We are seeking a compassionate and experienced Advanced Illness Management (AIM) Palliative Registered Nurse (PRN) to join our team. Reporting to the Director of Palliative Care, this role provides high-quality palliative care to patients in their homes, assisted living facilities, and skilled nursing facilities. The AIM Palliative RN collaborates with patients, families, and interdisciplinary team members to assess, educate, and coordinate care while ensuring comfort and quality of life.
Schedule: 1-3 days/week; some after hours coverage - Addison Area
Spanish Bilingual a plus
Essential Functions of Position:
Conduct in-person and telephonic RN assessments and screenings for palliative care patients.
Complete Risk Stratification Assessments to identify patient support needs in consultation with the Nurse Practitioner.
Observe patient visit frequency based on individual needs and risk identification.
Educate patients and families on disease processes, self-care, end-of-life care, medications, nutrition, and treatments.
Communicate findings to the Palliative Nurse Practitioner and/or PCP, including changes in condition or signs of decline.
Coordinate care with palliative team members and support hospice referral as appropriate.
Document timely clinical records including assessments, progress notes, care plans, physician orders, and treatments.
Support care plan development from initial assessment through discharge, addressing functional decline, symptom management, nutrition, cognition, and family/caregiver needs.
Collaborate on modifications to individualized palliative plans of care.
Facilitate care delivery and obtain additional resources as needed in accordance with the Plan of Care.
Participate in evening/weekend on-call rotation as required.
Facilitate prescription transmission to pharmacies, manage lab and diagnostic results, and request durable medical equipment as medically necessary.
Maintain effective relationships with supportive care team members and other healthcare providers.
Communicate and document clinical findings, treatment plans, and care provided in accordance with nursing standards and regulatory requirements.
Adhere to company policies, HIPAA privacy standards, business ethics, and mandatory compliance programs.
Implement marketing and promotional initiatives as directed.
Perform other related duties as assigned.
About You: Required Qualifications:
Active and unrestricted Registered Nurse (RN) license with no board actions
Minimum 2 years of clinical RN experience, preferably in skilled nursing, home health, palliative care, hospice, geriatrics, or oncology
Demonstrated ability to provide quality care in high-empathy environments
Proficiency in Microsoft Outlook, Word, Excel, and electronic medical records
Exceptional communication, time management, and critical thinking skills
Keen observation skills with the ability to identify changes in condition and escalate appropriately
Comfortable working independently in the field and within interdisciplinary care models
Valid driver's license and reliable transportation (field-based role)
Preferred Experience:
Previous experience in palliative care, hospice, or oncology nursing
Familiarity with risk stratification, symptom management, and home-based care planning
Experience working in or with assisted living and skilled nursing facilities

Job Summary
The Facilities Maintenance Attendant is responsible for the upkeep, repair, and overall cleanliness of the golf club's facilities, ensuring all areas meet the highest standards of functionality, safety, and appearance. * Maintain carpets clean by vacuuming and spot cleaning where necessary. Keep all floors swept, mopped, and polished, including courts, exercise rooms, stairwells, lobby and hallway areas, restrooms, and any other areas needed.
* Dust and wipe down furniture, shelves, chair arms, desks, credenzas, chandeliers, pictures, televisions, woodwork, and artwork, including all ceiling HVAC registers.
* Responsible for seeing that daily assignments are completed in their respective areas, to meet club standards and as communicated expectations (i.e. orderly/organized work areas, straighten/arrange entry area, picking up trash/debris, etc.).
* Perform or contract facility maintenance and housekeeping repairs and maintenance including, but not limited to electrical, plumbing, HVAC systems, building equipment, carpentry, flooring, walls, ceilings, special projects, and other related areas.
* Responsible for maintaining a maintenance and project checklist/schedule for preventive maintenance and ongoing improvements.
* Communicate effectively with supervisor to coordinate, prioritize, document, and complete work requests from Department Heads, members, and guests.
Additional Duties
* Due to the dynamic nature of the club, employees are expected to assist others as needed and take on additional tasks as assigned by management.

Drawing on UNT's trail blazing heritage as the first graduate program in the nation to be accredited by the Association for Behavior Analysis, and housed in UNT's College of Education, the UNT Kristin Farmer Autism Center (KFAC) provides clinical services to support learning, development, community participation, and quality of life of individuals on the autism spectrum. What we are looking for
We are seeking a highly motivated individual with a positive attitude to coordinate adult 50+ educational classes and trips at our center. Hours Required: Part Time, Candidates must be able to work evenings and weekends At TOCA, our Match Captains are the MVPs of our leagues. You're not just calling fouls-you're calling the shots to make sure every game is a total win for fun, excitement, and fair play. Love soccer? Love high-energy vibes? Then lace up your boots, because we're looking for YOU!
Your Game Plan (Role Breakdown): Game Day Playmaker (60%)
Be the captain of the match experience-hype up the energy, make sure the fun's off the charts, and give players the TOCA vibe they'll never forget.
Engage with players on the pitch, explain rules, and keep everything running smooth like a Messi dribble.
Keep games fair and flowing by enforcing the rules, but don't forget-fun comes first!
Player & Guest Experience Coach (25%)
Be the friendly face of TOCA-greet players, keep things positive, and make everyone feel like a part of the team.
Share the scoop on upcoming games, leagues, and events. Make sure our soccer community is as tight as your ball-handling skills!
Stat Tracker & Gear Guru (15%)
Keep track of game time, player attendance, and those all-important stats.
Make sure equipment is game-ready-no deflated balls or mismatched jerseys on your watch!
