Your APP - NursePractitioner Responsibilities:
Will primarily support in-home, patient-facing visits to support patient care needs in the market.
Performs primary care in home services, initial patient screenings/physical examinations, creates appropriate assessment/plan and completes documentation in a timely manner.
Partners with other clinicians in the Care Community managing the health and wellbeing of patients living in their homes.

Korzen Health seeks a full-time Wound Care NursePractitioner available 5 days/week to lead the Wound Care Clinic program in the Syracuse, NY market area.
This position is based in our outpatient wound care clinic and is exclusively dedicated to procedural wound care. Our standard work hours are daytime hours averaging around 40 hours per week. There are no night shifts, on-call responsibilities, holiday hours, or weekend hours. All patients are seen by appointment only; walk-ins are not accepted.
This is a procedure-oriented environment primarily serving geriatric patients. The successful candidate must possess at least 1 year of procedural background. Experience with minor procedures such as suturing, debridement, and/or incision and drainage (I&D) is required, and procedural case logs will be requested as part of the credentialing process. Other types of non-dermatological procedural experience may also be acceptable on a case-by-case basis.
Wound Care and Hyperbaric Oxygen (HBO) Therapy training are available. A full clinical team will be in place from the start date.
Scope of Practice Highlights
Full team support includes Case Manager, Hyperbaric Oxygen Therapy Nurses, Office Manager, and Front/Back End Support staff.
