RESPONSIBILITIES:
Under the supervision of the Program Director of the Supported Living Program the Case Manager provides client care, service documentation, and Facility monitoring for clients of assigned apartments, and has backup responsibility for other apartments, all of which are located in multi-level buildings.
Generally, the Case Manager responds to the treatment and rehabilitationneeds of assigned clients, monitoring theirprogress, stablity, and movement throughout the course of treatment in the program. Specifically, the case manager must:
A. Assume primary case coordinating responsibilities for all clients that are assigned, including but not limited to advocacy, counseling, crisis intervention, and treatment and discharge planning.
B. Orient new clients to the Supported Living Program and facilities or, whenever possible, enable another client to orient a new client.
C. Develop treatment plans with maximum client participation, and support clients' efforts to achieve them.
D. Communicate appropriately with other treatment personnel both within and outside the agency, exchanging information relevant to client treatment.
E. Conduct intake interviews and case conferences, as required to maintain census and coordinate client care.
F. Lead or co-lead group sessions, including house meetings, on a schedule determined by the Program Director or the Assistant Director.
G. Relay clinical and other client-related or site-related information to other program staff and/or the Program Director or Assistant Director at staff meetings and other appropriate times.
H. Provide services in accordance with all State and federal MediCal requirements and County policies and procedures.
I. Maintain program records including client records, progress notes, treatment plans, and other relevant information in accordance with County policies and procedures and State and federal MediCal requirements.
J. Assist in the training of relief case managers, volunteers and interns, when requested by the Program Director or Assistant Director.
K. Coordinate and schedule emergency relief coverage when assigned by the Program Director or Assistant Director.
L. Consult with the Program Director, the Assistant Director, and the Program Consultant, as necessary, on such issues as case review, medications, group dynamics, etc.
M. Attend and participate in staff meetings, supervision sessions, and other programmatic and/or administrative meetings, as assigned by the Program Director or the Assistant Director.
N. Participate in the on-call system of the program, including evenings and weekends.
O. Respond to emergencies in a professional, thorough, and timely manner. This includes staffing apartment sites as needed to effect 24-hour coverage during a crisis, and may further include overnight stays in apartments and scheduling and coordinating relief assistance.
P. Monitor the quality and needs of the program physical environments and assist the Program Director or Assistant Director in the coordination of necessary maintenance and repairs.
Q. Serve as a role model for positive employee relations, professional functioning, and for adherence to the principles of social rehabilitation.
R. Comply with all applicable federal, state and local governmental laws and regulations, as well as all agency policies and procedures, including the code of ethics.
S. Perform other related duties as assigned by the Program Director, the Assistant Director, or the Director of Clinical Services.
T. Inform the Program Director or Assistant Director of any personal events and/or circumstances which may affect the ability to perform the proscribed job responsibilities and functions or to meet minimum qualifications.
MINIMUM QUALIFICATIONS:
MHRS Required in a mental health related field. Master's degree Required
AA in any field + 6 years experience in any mental health related field
BA in any field + 4 years experience in any mental health related field

Specific experience in residential treatment and/or knowledge of social rehabilitation is preferred.
At least two year's experience in a community mental health program is preferred.

The Payroll and Benefits Specialist reports to the Controller and must have proven, extensive experience in all aspects of payroll and benefits process, including payroll tax and wage garnishment, and internal/external audit, City and County of San Francisco, County of Napa, Santa Rosa compliances. RESPONSIBILITIES:

The Seniors program includes Rypins House, a 6-bed transitional residential treatment program located in San Francisco's Mission District, Carroll House, a 6-bed residential treatment located in Bernal Heights and Rypins House Day Treatment, a day treatment program for Seniors Program clients and older adult consumers from the community. Rypins House has been provided by the agency under a contract with the City and County since 1980 and Carroll House since 1979. This program provides services to adults over age 60. This transitional residential treatment program for seniors is the only rehabilitation and recovery-based 24-hour alternative to institutional treatment, or jail, that is designed to meet the needs of behavioral health clients over age 60 in San Francisco.
PRIMARY RESPONSIBILITIES:
Under the supervision of the Program Director or the Assistant Director, Counselors provide support and assistance to clients of these two unique adult psychiatric transitional residential treatment program. RESPONSIBILITIES:
The Staff Primary Care Nurse Practitioner's/ Physician Assistant's primary responsibility is to provide consistently effective primary care clinical services to program clients. The Staff Primary Care Nurse Practitioner/ Physician Assistant is responsible for the direct physical clinical assessment of all newly admitted clients, the on-going monitoring and care of all clients being treated at Dore Residence, and the proper referral of clients to outpatient services upon discharge from the Residence.
The Staff Primary Care Nurse Practitioner/ Physician Assistant must demonstrate the on-going capacity work as an interdisciplinary team member in order to effectively respond to the needs of a diverse client population in psychological distress.
Under the direction of the Program Director and the medical supervision of the Primary Care Consultant, ensures that services are provided in a manner consistent with prevailing standards of medical care, with applicable Federal, State, City and County laws and regulations, and with Progress Foundation policies and procedures.
Direct services include screening, full history taking and physical evaluation and documentation, in situations where treatment if appropriate, and referral for the full spectrum of non-emergent and occasionally emergent ambulatory primary care conditions presented at the Residence. This will also include arranging a safe and timely transfer of clients to other needed treatment services.
LEADERSHIP
Provide services within the Residence that are congruent with Social Rehabilitation values, and promote SR values and philosophy within the Behavioral Health treatment community. (clinical)

Qualifications

The Community Resource Navigator assists program participants to access mental health services, supplemental perinatal services, and health-promoting resources. PRIMARY RESPONSIBILITIES:
