Typical Appointments

Job Description:
Working as a full-time Advanced Practitioner, your primary role involves delivering superior-quality, direct patient care services within a Post-Acute Care facility. You will play a cardinal role in our philosophy of delivering care that reinforces or augments each patient's clinical status and life quality.
Key Responsibilities:
- Uphold the ethos of the organization by delivering patient-centric care focused on maintaining or enhancing our patients' clinical health and life quality.
- Carry out Annual Wellness Assessments on all long-term Nursing Facility residents and, when required, conduct annual History and Physical examinations.
- Lead the clinical intervention, direct care visits, chronic co-morbid illnesses treatment, and appropriate re-assessment of care visits.
- Assess and treat residents for health changes and accurately analyze clinical abnormalities in all assigned patients.
- Collaborate with physicians for comprehensive medical management and provided care with other disciplines like Therapists, Specialists, and Social Workers.
- Attend family conferences when possible and maintain transparent communication with families.
- Perform additional duties as required.
Desired Skills and Experience:
- Hold a valid nurse practitioner or physician assistant license in the operating state and current board certification in the related field.
- Possess an accredited Bachelor's and Master's in Nursing.
- Experience in nursing is highly desirable, with preference given to candidates with a Nurse Practitioner/Provider background.
- Possess extensive knowledge in applying nursing and clinical Nurse Practitioner, alongside critical thinking skills, to the aged, chronically ill, and convalescent residents.
- Exhibit strong leadership abilities, excellent interpersonal skills, and problem-solving competencies.
- Proven excellent written and oral communication skills.
- Traits of punctuality, organizational abilities, and the adaptability to function within a team are a plus.
