The Palliative Care Nurse Practitioner/APRN provides direct patient and family care through palliative care consultation to ensure specialty services to patients with serious illness and a prognosis of one year or less who are not ready or eligible for hospice care.
Demonstrates effective and ethical use of approved evidence-based palliative medicine practices in the delivery of all patient
care based on physical, emotional, spiritual, psycho/social, ethical, educational, safety and related criteria, appropriate to the ages of patients served.
Demonstrates knowledgeable clinical expertise by incorporating palliative care body of knowledge to formulate an effective plan of care.
Gathers pertinent information systematically from all sources, including medical records, family history, and caregivers, and thorough specialty physical, emotional, psychosocial, and spiritual clinical patient assessment.
Completes a comprehensive assessment utilizing the palliative care domains of assessment, interprets findings, conducts medication reconciliation, and formulates problem-related diagnoses, initiates plan of care utilizing therapeutic interventions, and evaluates the results of these actions for adequate outcome responses.
RN/APRN licensure in state of practice
Nurse Practitioner/CNS with prescriptive authority. Must maintain all requirements for licensure
5 years of clinical nursing experience. At least 1 year as APRN with Hospice, Palliative Care or Oncology preferred
Advanced practice palliative care certification (ACHPN) is preferred. Certification in an appropriate advanced practice area will be considered. BCLS Licensure requirement. ACHPN Certification is encouraged following 2 years of employment or sooner when requirements for experience met.

Currently recruiting a Psychiatrist in Tacoma, WA to coordinate complex clinical operations at a facility that houses individuals detained by Immigration and Customs Enforcement. The core scope of practice includes but is not limited to: evaluation, formulation of diagnoses, and treatment of psychiatric and mental health care needs, including family units;urgent and emergency mental health care, public health management, case management, collaboration with an interdisciplinary team, and gender specific care, including transgender care and management of mental health care during pregnancy. All health care provided to ICE detainees is governed by IHSC policy and established detention standards.
DUTIES OF THE PSYCHIATRIST:
1. Provides direct care to patients within discipline Scope of Practice and performs functions in accordance with current accepted practice, licensure, certification, credentialing and or granted privileges.
2. Provides treatment of persons throughout the life span as appropriate to discipline and facility setting.
3. Provides telepsychiatry services for patients at other IHSC facilities as needed and as the local facility allows, only in the states the provider is licensed, to include:
Working with the patient s local primary care provider (PCP) to coordinate care for any identified medical concerns (patient s local administrative staff will coordinate the appointment).
Using IHSC video teleconferencing equipment and electronic medical records (EMR) software to examine and record the patient s plan of care.
Telepsychiatry services will be provided in accordance with the IHSC Tele-Mental Health Services policy.
OPTIONAL: Provider may choose to obtain additional state licensing, outside of the state of his/her duty station, to provide out-of-state telepsychiatry services.
4. May be expected to respond to and or coordinate response to medical emergencies.
5. Prescribes and monitors psychiatric medication treatment services including monitoring the side effects of medication and/or adverse reactions.
6. Offers comprehensive psycho-educational information with each medication/somatic treatment regarding the patient s mental illness, emotional disturbance or behavior disorder, treatment goals, potential benefits and the risk of treatment, self-monitoring aids, and identifies support groups for therapeutic assistance.
7. Utilizes the evidence-based algorithms for decision-making regarding patient assessment and medication management.
8. Utilizes evidence-based tools (i.E. Assessment of Involuntary Movement Scale) in the identification of risks associated with medication induced movement disorders.
9. Adheres to medical standards in accordance with IHSC policies, American Psychiatric Association standards, and legal requirements.
10. Determines the need for core treatment and specialty services for identified patients in a full range of psychiatric problems.
11. Performs various diagnostic and treatment procedures, including cognitive and behavioral psychotherapy, supportive individual and group psychotherapy, somatic therapies (including the use of traditional and atypical drugs).
12. Provides professional, recovery based psychiatric care considerate of cultural and ethnic diversity and focuses on empowering individuals to change their own lives.
13. Assures patients are treated with dignity and are consulted when evaluating and/or revising treatment and services.
14. Utilizes evidence-based practices to promote recovery, reduces stigma and unconditionally enhance the quality of life.
15. Assures a positive therapeutic milieu exists during treatment and emphasizes recovery goals.
16. Participates in comprehensive performance improvement and utilization management plans to include:

20. Assumes responsibility for the non-psychiatric medical evaluation, diagnosis and treatment of patients for whom she or he is responsible in the absence of non-psychiatric physicians in emergency situations and within incumbent s competency and scope of practice.
21. Spends at least 60% of time in direct patient care to meet the performance targets set by the IHSC.
22. Provides specialized medical services to outpatients and provides medical care to inpatients in facilities with a short stay unit. Specific duties involve all aspects of health care delivery. The incumbent will work in concert with both facility staff and consultant specialists in various clinical areas.
23. Acts as a mental health consultant to medial staff, providing oversight and direction related to the provision of mental health care.
24. Examines and orders appropriate diagnostic testing and provides necessary treatment and/or medications as required. Refers selected cases to consultant specialists when necessary. Refers residents requiring hospitalization to a community hospital and to an appropriate admitting physician.
25. Ensures that proper evaluations and treatments are carried out and documented. Incumbent shall constantly monitor and evaluate the quality of patient care, providing data to the departmental Performance Improvement program, and other agencies or committees as required.
26. May be required to see patients who are not in the category of his/her expertise but within his or her scope of practice.

30. Completes and passes all discipline specific competencies testing, initially and annually.

32. Maintains patient confidentiality, and confidentiality of medical records, in compliance with the Privacy Act and HIPAA regulations in all work activities.
33. Adheres to, and maintains awareness of, IHSC Policies, Procedures, Directives, Operational Memoranda and accreditation standards as prescribed by ICE/IHSC.

EDUCATION AND EXPERIENCE:
1. Doctor of Medicine or Doctor of Osteopathic Medicine from a school in the United States or;
2. Doctor of Medicine or equivalent degree from a foreign medical school that provided education and medical knowledge substantially equivalent to accredited schools in the United States as established by:
a. The National Committee on Foreign Medical Education and Accreditation (NCFMEA) and
b. Permanent certification by the Educational Commission for Foreign Medical Graduates (ECFMG).
3. Completion of four years of medical specialty training leading to board eligibility in psychiatry.

GENERAL SUMMARY
The Psychiatric Nurse Practitioner is responsible for providing psychotherapy and medical evaluation to a range of clients having mental illness and substance use disorders.
This position ensures confidentiality, security, and accuracy of records, and promotes positive employee, customer, and community relations. Strong written and verbal communication skills are required and outstanding customer service to fellow employees, customers, vendors, etc. is expected. Regular attendance is required, later hours or weekend time may be required, and a neat well-groomed professional appearance is essential at all times.
PRIMARY RESPONSIBILITIES
Conducts medication assessments and crisis response to both new and existing clients.
Develops individual service plans based on assessment data and provides therapeutic interventions as needed to include providing on-going medication management services.
Provides consultation and health education to NGHC staff, caregivers, and clients as it is relative to the treatment and medication needs of the clients.
Maintains documentation of services including patient service plans, progress notes, medication informed consent, treatment plan reviews, and all other required information within NGHC, federal, and state compliance standards.
Interprets diagnostic or laboratory tests such as electrocardiograms (EKGs), renal functioning tests, and medication serum levels.
Writes prescriptions for psychotropic medications as state and federal regulations allow.
Meets regularly with Mental Health and SUDP Counselors to identify and coordinate optimum treatment plans for enrolled clients.
Collaborates with consulting Psychiatrists as necessary to establish ongoing care of complex patient scenarios.
Develops practice protocols for mental health problems based on review and evaluation of published research.
Provides general health counseling services under the supervision of the Medical Director and follows the progress of patients. Makes appropriate records of findings during examinations and submits charts to the Medical Doctor for review and signature.
Monitors organization-wide performance improvement activities as a member of the clinic medical team and participates in staff meetings and/or consultations as requested.
Ensure security and confidentiality of all program data and operate in accordance with applicable regulations of Health Insurance Portability and Accountability Act (HIPAA) and 42 CFR Part 2.

MINIMUM QUALIFICATIONS Education and Experience

Five years' experience of professional work as a Psychiatric Nurse Practitioner.
Experience in substance abuse treatment.

Position Summary:
Responsible for provision of comprehensive primary care services in a primary medical care, ambulatory clinic setting, and in Health care for the Homeless facilities. Works independently and interdependently with other clinic providers.

Your contribution to the team includes:
* Provision of comprehensive primary care services for adolescents in a school based, ambulatory clinic setting.
* Provide care consistent with evidence based medicine.
* Provide consultation to other clinic providers either in person or by telephone.

* Assess patient needs for specialty referrals, diagnostic and in-patient care and make appropriate referrals for service.
Must have's to be successful:
* Graduate from an accredited nurse practitioner program and have a minimum of two years of experience with adolescents.
* Washington State nurse practitioner license; must be certified
* Demonstrated operating knowledge of computers: intermediate level ability with Word, Outlook, and other required software programs; prior experience with electronic medical records preferred.
