The Opportunity:
The One Medical Virtual Medical Team (VMT) is a leading provider of virtual clinical care, providing world-class, convenient, evidence-based virtual medical care to One Medical patients in concert with their primary care providers. Full time 40 hours including evenings and weekends
What you'll be working on:
Treating patients via tele-health visits, including telephonic triage calls, video visit appointments, and email follow-ups
Continuous learning during weekly Clinical Rounds and through other modalities
Ongoing collaboration with both virtual and in-office teammates via daily huddles
Utilization of your specific clinical training and opportunities to give exceptional care to patients virtually
Education, licenses, and experiences required for this role:
Completed an accredited FNP program with a national certification
In the past 5 years, practiced as an Advanced Practitioner for at least:
2 years in an outpatient primary care setting seeing patients of all ages (0+), OR 2 years in an urgent care or emergency medicine setting seeing patients of all ages (0+)
Currently licensed in CA with ability to obtain additional state licenses
Ability to work weekday and weekend shifts (every other Saturday AND or Sunday required)
Current shifts range from (6am - 12am PST)

One Medical is committed to fair and equitable compensation practices.
The base hourly range for this role is $59.10 to $65.50 per hour (plus evening and weekend differential) based on a full-time schedule. Final determination of starting pay may vary based on factors such as practice experience and patient care schedule. Qualifications:
Required:

2. Provide access to psychiatric evaluation and treatment within 2 business days of a crisis and within 5 business days for routine care for 90% of requests.

In collaboration with other PsychiatricNursePractitioner staff, contribute to 1+ point reduction in ANSA score for 2 or more measures in Life Domain Functioning, Behavioral Health Needs, and/or Risk Behavior within 12 months of patient engagement in medication management services (first visit) for 50% or more of patients
Essential Functions/Skills:
Assess, diagnose, treat, appropriately monitor, and educate individuals with psychiatric and substance use disorders.
Knowledge and effective use of ICD and DSM diagnoses.
Collaborate with primary care and specialty providers for comprehensive whole-person care.
Provide crisis assessment and manage level of risk for patients in distress and be available for occasional on-call phone support after hours.
Effective use of technology and ability to provide care to patients in numerous care settings, including office, field, patient homes, and via phone and teleconferencing technology.
Coordinate with and provide consultation to a multidisciplinary team for optimal patient care.
Document care in electronic medical record according to Medicare and Medi-Cal standards in a timely manner according to agency guidelines.
Submit written reports and oral testimony and presentation of opinion in commitment and other legal proceedings and in a wide range of treatment, rehabilitative, and support services matters (e.g., Social Security applications, prior authorization of medications, etc.)
Assist the Program Director and key clinical staff in the administration of the clinical program.
Train staff in the knowledge and skills basic to the integrated and stage-based treatment of severe and persistent mental illness and co-occurring severe substance use disorders.
Provide vacation/leave of absence clinical coverage for other providers.
Comply with all policies and procedures and collaborate with Quality Assurance staff to ensure all documentation meets county, state, federal, and payor standards.
Participate in biweekly supervision with Integrated Behavioral Health Care Services Director and Supervising Physician.
Precept trainees as directed by supervisor.

This role focuses on providing post-operative and follow-up care for adult patients while supporting a team of five surgeons and two physiatrists. The ideal candidate will have at least 2-3 years of experience in orthopedic spine surgery, neuro-spine surgery, or pain management with spine patients.
Responsibilities:
Manage 20-22 patients per day in person; if taking telehealth visits, the volume may increase to 26 patients per day.
Conduct post-operative and follow-up visits.
Triage patient calls and respond to messages.
Order imaging, prescribe medications, and manage outpatient clinical needs.
Perform minor in-office procedures such as staple removal, drain removal, and wound care.
Optional: If experienced, the APP may perform trigger point, hip, and/or knee injections.
Support & Work Environment:
Work alongside a team of 7 Advanced Practice Providers (APPs) in an outpatient clinic setting.
Supported by Medical Assistants (MAs) and Patient Navigators for scheduling and administrative tasks.

The NursePractitioner (NP) is responsible for delivering the highest level of patient care at a premier fertility center. The NP assesses, diagnoses and treats patients, administers and orders appropriate tests, interprets laboratory tests, provides education and appropriate management for women and men in need of reproductive related services. The NP collaborates with physicians and other clinicians in a team-based approach to patient care.
Essential Functions and Accountabilities:
Obtains detailed medical histories, reviews medical records, and performs appropriate physical exams.
Performs detailed infertility evaluations, including but not limited to: gynecologic exams, procedures, ultrasonography, and mock transfers; order appropriate tests, and interprets laboratory values.
Performs ?morning monitoring? which includes ultrasounds to monitor patient stimulation cycles, early pregnancy monitoring, and IUI (intrauterine insemination).
Collaborates with physician in development and implementation of treatment plan appropriate to individual patient.
Prescribes appropriate medications and provides appropriate referrals.
Provides detailed and comprehensive patient counseling and education.
