Opportunity Details:
Deliveries at Mercy Anderson Hospital, 20 minutes East of downtown (7500 State Road Cincinnati, Ohio 45255)
Monday through Friday schedule (typically 3 days office, 2 days surgery , but flexible here) + shared call
500 deliveries from our Mercy Health employed group per year with a Level II NICU (team works with University of Cincinnati for MFM support and high risk)
Excellent support staff: each provider has an MA or LPN + practice has 2 triage staff, 1 surgery scheduler, 2 front desk associates and a float chaperone.
Must have Board Certification or Eligibility in OB/GYN

JOB SUMMARY
Delivering primary care and continuous healthcare services to PACE participants, utilizing the knowledge and skills required to assess, plan, and provide care in alignment with PACE Program standards. Works closely with the clinical care team to conduct assessments, reassessments, develop care plans, and deliver comprehensive primary care.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Provide high-quality primary care with a strong focus on individualized evaluation, starting with a comprehensive history and physical exam, rather than defaulting to broad diagnostic testing panels.
Collaborate closely with the IDT before referring participants to specialists, ensuring referrals are necessary and that continuity of care is maintained.
Collaborate with the Medical Director to manage specialist referrals, initiate appropriate treatment prior to referral when indicated, review specialist notes promptly, and develop and communicate participant care plans that incorporate clinical judgment and consult findings to ensure comprehensive care.
Manage and monitor all of your participants' medications, aiming to exceed expectations in medication reconciliation, and medication interactions. This can be done in conjunction with the Medical Director and our collaborating Pharmacist vendor.
Stay actively informed of participant status during hospitalizations and facility stays, providing appropriate guidance to ensure safe transitions and care continuity.
Assume shared responsibility for all participants in the center, regardless of individual assignment, supporting the IDT model of team-based care.
Participate in and contribute to daily IDT meetings, offering clinical insights and collaborating on participant-centered care plans.
Ensure accurate, timely completion of all participant assessments and documentation in compliance with policy, regulatory, and quality standards.
Review and respond to labs, imaging, and consults in a timely manner, incorporating findings into ongoing care plans and communicating changes with the IDT.
Educate participants and caregivers on self-care strategies, medication use, and wellness practices, empowering them to participate in their own care.
Use the Emergency Department judiciously, prioritizing PACE resources unless symptoms are potentially life-threatening.
Accurately document diagnoses with attention to proper coding practices, supporting risk adjustment and quality reporting metrics.
Provide coverage for the 24/7 on-call rotation, addressing participant needs promptly and ensuring proper hand-offs and communication.
Contribute to discharge planning, supporting smooth transitions across care settings.
Assist with staff training and mentoring, contributing to clinical policy development and best practice implementation.
Participate in ongoing performance and quality improvement initiatives.
Demonstrate flexibility by stepping into other roles, as allowed by licensure and experience, when needed to support the team.
Maintain and report clinical resource needs to the Medical Director, supporting effective operations.
Consistently uphold all safety, privacy, and compliance standards.
EXPERIENCE AND EDUCATION:
MD, DO, APRN, or PA
NJ: Each nursepractitioner shall be so certified by the New Jersey State Board of Nursing.
Each physician shall be licensed or authorized by the New Jersey State Board of Medical Examiners to practice medicine in the State of New Jersey.
Current State license

Board Certified in Geriatrics preferred
BLS required (must have within 90 days of employment)
2 years experience providing direct patient care (preferred)
1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).
PRE-EMPLOYMENT REQUIREMENTS:
Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

This is an at least part-time position of minimally 8 10 hours a week that will provide and oversee comprehensive psychiatric evaluation and medication management services to an especially diverse population within a primary care setting. As a member of the behavioral health team at Community Health Centers of Greater Dayton (CHCGD), this position will function as a collaborator for the PMHNP and PCPs while working collaboratively with the Medical Director, Behavioral Health Director, Behavioral Health Consultants, Primary Care Physicians, and other administrative and operational staff to ensure continuity of care for patients meeting criteria for mental health, substance use, and other related psychiatric illnesses. This position will provide services and collaborative support for patients across the lifespan. The CHCGD organization currently has six site locations and services can be provided as office-based services and telehealth services as well.
Principal Duties and Responsibilities
Provides psychiatric evaluations and medication management services
Provides diagnosis and medication education to patients and their families
Functions as a collaborative support for PMHNP, PCPs, APNs, etc. as needed
Provides Behavioral Health consultations to Primary Care Providers and other member of the Behavioral Health team as needed
Facilitates the hospitalization of patients served who require inpatient care
Maintains agreed upon level of productivity
Complete or oversees completion of disability questionnaires for behavioral health patients who apply for disability if appropriate
Refers patients to appropriate resources in the community as needed for extended or higher level of care

Responsibilities of the NursePractitioner :
Inoculates or vaccinates against communicable diseases & prescribes medications or administers treatment of acute and chronically ill patients & performs minor surgery as training & competency allows

Examines, diagnoses & treats patients within scope of practice, as identified by the State of OhioNursing Board, as applicable
Conducts rounds on infirmary patients as deemed appropriate by the CMO
Writes up medical information gathered during patient interviews according to ODRC Office of Correctional Healthcare guidelines, policies, protocols & related American Correctional Association standards currently in practice, documenting all assessments & diagnostic impressions, as well as any other treatment plans & follow -up parameters

The NursePractitioner Advanced Illness Management (AIM) facilitates advanced illness management and palliative care for patients, ensuring a comprehensive, individualized treatment plan, while demonstrating excellent clinical skills and ethical, relevant judgment. Provides diagnosis, treatment, consultation, and follow-up under the direction of the Palliative Medical Director or Primary Care physician.
As a NursePractitioner, you will:
Work in collaboration with the Professional Services Corporation (PSC) Physician or supervising physician, Primary Care Physician, and other palliative team members to deliver advanced illness management and palliative care by providing diagnosis, treatment and follow-up for patients referred to the company.
Take responsibility for advanced illness management from the initial assessment through discharge. Assessment at a minimum should include: disease-specific changes, decline or change in function, nutrition and/or cognition, inadequately controlled pain or other distressing symptoms, falls, infections or similar events, the impact of the disease burden on the patient's condition, caregiver and/or family's quality of life.
Establish the plan of care, individualized for each patient, in consultation with the patient, family and other members of the Advanced Illness team, to include the PCP.
Facilitate obtaining care for patient when a need is identified in accordance with the Plan of Care.
Participates in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.
Prescribe medications including controlled substances to the extent delegated and licensed, in accordance with state law. Order and interpret laboratory, imaging and other diagnostic tests within the scope of professional practice. Order treatments and durable medical equipment as indicated.
Communicate clinical findings to the PSC Physician as needed, but no less than bi-weekly. Re-assess effectiveness of care plan on a regular basis and modify as needed.
Maintain effective working relationships with supportive care team (e.g., Social Worker and RN Care Manager), as applicable, and involve team (internally and externally) when patient's care plan warrants team participation to achieve desired outcome.
Establish and maintain effective working relationships with those contacted in the course of work.
Communicate and document clinical findings, treatment plan, and care provided in the patient's medical record in a manner consistent with acceptable standards in order to support sound medical practice and reimbursement for services provided.
Knowledgeable of Palliative Medicine Quality Measures collection, documentation and reporting.

Excellent leadership, interpersonal, written and verbal communication skills.
Independent decision making and self-motivation.
Working knowledge or understanding of: principles of Palliative Medicine, including when a patient may be appropriate for home health or hospice care; Medicare/Medicaid regulations; and interdisciplinary process; value based and/or accountable organization requirements
Meets mandatory continuing education requirements of Gentiva and licensing board.
Must be able to work efficiently with electronic medical record software.
Education/Experience:
Minimum of Masters of Science in Nursing from an accredited school of nursing, or an equivalent as allowed under state law
Minimum of two years NursePractitioner experience
NursePractitioner specialty in Family or Adult Medicine preferred
Value Based Care practice experience preferred
Practice experience with Part B billing
Able to work autonomously in an Advanced Practice Role
Licenses/Certifications/Agreements:
Current license as a registered nurse or equivalent as required by state law for NursePractitioner licensure without board actions
Current NursePractitioner certification/license without board actions
Collaborative agreement with a supervising physician, that is part of the Professional Services Corporation, that is agreeable to supervise work in advanced illness management and palliative care program, as required by state law
Part B Provider certification
Federal DEA license
State DEA license (if required)
ACHPN - Certification in Hospice and Palliative Care Preferred
Current automobile insurance and valid driver's license
Compensation may vary within the salary range provided based on several factors including but not limited to a candidate's location, experience, education, skills, licensure, certifications and department equity.
