Responsibilities:
About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member's current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member's overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers:
Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
Administer and order point of care and lab-based testing as needed
Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary
Provide services at the member's place of residence which can be in their homes or Skilled Nursing Facilities

Flexibility - Visits are scheduled based on YOUR availability. We conduct assessments Sunday - Saturday from 8 am - 7 pm. You can work during the day, evening, and/or weekend based on your schedule.
Competitive Pay - Earn income based on the number of assessments you complete. Increase your earning potential by offering greater availability and traveling to see members outside your home location. Those completing a monthly average of 20 - 100 visits earn $1,800 - $11,500 per month. This is based on the per assessment completion rate range of $90 - $115 depending on your state of residence.

QUALIFICATIONS
Education : Preferred possession of a Master's Degree in biological science or Public Health from an approved college or university with specialization in bacteriology, microbiology, epidemiology, or related field; For LTC: RN or LVN with two years LTC experience and infection surveillance in a LTC facility will substitute for master degree.
Minimum Experience : Working knowledge of statistics, microbiology, patient care practices, epidemiology, and occupational health preferred.
Required Licenses/Certifications : Obtain Certification in Infection Control (CIC) from Board of Infection Control and Epidemiology within 36 months of hire if not possessed. For LTC Infection Prevention and control certification by NADONA within 6 months of employment is required.
PAY RANGE
$39.44 to $65.77 per hour
The pay range for this position reflects the base pay scale for the role at Alameda Health System. Final compensation will be determined based on several factors, including but not limited to a candidate's experience, education, skills, licensure and certifications, departmental equity, applicable collective bargaining agreements, and the operational needs of the organization. Brief Description of Duties
* Teaches Family Nurse Practitioner Course Content
* Collaborates in maintaining Program and Course Outcomes
* Participates in program level meetings
* Collaborates to design or modify course content and teaching strategies

* Conducts face-to-face teaching on onsite days

Clinical Nurse II's may supervise the work of professional and paraprofessional staff of the unit to which assigned.
DUTIES & ESSENTIAL JOB FUNCTIONS: NOTE: The following are the duties performed by employees in this classification, however, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.
1. Accompanies, assists, and represents the needs of patients to other providers.
2. Accurately provide care with respect to medication administration, skin and wound care, ADL's and other essential patient care related activities.
3. Develops, implements, evaluates and makes modifications in the nursing care plan; prepares required records and reports.
4. Gathers and assess information about patients to meet nursing needs; assists and consults with physician in the performance of procedures and diagnostic tests; contracts physicians and/or other departments to obtain or provide patient information.
5. May provide oversight to staff who monitor telemetry systems; alerts primary nurse of changes in underlying rhythms and of any life threatening arrhythmias that may develop; interprets and posts telemetry strips at the hours specified; admits and discharges patients with telemetry units as appropriate.
6. Organizes, supervises, makes adjustments and accepts responsibility for the quality of care provided patients by co-workers for a given work period; at the level II assumes responsibility as a team leader or a charge nurse when assigned.

8. Participates in the implementation of patient care standards, infection control standards and quality assurance criteria; assist with conducting studies; participates in unit and other meetings.
9. Provides and evaluates patient care established standards and criteria in conformity with the nursing care plan. Monitors patients for significant and critical changes and initiates procedures as required; documents care given according to set standards and at required intervals. Interprets and explains procedures, regimens, and services to patients and families; teaches patients and family members health care and disease prevention techniques. Minimum Qualifications:
Education: Graduate of an accredited Nursing Program required; Bachelor's degree in Nursing preferred.
Minimum Experience: Six months experience as a Clinical Nurse I with AHS or the equivalent of one year full time recent experience at a comparable level in a minimum 100-bed hospital or in the area of specialty; (if experience is not recent, completion of an approved refresher course may be acceptable).
Preferred Licenses/Certifications: TNCC -Trauma Nurse Core Course (For Emergency department only).
Required Licenses/Certifications : Valid license to practice as a Registered Nurse in the State Of California.
Preferred License/Certifications: CCRN - Critical Care Registered Nurse.
