JOB DESCRIPTION:
The Physical Therapist plans and administers medically prescribed physical therapy treatment programs for patients in their residence to restore function, relieve pain and prevent disability following disease, injury or loss of body part. Consults with medical and professional staff to plan and coordinate joint patient management objectives. Acts as a case manager for patients when primary need is for physical therapy. Has contact with patients under a variety of circumstances. Is subject to varying and unpredictable situations and occasionally to irregular hours.
EDUCATION
Bachelor of Science or Master's degree in physical therapy from a program accredited by the American Physical Therapy Association (APTA), required.
EXPERIENCE
Two years of experience as a physical therapist required.
One year of experience in providing rehabilitative therapy to a variety of patients including the elderly, required.
Community/home health experience preferred.
LICENSURE/CREDENTIALS
Licensed to practice as physical therapist in the State of Washington required.
Current BLS certification required.
Valid Washington driver's license required.

Job Description
Position:
In
Sunnyside
, we service a not-for-profit, 26-bed critical access hospital that provides acute care and outpatient services. You would work alongside a
large and supportive interdisciplinary therapy team, including another Full-Time PT and a Full-Time PTA
! The hospital has a generalist practice with
exciting rehab program development
underway to support the orthopedic, neurologic and rheumatology practices.
Schedule:
Monday through Friday ~OR~ Tuesday through Saturday

Compensation:
Up to $100,000/yr, DOE!
Qualifications
New Grads are welcome to apply
License to practice as a Physical Therapist in the state of WA is required
Outpatient and/or Acute Care experience preferred

The Physical Therapist (PT) is responsible for PT evaluations and development and implementation of the PT plan of care in accordance with PACE policies. The PT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The PT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The PT assumes responsibility for participants treated by the PTA.
