The SocialWorker is responsible for coordinating the facility placement, community agency referral and social coping aspects for 100% of the assigned patient populations. They will serve as a direct provider, referral source and resource to other staff in an effort to promote the delivery of high quality, fiscally responsible continuing care services. The SocialWorker will provide professional social work services to patients, families, and/or caregiver(s), which reflect the biopsychosocial needs of the patient and enable him/her to achieve an optimal level of health in accordance with social work practice and hospital policy and procedures. The SocialWorker will perform other related duties as required or directed.

ACCOUNTABILITIES
Initial Assessment and Discharge Plan: Complete initial assessment within 24-48 hours (or 72 hours in Michigan) and compile discharge plans reflecting clinical and social needs, in collaboration with the patient, family, and medical staff.
Multidisciplinary Collaboration: Participate in treatment team meetings, provide recommendations for discharge amendments, and gather necessary data from the patient and family for effective discharge planning.
Discharge Placement and Resources: Initiate discharge placement options, educate patients and families on facility placements, and liaise with community mental health centers and other agencies.
Patient Counseling and Education: Provide individual and group counseling, facilitate at least three weekly patient educational meetings, and assess patients' needs, including financial limitations for referrals.
Safety and Crisis Management: Assess patients' suicidal/homicidal potential, take appropriate actions, and implement seclusion and restraint procedures as needed according to department policy.
Documentation and Communication: Maintain accurate documentation of patient interactions, ensure timely completion of necessary paperwork, and engage in ongoing communication with relevant departments.
Professional Development and Leadership: Stay current with social work standards, develop in-service programs, and provide leadership and consultation to staff, patients, families, and community agencies.
Customer Satisfaction and Advocacy: Address customer satisfaction issues, promote continuity of care, and advocate for patients' and families' rights, ensuring a positive, professional role model within the team.
JOB REQUIREMENTS
Education: Bachelor's Degree in Social Work or related field from an accredited institution.
Skills: Extensive knowledge of the community and discharge planning . Strong interpersonal skills to work independently. Knowledge of community resources and/or federal regulations.
Years of Experience: 3-5 years of experience in a comprehensive inpatient Psychiatric Unit (or comparable work experience).
License: L.S.W., MSW or Professional Counselor from the State of Ohio. Michigan-BSW from the State of Michigan
PREFERRED REQUIREMENTS
Certification: Certified in Nonviolent Crisis Intervention.

Integrated Services for Behavioral Health (ISBH) is a community-minded, forward-thinking behavioral health organization helping people along the road to health and well-being. We meet people in their homes and communities and help connect them to the resources they need. We serve Southeastern and Central Ohio with a comprehensive array of behavioral health and other services working with local partners to promote healthy people and strong communities. All of our services are intended to be collaborative and personalized for the individual.
We are seeking a full-time licensed counselor or socialworker to serve youth and adults in need of Behavioral Health services.
Licensure as a Counselor (LPC/LPCC) or SocialWorker (LSW /LISW) is required. Independent licensure is strongly preferred.
The salary range for this position is based on experience, education, and/or licensure:
Dependently Licensed: $22.29 - $26.82/hr.
Dependently Licensed, Seeking Independent Licensure: $24.93 - $29.20/hr.
Independently Licensed: $61,174.73-$71,675.95
Essential Functions:
Assesses or defines the strengths and needs of referred individuals and families.
Directly provides and coordinates clinical and social services that are problem focused and build on family and community strengths.
Carries out utilization review and quality assurance activities as directed.
Maintains necessary documentation, participates in program evaluation, attends team and program planning meetings, cross-systems training, and acquires knowledge of community resources.
Meets billing productivity requirements established by Integrated Services for Behavioral Health.
Other duties as assigned.
Minimum Requirements:
Must meet requirements for licensure as defined by the Ohio Counselor, SocialWorker, and Marriage & Family Therapist Board. (LPC, LPCC, LSW, LISW)
Demonstrated high degree of cultural awareness.
Experience with multi-need individuals and families.
Broad knowledge of community service systems.
Willing to participate in and lead cross-systems team-building activities.
Able to effectively communicate through verbal/written expression.

ESSENTIAL FUNCTIONS:
Apply psychotherapeutic techniques and interventions in the delivery of services to individuals and families for the purpose of treating such diagnosed emotional and mental disorders.
Conduct individual, group or family therapy as appropriate for the treatment plan of the patient.
Conduct a psychosocial assessment and develop an initial discharge plan.
Provide initial treatment planning for patient based on assessment and coordinate any additional services needed and revise as necessary.
Maintain documentation of therapy services provided to each patient according to facility and professional standards, revising as necessary.
Maintain a caseload as assigned.
Participate in treatment team meetings and collaborate with other staff to facilitate services for the patient as appropriate to their treatment plan.

Job Description Summary:
Responsible for the delivery of social work services for a variety of patients including infant, pediatric, adolescent, adult, and geriatric populations to all medical areas of the hospital. Conduct brief solution focused/strategic clinical interventions utilizing motivation interviewing.
Complete legal paperwork to purse involuntary psychiatric hospitalizations and probate court guardianship hearings. Collaborate with hospital attorney and testify when required.
Specialized knowledge of chronic illness and the impact on patient mental health.
Knowledge of community service organizations (i.e. CMH, DHHS, shelters, transportation, etc.) and the types of assistance they provide.
Work by consult (i.e., physician, RN, patient, family, etc.).
Participates in multidisciplinary rounds and provides necessary report to advance patient care.
Collaborates with other members of the patient care team to understand each situation from different perspectives and recommends or provides immediate treatment and solution focused interventions.
Identifies needs, develops plan of care, and makes referrals to community agencies and other healthcare specialists and plans for patient discharge to ensure the appropriate follow-up treatment and support is available and provided to support a safe discharge.
Thoroughly documents each case and enters appropriate information in patient's electronic medical record in an effective and timely manner.
Participates in the development of departmental policies, procedures, and protocols.
Identifies and provides for learning needs of patients, families, and health care co-workers.
