About WISe (Wraparound with Intensive Services) WISe is a family- and youth-driven approach that supports Medicaid-eligible individuals up to age 21 with complex behavioral health needs. Services are provided in natural settings - home, school, and community - to help families thrive and avoid restrictive out-of-home placements. ???? Watch this short video to learn more about WISe What You'll Be Doing Provide case management, treatment planning, and therapeutic coordination. Partner with caregivers and natural supports to promote recovery and stability. Deliver services in the community with flexible scheduling, including evenings.
