The primary goal is to increase the efficiency and productivity of the physician. It will be the responsibility of the Medical Scribe to annotate any dictated or written information for the treatment of patients following all local, state and federal guidelines for documentation into the electronic health record system. The Medical Scribe will be responsible for clearly recording each patient's symptoms, history, physical exam and documenting the diagnosis. This position will have no direct involvement in patient care but has an interdepartmental role in assisting nursing and front office staff.JOB DUTIES AND RESPONSIBILITIES The primary role of the Medical Scribe is to assist the clinician with documentation of each patient's medical chart during his or her visit. The medical scribe annotates any dictated or written information for the treatment of patient following all local, state, and federal guidelines for documentation into the electronic health record (EHR). The medical or specialist scribe will be responsible for clearly recording each patient's symptoms, history, physical exam, and documenting the diagnosis as well as treatment plans. The medical scribe is not a direct patient care role. This positions requires working at different clinic locations in Marin County (San Rafael, Larkspur and Greenbrae), clinics are within close driving distance of each other. Responsibilities Scribe-Specific Tasks and Duties: The primary role of the scribe is to assist the provider with documentation of each patient's clearly recording each patient's symptoms, history, physical exam, and documenting the diagnosis as well as treatment plans in the medical record during a clinic visit. At the start of the visits scribes checks the “document scribed” checkbox on the intake template at the top center. Under the direction of the provider, transcribes and places patient orders, including laboratory tests, radiology tests, referrals, etc. Documents any procedures performed by the provider or nurses in the EHR. Transcribes any consultation or discussions with family and/or the provider. Completes the patient's chart by transcribing results of any labs, x-rays, or other evaluations. In doing so, the scribe continuously checks on the progress of this data in order to get the patient's workup complete so that the provider is able to make his/her decisions regarding that patient. Scribe lists all proper diagnoses as well as any follow up instructions and prescriptions, as dictated by the provider. Scribe does not directly assist with patient care. The scribe documents orders generated by the provider. Scribe assists the provider and support staff with paperwork, telephone calls, faxing, and chart abstraction and may facilitate communication among the care team. Scribe is able to process portal communications and either task to appropriate member of the care team or get information from the provider to respond to the patient in a timely way. The Medical Scribe does not perform the following: Medications: A scribe cannot prescribe medications. Scribes cannot stop, start, or make any manipulation to any medications in the "medication module”. Orders: A scribe cannot order any in-house medications that are injected, ingested, or inhaled into the body including nebulizer treatments. Document Sign Off: The provider should always review the record and attest to its correctness and completeness. At the end of the day or shift, the provider should review the scribed documentation to ensure all events were recorded accurately. The provider generates the document, creating the completed "Master IM." The scribe does not. Patient Care: The scribe is not directly involved with patient care; can't touch the patient; can't handle bodily fluids; can't act independently without direct clinical oversight; conduct other duties while acting as the medical scribe; however, he/she may be asked to give patient paperwork during visit or discuss discharge instructions with a patient. General Documentation: A scribe cannot document any of his/her personal objective findings or be involved in any of the medical decision-making. Marin Community Clinics, founded in 1972, is today, a multi-clinic network with a wide array of integrated primary care, dental, behavioral, specialty and referral services. The medical scribe at Marin Community Clinics (MCC) is an integral part of MCC's team care model, enhancing the quality and efficiency of our patient care. The medical scribe annotates any dictated or written information for the treatment of patient following all local, state, and federal guidelines for documentation into the electronic health record (EHR). The medical or specialist scribe will be responsible for clearly recording each patient's symptoms, history, physical exam, and documenting the diagnosis as well as treatment plans. The medical scribe is not a direct patient care role. This positions requires working at different clinic locations in Marin County (San Rafael, Larkspur and Greenbrae), clinics are within close driving distance of each other. Work schedule: Monday through Friday 8:00am to 5:00pm, evening shifts of 11:00am to 8:00pm and Saturday shifts with advanced notice. Responsibilities Scribe-Specific Tasks and Duties: The primary role of the scribe is to assist the provider with documentation of each patient's clearly recording each patient's symptoms, history, physical exam, and documenting the diagnosis as well as treatment plans in the medical record during a clinic visit. At the start of the visits scribes checks the “document scribed” checkbox on the intake template at the top center. Accompanies the clinician into the patient examination area, or joins the appointment remotely by telephone, in order to transcribe a history and physical examination as given by the patient and provider. Under the direction of the provider, transcribes and places patient orders, including laboratory tests, radiology tests, referrals, etc. Documents any procedures performed by the provider or nurses in the EHR. Transcribes any consultation or discussions with family and/or the provider. Completes the patient's chart by transcribing results of any labs, x-rays, or other evaluations. In doing so, the scribe continuously checks on the progress of this data in order to get the patient's workup complete so that the provider is able to make his/her decisions regarding that patient. Scribe lists all proper diagnoses as well as any follow up instructions and prescriptions, as dictated by the provider. Scribe does not directly assist with patient care. The scribe documents orders generated by the provider. Scribe assists the provider and support staff with paperwork, telephone calls, faxing, and chart abstraction and may facilitate communication among the care team. Scribe is able to process portal communications and either task to appropriate member of the care team or get information from the provider to respond to the patient in a timely way. The Medical Scribe does not perform the following: Medications: A scribe cannot prescribe medications. Scribes cannot stop, start, or make any manipulation to any medications in the "medication module”. Orders: A scribe cannot order any in-house medications that are injected, ingested, or inhaled into the body including nebulizer treatments. Document Sign Off: The provider should always review the record and attest to its correctness and completeness. At the end of the day or shift, the provider should review the scribed documentation to ensure all events were recorded accurately. The provider generates the document, creating the completed "Master IM." The scribe does not. Patient Care: The scribe is not directly involved with patient care; can't touch the patient; can't handle bodily fluids; can't act independently without direct clinical oversight; conduct other duties while acting as the medical scribe; however, he/she may be asked to give patient paperwork during visit or discuss discharge instructions with a patient. The scribe can't give orders to Registered Nurses. They interact with patients on a limited level. For exceptions to this section, please see the “Optional Trainable Skills” section below. General Documentation: A scribe cannot document any of his/her personal objective findings or be involved in any of the medical decision-making. A scribe cannot act independently; instead, he/she must simply document the provider's dictation and or/activities during the visit. Optional Trainable Skills: With the permission of medical/operational leadership and participation in an in-house training/competency program, a Scribe can be trained to perform vitals, room patients. MISSION The mission of Ravenswood Family Health Network (RFHN) is to improve the health of the community by providing culturally sensitive, integrated primary and preventative health care to all, regardless of ability to pay or immigration status, and collaborating with community partners to address the social determinants of health. POSITION SUMMARY The Medical Scribe assists the provider with medical documentation to increase the efficiency and productivity of the clinical visit. The Medical Scribe documents each patient's symptoms, history (HPI), physical exam and diagnosis in the electronic health record (EHR), and dictated or written information for the treatment of patients. The Medical Scribe does not provide direct patient care and follows local, state, and federal guidelines in an interdependent role in assisting clinical and front office staff. The bilingual Medical Scribe will communicate, translate and interpret with patients and providers in Spanish. DUTIES AND RESPONSIBILITIES To be performed in accordance with RFHN Policies and Procedures * Reviews charts with the provider prior to patient visits to identify preventive care to be addressed during the visit. * Documents the history and physical exam findings during the patient visit as communicated in the patient and provider encounter. * Documents all procedures performed by the provider or clinical staff in the EHR. * Uses templates and work-flow documentation tools effectively to ensure that encounters are completely documented as well as enabling the collection of data for statistical reports. * Transcribes the consultations and discussions between family members and the provider. * Enters the results of labs, x-rays, or other evaluations into the EHR. * Monitors the progress to completion of the patient's work-up data to support the provider's decisions regarding the treatment plan. * Documents confirmed diagnoses as well as follow-up instructions and pends orders for diagnostic imaging, labs, referrals, and prescriptions, as dictated by the clinician. * Facilitates the consistent and complete use of PHQ9 or PHQA, drug alcohol tobacco brief screen, and any other screenings assigned to medical scribes. * Facilitates medication reconciliation. * Distributes "Reach out and Read" books. * Completes documentation on the patient record by end of regular scheduled work hours. Assesses for barriers to obtaining services (transportation, access, etc.) and follows up on what resources the patient might need to obtain screening. * Adheres to the scope of the role of medical scribe when documenting the patient's response to the provider's questions: a. Collects patient HPI data only while the provider is in the room, except for screening tools. b. Documents patient history as obtained by the provider; history is not taken independently. c. Documents the review of systems independently and records patient responses to provider questions. * Completes documentation of the physical exam, assessment and plan with the direction of the provider. The medical scribe role responsibilities do not include or permit medical decision-making. * Seeks clarification on details of the physical exam as needed and verifies they are choosing the correct assessment as it relates to lab, diagnostic tests, referrals, procedures, and other orders. They will review the chart completely with the provider. Ensures that correct billing codes are applied before submission of the record. * Addresses immunization and health maintenance screening gaps in care under the direction of the provider. * Medical scribes may not interpret lab results or explain lab results to a patient. * Completes medication reconciliation and may prepare orders for the provider to send to the pharmacy. * Schedules follow-up, lab, imaging, and vaccine appointments for patients at the request of the provider. * Drafts letters for patients under the express direction and on behalf of the provider for work/school excuse, surgery, and dental clearance, etc. * Responds to in-basket tasks including refill requests, lab results follow-up, and patient calls only at the request of the provider and only in the capacity to order requested labs, referrals, pass along instructions, or schedule lab or follow-up appointments per the provider. * Translates or interprets for patients or patient's family members present. * Other duties as assigned and requested. Qualifications QUALIFICATIONS * High School Diploma or equivalent (GED) required. * Bilingual Spanish/English proficiency required. * Working knowledge of medical terminology, anatomy, physiology required. * Coursework or degree in health sciences preferred. * One-year experience working in a healthcare setting; medical scribe experience in primary care strongly preferred. * Excellent interpersonal, oral and written communication skills. * Strong organizational, problem-solving, and analytical skills; able to manage competing priorities and workflow. * Proficient in data entry and Microsoft Office programs; experience with Epic electronic health record preferred. * Able to travel to all clinic locations and work flexible hours that may include evenings and Saturdays. * Current California drivers' license and proof of auto insurance required. * Current Basic Life Support (BLS) certification. * Keyboard skills 55 WPM or greater. * Up to date with COVID-19 vaccines per current CDC guidelines strongly recommended. The pay range for this position is $26.00 to $34.00 per hour. However, the final base salary will be determined upon a number of individualized factors such as (but not limited to) the scope and responsibilities of the position, job-related knowledge, skills, experience, education and certification levels, and departmental budget. The Phlebotomist performs venipuncture procedures for clinical or medical testing and transfusions. Procures blood samples from patients and labels accurately to preserve the integrity of specimens for proper transporting, processing and preparation of all samples. Reviews lab requisitions and performs quality review and troubleshooting. Ensures adequate supplies and inventory, and performs all clerical functions including but not limited to: data entry, maintaining records, answering phones and providing reports. Provides support and facilitates communications by effectively navigating computer and phone systems, building and maintaining customer relationships, and providing a positive, professional and empathetic understanding to all customers. Identifies patient, observes the correct procedure for venipuncture, collects and labels specimens exactly as outlined in each medical requisition. Accurately prints collection batch and labels for phlebotomy collections. Adheres to infection control standards, properly handles each specimen and ensures that every specimen is delivered to the laboratory in a timely manner per laboratory protocol. Accurately processes specimens for send-out according to appropriate procedures ensuring timely processing/dispatch.|Actively participates in departmental process improvement.
