What we Ask of YOU:

Position Summary:
Responsible for provision of comprehensive primary care services in a primary medical care, ambulatory clinic setting, and in Health care for the Homeless facilities. Works independently and interdependently with other clinic providers.

Your contribution to the team includes:
* Provision of comprehensive primary care services for all ages in a primary medical care ambulatory clinic setting, and in Health care for the Homeless facilities working independently and interdependently with other HealthPoint providers.
* Provide care consistent with evidence-based medicine.
* Assess patient needs for specialty referrals, diagnostic and in-patient care and make appropriate referrals for service.
* Provide patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each patient visit. Verify and document that the patient understands diagnosis, treatment and follow up recommendations.
* Complete accurate and timely documentation in patient records and superbills.
* Participate in patient triage system.
* Participate in a shared call system with other providers.
* Perform minor outpatient procedures in accordance with the scope of practice and HealthPoint privileging policy.
* Provide consultation to other clinic providers either in person or by telephone.
* Consult with clinic physicians, specialists, or other health care professionals as appropriate.
* Administer medications in clinic as needed.
* Assist with meeting clinic patient care team productivity and quality care standards.
* Participate in development and utilization of HealthPoint practice guidelines and procedures.
* Participate in audits, and meet standards of HealthPoint Quality Improvement, Risk Management and Utilization Management programs.
* Maintain credentials essential for practice, to include licensure, board certification, society memberships and CPE.
Must have's you'll need to be successful:
* Graduate from an accredited nursepractitioner program.
* Washington State nursepractitioner license. Board certification required.
* Demonstrated operating knowledge of computers: intermediate level ability with Word, Outlook, and other required software programs; prior experience with electronic medical records preferred.

About the Role
Clinicians work directly with patients to provide medication-assisted treatment for Substance Use Disorders (SUD) and empower patients in their individual recovery journeys as a member of Boulder's multidisciplinary care team.

QUALIFICATIONS
Physician Assistant: Master's degree in Physician Assistant Studies with certification by the NCCPA OR Family NursePractitioner: Master of Science in Nursing - Family NursePractitioner
Must have at least 1-2 years of experience post-graduation working in urgent care, primary care, or emergency care.
Experience seeing patients of all ages.
Eligible to participate in the Medicare program.
Must be able to demonstrate 2000 hours of work experience at the time of job offer.

Fit tested for an N-95 mask, must be clean shaven where the respirator seals to the face for fit testing or when wearing the respirator.

Perform in-depth physical assessment and management of illnesses or conditions within the ARNP scope of practice and in collaboration with MD, pharmacists, and other interdisciplinary team members.
