Develops the community network program within the department and serves as an interface between the general public, hospitals, and medical professionals for the purpose of expanding cancer awareness. Works closely with the corporate communications and marketing department on targeted cancer initiatives and campaigns both locally and regionally. The PDS III serves as a role model and community liaison and works as part of the medical case management team to address barriers to connection and retention in care. They support providers and the case management team through an integrated approach involving care management and community outreach. The PDS III assists clients access services and resources and address issues of adherence through individual and group sessions.
Among the key duties of this position are the following:
Works with identified community partner(s) to establish rapport and assist clients with linkage to care among those who have been lost to follow-up or who have never accessed care. a. Manages an assigned caseload. Conducts street outreach activities and condom distribution for the program in Newark and Essex County.
