Summary We are looking for an experienced and compassionate Psychiatric Nurse Practitioner to provide high-quality mental health care across various settings, including Skilled Nursing Facilities (SNF), Long-Term Care (LTC), Assisted Living (AL), and Independent Living (IL). Essential Duties & Responsibilities Psychiatric Care and Rounding: Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings. Collaborative Care Coordination: Partner with attending physicians, consulting specialists, referral sources, and facility staff to ensure comprehensive, coordinated care. Patient Evaluation and Management: Assess and manage the mental health needs of residents, perform physical exams, and gather complete medical histories. Diagnostics and Therapeutics: Order and interpret lab and diagnostic tests, make preliminary diagnoses, and develop effective therapeutic care plans for mental and physical health. Medication and Treatment Management: Prescribe and administer medications and treatments; provide injections as necessary. Patient and Family Education: Educate patients and families on medications, care plans, and preventive mental health practices. Acute Care Collaboration: Work alongside physicians and facility teams to address acute psychiatric and medical needs promptly and effectively. Qualifications Graduation from an accredited Nurse Practitioner program and ANCC certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP). Active DEA license.
