JOB PURPOSE: Provides cleaning services to provide a safe sanitary, comfortable and homelike environment for residents, staff and the public. Housekeeping services provided are in accordance with facility policies and procedures and consistent with state and federal laws and regulations to maintain an environment that enhances the quality of life for residents
KEY RESPONSIBILITIES:
1. Sweeps, damp mops, and wet mops tile floors. May occasionally be required to spray buff, strip or wax floors, following center procedures, including the use of wet floor signs.
2. Vacuums carpeted rooms as applicable.
3. Empties trash cans, cleans exterior, cleans interior, and replaces with a clean can liner
4. Cleans bathroom sinks, fixtures, and toilets, following center procedures.
5. Cleans glass surfaces such as mirrors, pictures, and windows.
6. Cleans behind and underneath furnishings.
7. Follows infection control standards while performing cleaning.
8. Prevents/removes wax/dirt buildup in corners, around baseboards, and in doorways.
9. Changes soiled privacy curtains and draperies for the outside windows as needed.
10. Removes and re-hangs curtains and draperies for the outside windows, using a ladder.
11. Dust/damp cleans surfaces in patient/resident rooms (furniture, bed frames, windowsills, etc.) following center procedure, including the use of wet floor signs.
12. Moves furniture to clean behind/underneath, according to center procedures.
13. Turns bed mattresses after cleaning.

Cook - Dietary Aide HCC - 2516104 Description JOB PURPOSE: The Dietary Aide Assists with food preparation in accordance with our established procedural guidelines, and as directed by the Food Service/Dietary Manager/Supervisor. The Cook prepares palatable, nutritionally sound meals consistent with departmental policies and procedures and consistent with state and federal laws and regulations that meet daily nutritional and special diet needs of each resident. Responsible to ensure that foods are prepared and served at the proper temperature and the proper time. Works with Director of Food Service to allocate resources in an efficient manner so that each resident receives food in the amount, type, consistency and frequency to maintain acceptable body weight, nutritional values, and quality of life.
KEY RESPONSIBILITIES:
1. Assists cook in the preparation of meals, salads, desserts, snacks or other dishes for patients/residents, also prepares between-meal nourishments (make sandwiches, etc.)
2. Follows diet orders and NPO diet orders
3. Serves nourishments in accordance with procedural guidelines
4. Properly stores leftovers/ opened food
5. Follows procedures for serving partner meals
6. Offers menu substitutions and records them in accordance with procedural guidelines.
7. Complies with infection control policies in the work area
8. Delivers trays to patients/residents on halls and in dining area
9. Assists with the serving/pouring of juice, milk, water, and tea, etc.; makes coffee/ tea as requested
10. Cleans all tables in the dining area after each meal
11. Cleans work area at the end of shift and after meals, including sweeping and mopping and stores all cleaning equipment appropriately
12. Cleans kitchen equipment such as, carts, tables, counters, ice machine, buckets, blender, mixer, meat slicer, freezer, refrigerator, stove, steamer, garbage disposal, dish machine coffee/ tea maker, steam table etc.
13. Removes garbage from kitchen areas and hoses out garbage containers
14. Scrapes dishes, washes dishes, pots and pans and stores properly
15. Records freezer and walk-in refrigerator temperatures in accordance with established procedures
16. Maintains dishwasher and dish sink; checks and records water temperature (dishwasher, pat and pan sink, etc.); checks dishwasher with sanitizer strips; checks chemical levels and replaces if needed
17. Maintains supply stock and properly stores items according to established procedures
18. Sets up tray line and performs tray line service
19. Operates the dish machine, garbage disposal, mixer, blender, meat slicer, steamer, coffee and tea maker
20. Uses, cleans and maintains serving equipment (ladle, spoon, scoops, etc.) according to established procedures
21. Working knowledge of food handling, preparation and storage techniques that comply with state and federal laws and regulations. Ability to apply regulations to work situations of cook and to supervision of staff assistants.
22. Follows recipes and prepares foods that correspond to menu cycles prepared by Dietitian. Cook or prepares palatable, attractive, nutritionally adequate meals in institutional quantities that are served at proper times.
23. Assists with supervision kitchen staff that assist with food preparation, assembling trays, cleaning and storage of supplies and equipment. Continuously cleans food service work areas as food preparation and service is done. Uses food supplies and equipment in an efficient and economic manner to prevent waste.
24. Prepares and maintains supply of food substitutes to accommodate resident choices, cultural, ethnic and religious preferences. Inspects trays following meal service to monitor and record resident acceptance of menu items. Reports findings to Dietary Manager.
25. Uses food preparation equipment according to manufacturer's instructions. Follows proper cleaning techniques and preventative maintenance schedules. Handles, stores, dispose of food supplies and prepared food in accord with department procedures and in compliance with state and federal regulations.
KNOWLEDGE, SKILLS, ABILITIES:
• Participate in center/agency surveys (Licensure / JCAHO) and any subsequently required reports.
• Attend and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.
• Attend and participate in mandatory in-services.

• Knowledge of emergency and disaster procedures of facility. Able to locate nearest exit, to understand and respond to written or oral instruction in case of emergency. Able to arrive and to begin work on time and to report for duty as scheduled on all shifts, weekends and holidays
• Working knowledge and ability to comply with facility policies and procedures for workplace safety including infection control procedures, application of universal precautions for blood borne pathogens, use of personal protective equipment and handling of hazardous materials
• Follow facility policies and procedures for infection control at all times to maintain a sanitary food preparation and storage area. Uses personal protective equipment when indicated. Follows facility procedures for resident trays that require special handling. Inspects food service work areas for evidence of rodents or pests, safety or fire hazards, housekeeping and maintenance problems.
• Corrects problem and/or reports problems promptly to Dietary Manager. Communicates with Dietary Manager throughout shift as needed to best meet needs of residents. Assists Department Head with preparation for state inspections. Attends survey training. Interacts with state surveyors as instructed by supervisor. Reports injury to self or other to supervisor immediately. Qualifications MINIMUM EDUCATION REQUIRED:
Sufficient education to demonstrate functional literacy
MINIMUM EXPERIENCE REQUIRED:
A minimum of one (1) year institutional food service experience as a cook.

9. Assesses the medical necessity/effectiveness of ancillary services to determine the appropriate initiation of benefit events and communicate the process to providers and appropriate team members
10. Coordinates care as member's transition through different levels of care and care settings, ensures that the ICP is communicated immediately and/or no later than 24 hours to the receiving care setting Qualifications
MINIMUM EDUCATION REQUIRED:
Graduate of an accredited Adult or Family Nurse Practitioner Master's Degree program or Physician Assistant Studies
MINIMUM EXPERIENCE REQUIRED:
Minimum 2 years clinical experience as a Nurse Practitioner or Physician Assistant in geriatric, adult or family practice setting. Proven ability to perform comprehensive adult/geriatric physical exams.
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Current and unrestricted, RN and NP licensure, or PA licensure in state of practice. Board certification by AANP or ANCC, in Adult, Family Practice and Geriatrics or Adult-Gerontology Primary Care. Current CPR certification
• Active DEA license
Must have an active national provider number (NPI) and an active Medicare provider number
Must be eligible to participate in the Medicare program as a provider and registered
ADDITIONAL QUALIFICATIONS: (Preferred qualifications)
Graduate of an accredited Geriatric, Adult or Family Nurse Practitioner Master's Degree program. Previous long term care experience strongly preferred.

6. Complete all regulatory required monthly, quarterly, and/or annual visits and acute visits as needed.
7. Maintain appropriate and compliant documentation within our EMR (electronic medical record) software
8. Ensures all diagnoses are ICD-10 coded accurately and documented appropriately to support the diagnosis at that visit
9. Reviews orders and interventions for appropriateness and response to treatment to identify the most effective plan of care that aligns with the member's needs and wishes
10. Participate in family conferences and provide advanced care planning and education when needed
11. Address any pharmacy recommendations or pharmacist communications regarding residents
12. Providers are expected to be present in assigned facilities during work hours to ensure that facilities have adequate face-to-face coverage to meet resident care and facility needs
13. Practice ethically and in accordance with the Scope and Standards of Practice of their profession and Board Certification
14. Rotating on-call responsibilities and after-hour coverage is required.

1. Coordinates clinical competency assessment and training needs of clinical staff, based on ongoing evaluation, patient needs and acuity.
2. Provides relevant and creative education for clinical staff on a variety of topics, focusing on development of clinical skills, critical thinking, and interpersonal skills of partners.
3. Participates in the interview and selection process of clinical staff.
4. Completes initial competency assessment of clinical staff during orientation process, annually and as needed.
5. Identified and trains clinical staff who will serve as preceptors for new partners, using train-the-trainer approach. Oversee preceptor program and meet with preceptor team on a regular basis, to ensure that organizational standards, policies and procedures are maintained.
6. Conducts competency assessments to determine partner learning needs based on patient conditions and acuity.
7. Meets regularly with DHS to review learning needs, coordinate training and to implement plans for education and follow-up.
8. Communicates regularly with unit managers regarding new partner orientation and competency status and ongoing educational needs of unit staff.
9. Reviews with DHS outcomes of case mix and clinical meetings, in order to identify clinical learning needs for partners.
10. Participates in performance improvement activities.
11. Serves as Pruitt University system administrator for facility, providing training for partners and promoting ongoing use of this education tool among all partners.
12. Maintains documentation of all partner education in accordance with regulations, policies and procedures.
13. Develops and direct facility orientation program and training for new employees including general orientation to facility policies, procedures, philosophy of care and regulations that govern long term care.
14. Communicates and interpret policies and procedures to nursing staff.
15. Monitors practice for effective implementation.
16. Establishes and implement infection control program designed to provide a safe, sanitary and comfortable environment and to prevent the development and transmission of disease and infection.
17. Conducts infection control surveillance activities to investigate, control and prevent infections in the facility.
18. Provides consultation to all departments on application of infection control principles and procedures to specific situations.
19. Conducts staff training and monitor performance particularly for proper hand washing, isolation techniques and universal precautions.
20. Implements workplace safety programs in the facility including hazard communication, occupational exposure to blood borne pathogens and required record keeping of occupational illnesses and injuries.
21. Maintains material safety data sheets and exposure control plan for blood borne pathogens readily accessible to all staff, 24 hours a day.
22. Coordinates Hepatitis B Vaccination Program and post exposure evaluation and follow up requirement.
23. Maintains confidentiality of employee medical records.
KNOWLEDGE, SKILLS, ABILITIES
• Computer skills including Microsoft Word required, Excel and Power Point preferred.
• Knowledge and use of Adult Learning Principles.
• Work history should document volunteer or paid teaching experiences including competency assessment of skills.
• Demonstrates excellent clinical assessment skills and ability to teach these skills to facility partners utilizing the principles of Adult Learning.
• Maintains personal clinical competency through continuous use of Pruitt University and any other sources needed to maintain and improve skills, including training required by organization.
• Participates in center/agency surveys (Licensure/JCAHO) and any subsequently required reports.
• Attends and participates in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification as required.
• Attends and participates in mandatory in-services.
• Honors patients/residents' rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.
• Complies with corporate compliance program.
• Reports job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.
• Follows established safety regulations, to include fire protection and prevention, smoking regulations, infection control, etc.
• Follows established safety procedures when performing tasks and/or working with equipment. Qualifications MINIMUM EDUCATION REQUIRED:
Nursing Degree from an accredited college or university, or is a graduate of an accredited/approved RN program.
MINIMUM EXPERIENCE REQUIRED:
Experience in implementing and maintaining policy-driven practices is highly desirable; history of working with state/federal regulations a plus.
Experience in providing patient, family and/or staff education strongly encouraged.
Recent acute care or LTC experience.
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Current and unrestricted Registered Nurse (RN) licensure in state of practice.
Current CPR (BLS) certification
Within six (6) months of hire successful completion of organizational training programs: IV Certification, Pain Management, Wound Care, Respiratory Care, and CCC Certification.
Within twelve (12) months obtain American Heart Association CPR Instructor certification.
ADDITIONAL QUALIFICATIONS: (Preferred qualifications)
CPR Instructor preferred. Certification by Nationally accredited Nursing Organization highly preferred (i.e. Gerontological Nursing, CRNI, or Certified Nurse Educator).
