Facilitate outpatient psychiatric patient visits for the evaluation and management of established patients. Ability to maintain strict client confidentiality. Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, and other pertinent information. Recognize impact of Social Determinants of Health factor as drivers for behavioral health needs and implement intervention and evaluation approach. Evaluate patients for the need for a higher level of care, including hospitalization Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare service. Prescribe psychiatric medications as indicated. Ability to be respectful of the diverse cultures of the people served and to provide culturally appropriate, competent, and individualized treatment according to each client's age, gender, race, ethnicity, and culture Assist with ensuring hospital discharge recommendations are understood by consumer and followed through with by the patient. ‘ Ensure interventions are recovery‐oriented, culturally congruent, and developmentally appropriate Maintain high quality documentation of all case work, completed in a timely manner, and consistent with outlined program policies and licensure requirements.
