Day (United States of America) Job Summary:
The Infection Preventionist II (IP II) is a professional who functions under the Infection Prevention Manager, or Director/ED/AVP of Safety and Quality, with a matrix relationship to the Executive Director of Infection Prevention at the system level. The Infection Preventionist II is responsible for monitoring and responding to Healthcare Associated Infections (HAI's) and communicable infections by infection detection, data collection and analysis; monitoring; and evaluation of preventive interventions. Additional responsibilities include providing input on relevant policies and procedures, conducting risk assessments, ensuring adequacy of cleaning, disinfection, and sterilization practices, educating, and coaching staff, and assuring regulatory compliance in all aspects of infection surveillance, prevention, and control. Major Duties and Responsibilities:
The SUD Psychiatrist is a core member of the interdisciplinary substance use treatment team, responsible for the psychiatric care and medication-assisted treatment (MAT) of Veterans with substance use disorders and co-occurring mental health conditions. The psychiatrist works closely with psychologists, social workers, addiction therapists, peer specialists, and nursing staff to deliver evidence-based, recovery-oriented care.
Key duties include but are not limited to:
* Conduct comprehensive psychiatric evaluations for Veterans with substance use disorders, including assessment of co-occurring psychiatric and medical conditions.
* Initiate and manage pharmacologic treatments, including buprenorphine and other MAT options, in accordance with VA policy and clinical guidelines.
* Provide integrated care for Veterans with dual diagnoses, including mood, anxiety, trauma-related, and psychotic disorders.
* Collaborate closely with the interdisciplinary SUD team to coordinate individualized treatment planning and progress monitoring.
* Participate in group or individual psychiatric follow-up as clinically indicated.
* Complete all documentation requirements in the Computerized Patient Record System (CPRS) in accordance with VA standards.
* Provide consultation to other providers and teams on the diagnosis and management of substance use and co-occurring disorders.
* Participate in multidisciplinary team meetings, case reviews, and clinical huddles to support integrated treatment delivery.
* Monitor patient safety and contribute to suicide prevention practices through risk assessments, safety planning, and appropriate care coordination.
* Remain informed of and compliant with all relevant VA policies, federal regulations, and Joint Commission standards.
Work Environment and Supervision:
The SUD Psychiatrist works within a multidisciplinary team located in the outpatient substance use clinic. The psychiatrist exercises independent clinical judgment in their area of practice and is supervised by the Psychiatry Section Chief (or designee). What Are We Looking For?
Your Health is currently looking for a NursePractitioner to join our growing primary care family. A successful Provider will be able to perform these essential duties and responsibilities. Areas of Responsibility:
Evaluate high-risk patients with complex medical needs and develop individualized care plans.
Coordinate care activities with other members of the interdisciplinary team to ensure continuity of care and optimal patient outcomes.
Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members to coordinate patient care.
Participate in interdisciplinary team meetings to discuss patient progress, treatment plans, and care coordination efforts.
Foster effective communication and collaboration to facilitate seamless patient management and interdisciplinary teamwork.
Meet staff efficiency expectations while providing high-quality care to patients in various settings, including homes, facilities, telehealth, clinics, and specialties.
Document care management activities accurately and efficiently to support patient care plans and regulatory compliance.
Provide patients with education and resources related to their conditions, treatment plans, self-management strategies, and preventive care measures.
Offer support and guidance to patients and their families to enhance their understanding of healthcare options and promote active participation in their care.
Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.
Monitor quality measures and gaps in care, identifying opportunities for improvement and implementing corrective actions as needed
Qualifications:
NursePractitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required. License must be in good standing.
Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting.
Strong communication, collaboration, and interpersonal skills, with the ability to work effectively within an interdisciplinary team.
Ability to evaluate high-risk patients, develop care plans, and coordinate care activities to meet patient needs and improve outcomes.
Commitment to delivering compassionate, patient-centered care and promoting a culture of excellence in healthcare delivery.
Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively.
Ability to read and communicate effectively.
Strong written and verbal skills.
Basic computer knowledge.
Ability to manage and demonstrate effective leadership skills.
Should demonstrate good interpersonal and communication skills under all conditions and circumstances.
Ability to foster a cooperative work environment.
Team player with ability to manage multiple responsibilities and demonstrate sound judgment.
Must be able to work flexible hours and travel between offices, facilities, etc. Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.

Home Health NursePractitioner
Your role in this new full-time permanent career will be to support patients at home who are suffering from Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) by forming deep-rooted relationships and preparing them both emotionally and physically for the challenges of managing kidney disease. Experience with Palliative or dialysis patient care is a bonus. Expect a competitive salary starting at $110k and a 10% bonus based on experience.
Requirements:
Fully vaccinated
Ability to work Full Time, Mon-Fri 8 AM - 5 PM
2+ years of NP experience in home health care or hospice
2+ years of managed care experience
Active and unrestricted NursePractitioner license
Current and unrestricted DEA certificate/license
Ability to work without direct in-person supervision and practice autonomously
Heavy daily travel; access to transportation, a valid driver's license, and car insurance.
2-hour travel radius
80%+ of the job is on the road visiting patients.
Experience with computer-based data management programs
Experience conducting annual wellness visits or similar comprehensive visits and appropriately charting HCCs; general knowledge of risk adjustment.
Preferred Skills (Not Required):
Bilingual in Spanish preferred.
Experience in kidney disease and control of related co-morbidities (HTN, Diabetes, CHF) preferred
Experience in end-of-life discussions, palliative care, or hospice.
Experience practicing in managed care, and value-based context.
Experience conducting annual wellness visits, comprehensive visits and appropriately charting HCCs.
General knowledge of risk adjustment.
Responsibilities:
Deliver evidence-based, timely care in a manner that reduces hospitalizations, maximizes the quality of life, and puts the patient's health and satisfaction first.
Conducts assessments, including comprehensive annual wellness exams.
Prescribe medications, order tests, and collaborate with physicians.
Collaborate with care management RNs and Social Workers to ensure consistent highly satisfying patient experience, accurate documentation, and effective care planning.
Perform effectively, as reflected by improved patient quality outcomes, which will be measured and reported daily.
