Department: Administration Hours: 40 Shift: Days (United States of America) As the Pediatric Primary Care Social Worker, you will provide proactive care management for high-risk pediatric patients and their families by addressing psychosocial issues that may affect medical care and health outcomes.
You will interview pediatric patients, families, and caregivers to assess personal and social adjustments and look for any needed interventions to mitigate risks. Once identified, you will deliver interventions and connect patients to appropriate resources.
In this role, you will focus on supporting families in managing complex conditions hindering overall well-being.

REQUIREMENTS
* Education: Master's Degree in Social Work
* Skills: Exceptional problem-solving, critical thinking, organizational, interpersonal, and written/verbal communication skills. Ability to work in self-directed environment with attention to detail, follow through, and has a high level of computer literacy. History of positive rapport with patients, families, physicians, and interdisciplinary team.
* License: Active LISW license

* Years of Experience: Experience in a medical or other multi-disciplinary agency is desirable, and experience that matches the assigned practice specialty.

Skills and Experience Required:
Master's degree (MSW) from a school of Social Work accredited by the Council on Social Work Education

The role
The Medical Social Worker (MSW) is a qualified professional who, in accordance with the plan of treatment, assists the physician and other team members in understanding the significant social and emotional factors related to the health problems.
Key Responsibilities:
Provide social casework as an entity, where appropriate, to individuals and families receiving nursing or other services from the Agency
Assess, when appropriate, a family's financial situation taking into consideration the patient's prognosis and medical needs, and referring to an Agency for financial assistance if indicated, interpreting the medical situation to the referring Agency and generally facilitating the process of referral
Respond to referrals for casework by Agency staff or professionals from outside the Agency providing service as appropriate

Refer patients and families to community agencies with appropriate follow-up
Participate in case conferences with other agencies
Interpret social resources to staff and health services to special agencies

Skills and Experience Required:
Master's degree (MSW) from a school of Social Work accredited by the Council on Social Work Education

Department: Care Navigation Acute Hours: 20 Shift: Days (United States of America) As the Social Worker/Discharge Planning Coordinator, you will offer supportive intervention to patients and caregivers and coordinate and facilitate the development of a discharge plan of care for high-risk patients.
You will assess psychosocial risk factors for patients and caregivers and provide emotional, social, and financial support by connecting them to community resources.
You will serve as a patient advocate during hospitalization, ensure continuity of care, and address psychosocial concerns such as substance abuse, mental health, and guardianship.

REQUIREMENTS
* Education: BSW or higher degree; will consider a limited license SW on individual basis.
* Skills: Must have knowledge of regulations, requirements, and community resources. Must be able to function effectively in a critical care environment. Written and verbal skills are essential. Must be able to establish priorities and communicate and respond to inquiries. Excellent interpersonal communication and negotiation skills. Critical thinking and problem-solving skills. Customer service skills. Ability to work and communicate with people of all social, economic, and cultural backgrounds; be flexible, open-minded, and adaptable to change. Effective organizational skills. Computer proficiency with Outlook e-mail and electronic medical records. Flexible in a complex and changing healthcare environment. Understanding of pre-acute and post-acute venues of care and post-acute community resources. Maintains a current working knowledge of services available in the local community, particularly services available to patients with limited or non-existent payment resources.

* Skills: Applicable experience in patient advocacy, care management, and knowledge of hospital and community resources.
* Years of Experience: Previous case management or discharge planning experience.
