The Opportunity:

Full time 40 hours including evenings and weekends
What you'll be working on:
Treating patients via tele-health visits, including telephonic triage calls, video visit appointments, and email follow-ups
Continuous learning during weekly Clinical Rounds and through other modalities
Ongoing collaboration with both virtual and in-office teammates via daily huddles
Utilization of your specific clinical training and opportunities to give exceptional care to patients virtually
Education, licenses, and experiences required for this role:
Completed an accredited FNP program with a national certification
In the past 5 years, practiced as an Advanced Practitioner for at least:
2 years in an outpatient primary care setting seeing patients of all ages (0+), OR 2 years in an urgent care or emergency medicine setting seeing patients of all ages (0+)
Currently licensed in CA with ability to obtain additional state licenses
Ability to work weekday and weekend shifts (every other Saturday AND or Sunday required)
Current shifts range from (6am - 12am PST)

One Medical is committed to fair and equitable compensation practices.
The base hourly range for this role is $59.10 to $65.50 per hour (plus evening and weekend differential) based on a full-time schedule. Final determination of starting pay may vary based on factors such as practice experience and patient care schedule. Job Title & Role Description:
The NursePractitioner (NP) is responsible for providing high-quality, patient-centered, comprehensive primary care. This role primarily involves virtual patient care with periodic or as needed in-person care to support transitions of care, annual wellness visits, risk adjustments, primary care needs, and urgent visits. The NP collaborates closely with a multidisciplinary team to provide whole-person care to our patients in their homes.
Key Responsibilities:
Triage, diagnose, and treat patients in person or via our virtual care platform.
Serve as a day-to-day clinical leader, providing decision support and directing the multidisciplinary team under the guidance of the Medical Director.
Conduct primary care, urgent visits, and annual wellness exams, ensuring all appropriate screening tests and quality measures are addressed.
Refer patients appropriately for behavioral health and specialty services.
Coordinate care transitions to prevent hospital readmissions by collaborating with PCPs, hospitalists, and SNF providers.
Participate in the on-call provider schedule to support the 24/7 patient hotline, requiring after-hours and weekend availability on a rotating and recurring basis.
Foster a collaborative working environment with all members of the interdisciplinary team, contributing to daily huddles, team meetings, and patient discussions.
Monitor and respond to remote patient data collected through monitoring devices.
Perform other duties as assigned.
Knowledge, Skills & Abilities:
Perform and document accurate and thorough risk assessments for the purpose of accurate documentation of condition(s).
Expertise in managing complex, polychronic patients, with demonstrated ability to assess and treat high-acuity patients.
Familiarity with caring for Medicare and Medicaid populations, with experience in both inpatient and outpatient or home care settings.
Proficiency in motivational interviewing, trauma-informed care, and supporting vulnerable populations is preferred.
Excellent written, computer, and oral communication skills, with the ability to communicate efficiently and proactively with the clinical team.
Dedication to health equity and transforming care for patients in their homes.
Strong organizational skills and ability to provide clinical leadership and coordination.
Qualifications:
Minimum of 5 years' experience as a provider, with a minimum of 3 years in a primary care, or related, setting.
Multi-state Advanced Nursing License or willingness to obtain.
A valid driver's license and auto liability insurance.

Effective use of technology and ability to provide care to patients in numerous care settings, including office, field, patient homes, and via phone and teleconferencing technology.
Coordinate with and provide consultation to a multidisciplinary team for optimal patient care.
Document care in electronic medical record according to Medicare and Medi-Cal standards in a timely manner according to agency guidelines.
Submit written reports and oral testimony and presentation of opinion in commitment and other legal proceedings and in a wide range of treatment, rehabilitative, and support services matters (e.g., Social Security applications, prior authorization of medications, etc.)

WHAT OUR (LVN) LICENSED VOCATIONAL NURSES DO:
Provide direct nursing care to patients through routine and urgent in-home and virtual visits under the direction of an authorized health care practitioner.
Facilitate virtual visits in the patient's home to support care delivery under the direction of a remote clinical practitioner.
Provide support for clinical visits as directed, including chart preparation, clinical intake, after-visit care coordination, and clinical documentation and order entry support (i.e., bedside scribe function).
Coordinate and support patient care by implementing care plans and performing outbound calls and tasks at the direction of and under the clinical supervision of an authorized health care practitioner.
Process and track orders and referrals placed by our providers, working to ensure that patients complete orders and referrals in accordance with their treatment plan.

Deliver exceptional customer service to our patients, caregivers, and clinical partners including proactive, timely, and comprehensive communication from a holistic patient-centered perspective.
Clinical skills and procedures: Blood draws, laboratory specimen collection, medication and vaccine administration, foley catheters, wound care, removal of sutures/staples, basic hearing and vision tests, home safety evaluations, IV insertion, EKG, ABI. All clinical skills require competency evaluation and are performed under the supervision of a clinical practitioner, with scope of work and level of supervision as determined by state regulations.
Accurate and timely documentation in the medical record of all patient care support activities.
Active participation in team huddles and organizational meetings as requested.
Adherence to compliance policies, procedures, and standards of conduct including all applicable laws and regulations.
