This program focuses on women who have had no previous live births. Nurses complete nursing assessments utilizing the Nursing Process. Nurses provide education related to pregnancy, health, community resources, parenting, safety, child development and other topics as needed. * Prepares for and actively participates in reflective supervision, case-conferences and team meetings.
* Monitor and assess the health status of first-time mothers and infants to age two utilizing the Nurse-Family Partnership Guidelines, program guidelines and Physician Approved Protocols.
* Works with the client/family to develop long-term and short-term personal and family goals.
* Develop a plan of care based on nursing assessment. Provide nurse case management and health education.
* Deliver professional nursing care in the client's home, or other setting.
