% of the premium), and after one-year of employment, contributions to a 403(b) retirement plan.

Under the direction of the Regional Director the Instructional Coordinator supports Early Learning Services sites throughout Chicago and surrounding suburbs by providing coaching, expertise, oversight, training, and leadership in the area of education and child development services for all Easterseals HS/PFA programs and services in compliance with the Head Start Program Performance Standards, the Head Start Act, and the Illinois State Board of Education. The Instructional Coordinator ensures that all education staff are implementing evidence-based instructional practices and designs and delivers a plan for intensive and individualized professional development that results in exemplary teacher-child interactions, and instructional quality. Provides content area support and oversight in the areas of education, disabilities, and mental health services to the Easterseals Head Start/Early Head Start programs in an assigned geographic area.

MAJOR FUNCTIONS/ACCOUNTABILITIES:
Work with the Instructional Coaches, other Easterseals staff, partner site directors and staff, and the community to ensure consistent, high-quality education and disabilities services at all assigned HS/EHS programs.
Work with Easterseals staff, site directors and staff, and the community to ensure that programs recruit and enroll Head Start/Early Head Start children, including children with disabilities, to meet their funded enrollment numbers and to keep an active waiting list.
Provide supervision to Instructional Coaches and Preschool for All Teachers, including annual appraisals and development of professional development plans.
Participate in program planning based on analysis of child and program educational outcomes.
Provide support to Instructional Coaches and Preschool for All Teachers at sites on a regular basis, including assisting with building collaborative relationships with the Family and Health Services team, and site directors and staff, in order to provide integrated services to families.
Ensure that a family strengths-based approach is understood and followed by staff.
Participate in active supervision to ensure the safety of all children and that no child is ever left alone or unsupervised.
Participate in program planning and implementation of all service areas as part of an integrated leadership team.
Participate in the self-assessment process, community needs assessments, and strategic planning.
Participate in the monthly analysis of monitoring data with the leadership team, provide recommendations, and ensure that data is utilized to guide planning.
Participate in site visits, presentations, and orientation for prospective new sites as part of an integrated leadership team.
Implement a system to ensure that all education and disabilities information is entered into Child Plus and Teaching Strategies GOLD in a timely manner, including developmental screenings, referrals, disability status, home visits, parent-teacher conferences, observations, and assessments.
Prepare monthly reports with updates about PIR information and information tracked for education and disabilities.
Facilitate and/or participate in regularly scheduled content area, team, director, and other meetings.
Implement a system to ensure that all developmental screening information is completed within 45 days of children's enrollment.
Implement a system to ensure that all children identified as benefitting from further evaluation are referred and evaluated according to timelines.
Implement a system to ensure that all children and families who are identified as benefiting from mental health services receive support in a timely manner.
Implement a system for supporting children with behavioral concerns, including a behavior review process and behavior plans.
Participate in the semi-annual CLASS assessment of all classrooms.
Implement a system of ongoing monitoring for education, disabilities, and mental health to be completed during daily/weekly visits by direct support staff and content area staff.
Implement a system for tracking non-compliance and developing and monitoring action plans.
Implement a system for ongoing review of Child Plus and the PIR to ensure information is accurate and complete, including monthly tracking and reporting of information.
Participate in the ongoing self-assessment of the program.
Travel to Easterseals and partner sites regularly.

Requirements
Education/Certification:
Bachelor's Degree in Early Childhood Education or Child Development, or in another field with at least 30 semester credit hours in ECE required.

The Purpose of the Peer Support Program is to empower emerging student leaders to serve the Kennedy King Community by providing informed and structured support to their fellow peers. These Peer Advocates work in tandem with the Wellness Center to positively promote counseling, share their experiences, encourage self-sufficiency, partner with students as they pursue their goals and appropriately refer students to the Wellness Center.
ESSENTIAL DUTIES
* Acts as a Wellness Center student representative, current students, faculty, staff, and the public.
* Attends Peer Support training sessions as required. Must work within the scope of the training and make appropriate referrals to licensed professionals when necessary.
* Refer current students to the Wellness Center.
* Assist students in making appointments with the Wellness Center.
* Provides information about resources and counseling services available to students.
* Supports and assists planning of Wellness activities, including tabling events, workshops, classroom events and other duties as needed.
* Provide constructive feedback of Wellness services to improve the students' overall experience.
* Provides a positive/supportive example to the college for new and returning students.
* Adheres to the City Colleges of Chicago Customer Service Excellence Standards. Applicant must participate in mandatory trainings, scheduled meetings and a developmental relationship with Wellness Center staff.

Work Schedule
Minimum 40 hours per week. Schedule to be arranged, but typically, Monday-Friday from 8:30 am - 5:00 pm some nights, weekend work WILL BE required. Will be required to work at least 1 Saturday per month and overnight travel on college tours. This is an EXEMPT position with various work times and hours.
Department Summary: TRIO Upward Bound and Educational Talent Search are educational program funded by the U.S. Department of Education. Educational Talent Search designed to help middle and high school students to continue to the next grade level, graduate from high school, and enroll in and complete a college education. The TRIO Educational Talent Search Program at GJTF serves 500 students from target schools in the Southland. Upward Bound serves 60 students in 9th-12th grade with post-secondary attainment.
Duties:
Under minimal supervision of the TRIO Director, the Educational Counselor will work independently to:
Assist with outreach, recruitment, and identification of eligible participants
Serves as liaison between Program, target schools, and community agencies
Develops Individual Academic Plan (IAP) for each student
Counsel and assist program participants in middle school and high school graduation requirements, college financial aid requirements and college admission
Develop and facilitate the career development, self-development, and college placement classes and/or workshops
Coordinate and develop progress reports on students' academic advising sessions and monitors academic progress
Provide guidance to Student Tutor/Mentor
Act as point of contact and liaison between foster care/homeless participants and foster care/homeless resources and target school personnel.
Assess and monitor rigorous secondary school program of study (AP Requirements) for assigned target high school participants
Support and assist participants in completing and submitting postsecondary admissions and financial aid applications
Provide assigned target school participants with information and guidance on financial and economic literacy
Collaborate with TRIO staff to support the transition of middle school participants to high school
Assist with collecting and entry of data for the Annual Performance Report, including collection, accuracy and verification of participants' academic data
Support Program Director and Assistant Director in maintaining Program records in compliance
Develop, coordinate and implement other program related projects as assigned.
Assist with planning and attending college tours, career and cultural excursions, weekend activities
Create and develop workshops for participants
May be asked to assist with Crete Access Granted Students and TRIO Upward Bound participants
Perform all other duties as assigned.
Minimum Qualifications
Education: Applicants must have a bachelors degree
Experience: A minimum of one year of professional experience.
