Manage acute and chronic illness collaborating as needed with the PCHC M.D. and assist in coordinating care for these patients with other members of the health center and/or with services outside of the health center. Provide patient education and preventive medicine as clinically indicated. Provide counseling and support as clinically appropriate, in areas of social problems recognizing the emotional component of illness, making referrals to mental health professionals as needed.     General Responsibilities: The faculty nurse practitioner is responsible for providing primary medical care, within the limits of the training of a nurse practitioner, to assigned patients. The Primary Care Nurse Practitioner Fellowship program is an accredited program through the Consortium for Advanced Practice Providers to provide recent NP graduates with on-the-job training to gain confidence and competence to start a successful career. The fellowship consists of three core elements: precepted clinic, didactics, and specialty rotations. Fellows are precepted through the primary care experience by expert MDs and NPs that are fully dedicated to their training and patient panels. Fellows also participate in weekly post-graduate didactics sessions which are CME approved and focus on case studies observed in clinic. Fellows participate in a weekly specialty rotation to expand the breadth of the knowledge that they bring back to primary care. Must be able to complete assignments in variable weather conditions and ever-changing and sometimes allergenic environments in participant homes, including extreme heat, humidity, dust, smoke, pet contamination, and cleaning supplies.
