The Program Support Specialist reports directly to the Clinical Team Manager with overall program supervision by the Director.
QUALIFICATIONS
ESSENTIAL
Knowledge of and experience using confidentiality principles and guidelines. Ability to have contact with consumers and demonstrate respect and principles of recovery. Commitment to improve one's own cultural sensitivity. Ability to work independently and set priorities. Computer experience; accurate and prompt word processing ability. Excellent writing, editing and organization skills. Possession of a valid driver's license, a favorable driving record, adequate insurance (per WI law) and access to an automobile.
PREFERRED
Experience in a healthcare setting, working directly with consumers with mental health and substance use concerns. Computer experience with a Macintosh system. Accounting experience. Experience and expertise in performing clerical duties in a culturally sensitive manner. Bilingual (Spanish or Hmong).
Responsibilities:
Administrative
Assist staff in day-to-day functioning: answer phones, welcome consumers, fax and file paperwork, assist with computer questions/problems, prepare and maintain daily schedules for staff and meeting rooms Maintain databases to track consumer outcomes and paperwork due dates Work with JMHC credentialing specialist to ensure CCS staff obtain and maintain CCS credentials Maintain a clean, professional and friendly workspace

The Program Support Specialist (Care Coordination Specialist) will provide HIV case management services to patients enrolled in the FXB Center HIV Clinical Practice programs. The services provided include biopsychosocial assessments, adherence counseling, and referral/linkage to other services to optimize patient health outcomes. This position will also facilitate the continuity of care and provide ongoing support and resources to family members of the patient. Some key responsibilities include:
Interview each patient and their family to assess their social, cultural, family financial and continued medical health care needs.
Provide adherence counseling to facilitate long-term engagement in care, and to promote better compliance with assigned medication therapies.
Track laboratory result for each patient. Provide those living with HIV / AIDS a comprehensive continuum of services through critical linkages with ancillary medical and social support services. Provide referrals to community-based ancillary services, faith-based services, or medical care services as needed. Document and track the linkages of all referrals made. Complete bi-annual social service needs assessment and service plan on each patient or family. Identify and resolve barriers clients may have in following through with their service plan.
Minimum Education and Experience
Bachelor's degree in social work or other health profession plus two years of experience in a healthcare setting. Equivalent education, experience and/or training may be substituted for the degree requirement.
