Duties and Responsibilities
Provide counseling sessions to assigned patients
Complete all required documentation accurately and timely
Performs initial assessment and on-going care
Participates in the development of individualized care plans
Addresses co-occurring disorders
Work as a member of the multi-disciplinary treatment team
Coordinate with Case Managers and patient support staff
Connect patients to internal/external resources as appropriate
Performs other duties as assigned
Requirements
Must have an active registration in good standing with the State of California as a Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselors (LPCC) or Clinical Psychologist.
Work Environment
FHCSD's services are all outpatient and designed for those struggling with mild to moderate acuity levels. You will have support from other therapists, case managers, a scheduling team, administrators, and a wide range of other medical professionals in our facilities. The Mental Health Department strives to create an inclusive community that fosters teamwork and continuous learning. In addition to a structured orientation process, all newly hired therapists participate in our Buddy Program so you have a designated peer to help you adjust and succeed in your new position.
Our History
FHCSD is a 501(c)(3) nonprofit organization rich in history and tradition. In 1911, the San Diego Industrial School was built on the site where our first clinic now stands. This school eventually transformed into a settlement house that provided Barrio Logan's immigrant residents with dearly needed social services. When it was announced that the city was closing the building in October 1970, Laura Rodriguez and a group of community members attended a Barrio Logan Community Action Committee meeting where they passionately advocated that the location should continue to function as a community health clinic. When their request was quickly rejected, the group occupied the building in protest.
After eight days of negotiations the City of San Diego and the community leaders reached a resolution. This led to the gifting of the building to the people of Barrio Logan and the establishment of a free clinic. Medical care delivery began two evenings a week, and the health center saw approximately 150 patients each month. This building became our flagship clinic, Logan Heights Family Health Center, and we're proud to honor our heritage by continuing to increase access to quality health care in San Diego. Over the past several decades FHCSD has transformed into one of the 10 largest Federally Qualified Health Centers in the country and will continue to grow while we strive to meet the needs of our community.

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth. The Marriage and Family Therapist will be required to provide, evaluate, and develop individualized plans for individuals, families, and teenagers. Primary Duties:
Report directly to Clinical Supervisor.
Provide treatment in individual and group settings for individuals and families of individuals with substance use disorder.
Provide regular feedback regarding client progress to third-party monitors.
Handle phone inquiries from community members requesting information about outpatient chemical dependency treatment.
Maintain up-to-date, accurate progress notes and client records on personal caseload.
Evaluate and assess potential patients for appropriateness for outpatient treatment.
Participate in contacting community referral sources and disseminating information about outpatient chemical dependency treatment and the neurobehavioral model.
Attend regular training sessions and other events as required.
Do whatever is necessary to maintain a successful, professional, and effective clinic site.
Adhere to personnel policies and clinical protocols.
Orient new patients to the program rules, policies, procedures, and patients rights.
Conduct Intakes, Transition/Discharge Planning, Initial ASAM Assessment and Initial Tx Plans and Tx Plan Updates with patients in a professional manner and within regulatory guidelines.
Recognize and apply patients Strengths, Needs, Ability, and Preferences (SNAP), to develop and implement individualized treatment plans that effectively help patients reach their short- and long-term goals.
Providing outside referrals for specialized services and conduct follow up.
Reviewing Physician notes following intake or other medical appointments. Including any high-risk factors in patient treatment plan.
Refer high-risk patients to clinic Physician as needed (e.g., new pregnancy, medication instability, other reported ailments).
Scheduling individual and group counseling sessions with patients, documenting rescheduled or missed appointments. Reviewing missed counseling appointment (hand out) with patients.
Engaging in community relations and outreach efforts when appropriate.
Collect research data when requested.
Present clients and participate in case conferences.
Have documentation signed within designated time frames as determined by policies. and procedures as well as regulatory requirements.
Adopt a team approach concept and collaborate with clinical and medical staff to ensure quality patient care.
Proactively ensure that clients have enough counseling as appropriate for their condition, including proactively working with front office and medical staff to see patients for counseling when they are in the clinic for dosing.
Perform diverse tasks or other duties as assigned by Clinic Supervisor.
Minimum Requirements:
Graduate level, behavioral health degree or extensive history of working in the chemical dependency field as a counselor.
Must be licensed through the California Board of Behavioral Sciences or certified (or registered) by a California recognized drug and alcohol certifying body.
Prior training in counseling or substance use disorder treatment.
History of performing in a professional capacity.
Willing to follow Revive Pathway personnel policies.
Able to work the hours and days required by position, including daily 5:30 am start times.
Be willing to learn and able to deliver the Revive Pathway model of outpatient chemical dependency treatment.
Ability to provide health and TB clearance.
Must pass background check and urine drug screen.
Read understand and comply with Revive Pathway Code of Ethics.
Observe proper chain of command and reporting processes.
Report inappropriate behavior and misconduct of other staff.
Maintain a non-smoking, safe, healthy, pleasant, and orderly working environment.
Maintain required daily work schedules, punctuality and reporting of absences or scheduled time off in accordance with Revive Pathway policies and procedures.

More specifically, our masterful Massage Therapist:
Performs exceptional massages and body treatments following award winning protocols
Fully prepares each and every treatment based on our treatment criteria
Educates the guest on the benefits of the service performed and recommends products for home use
Talks up all the other great services we offer and provides enhancement treatments
Has a most recent and up to date massage license (VA please)
Is a steward of the Woodhouse Brand making sure all treatment areas are prepared to the Woodhouse standards
Always improving skills by attending continuing education classes

This key role on our healing arts team provides care and compassion in addition to healing through touch. We are interested in someone with exceptional skills, deep experience providing bodywork services, and certification in Esalen Massage.
KEY RESPONSIBILITIES
Provide excellent bodywork services
Work 2 days per week (Saturdays and Sundays), 3-4 sessions per day at specific session times agreed upon with Healing Arts management
Conduct self in a professional manner with clients and co-workers
Keep work area clean and professional
Keep all paperwork current: permits, licensure, and insurance
Adhere to Healing Arts department and role guidelines, expectations, and policies
Develop and maintain positive working relationships with coworkers and peers
Be responsive to management
Other various tasks as assigned
QUALIFICATIONS
Certified in Esalen massage with at least 2 years of professional massage experience preferred
Massage license certification through CAMTC
Proof of current liability insurance policy
If new to the Esalen Healing Arts crew, must give 4+ evaluation sessions to designated Healing Arts staff
A passion for exceptional service
Ability to provide references and appropriate documentation of experience
Good listening and communications skills
High level of client relations experience, supporting a safe environment and a caring experience for our guests, following ethics for practitioner best practices
Embrace a strong work ethic and teamwork focus, with flexibility and willingness to support others and maintain a professional workplace
Possess a good understanding of the physical body, how to evaluate, assess, and support client needs

*Based on a 1.5 hour session rate. Does not include tipping. Set-up before and clean-up after hourly rate is minimum wage of $16.50/hour.
