Typical Appointments

Quality and Compliance
The NP will provide face-to-face recertification visits and will assure the accurate and timely documentation of those visits to assure seamless care of Unity's hospice patients.
The NP will identify practices of concern, risk, and learning opportunities.
The NP will assist in the development and delivery of comprehensive, quality plans of care for patients/families in accordance with Unity program standards, collaborating with other members of the IDT.
The NP will accept the role of overseeing the patient's end-of-life care needs if/when needed or requested.
The NP will ensure the accuracy and timeliness of documentation to support CPT codes and billing.
The NP will adhere to Unity's policies regarding the protection of all health information.

We are seeking an experienced Nurse Practitioner to provide compassionate, high-quality primary care in a correctional or institutional healthcare setting. The NP will work a consistent weekday schedule and collaborate with an interdisciplinary team to deliver comprehensive medical care to patients.
Key Responsibilities:
Provide primary and preventive care, including assessment, diagnosis, and treatment of acute and chronic conditions.
Manage an average daily caseload in accordance with facility needs.
Collaborate with physicians, nurses, and ancillary staff to ensure coordinated patient care.
Maintain accurate medical records and documentation per facility and state standards.
Perform minor procedures as applicable (preferred but not required).
Ensure timely communication and follow-up regarding patient care needs.
Qualifications:
Active and unrestricted Nurse Practitioner license in the state of practice.
Minimum of 2 years of clinical experience in primary care, family practice, or internal medicine.
Current BLS certification.
Strong communication and clinical assessment skills.
Prior correctional or institutional healthcare experience preferred but not required.

Responsibilities
Knowledge of CMS Regulations and NCQA HEDIS Guidelines;
Knowledge of Evidence-based Clinical Practice Standards: American Diabetes Association (ADA), American College of Cardiology (ACC), American Heart Association (AHA) Familiarity with ICD-9, ICD-10 and CPT-4 coding practices;
Complete comprehensive, accurate and thorough review of the assigned member population, including timely completion and submission of all required encounter documentation (paper or electronic);
Ensure that all pertinent and active medical conditions are documented in the medical record in a manner compliant with CMS/DHHS, Inovalon policy, and client requirements;
Support the physician/patient relationship and ensure timely and adequate communication, documentation of assessment findings, recommendations, need for additional services, emergency services required if necessary and need for follow up and timelines for follow up to primary care provider (PCP) and health plan as required;
Make general recommendations to members intended to improve members' knowledge of their chronic condition(s), such as information concerning recommended testing;
Address and close identified gaps in care (disease-specific or preventive);
Recognize emergent or urgent situations requiring escalation and take appropriate action as specified in Inovalon policies, and as determined by reasonable professional judgment and ethical professional practice standards; and
Perform, document and communicate results of Point of Care (POC) Testing.
Maintain compliance with Inovalon's policies, procedures and mission statement;
Adhere to all confidentiality and HIPAA requirements as outlined within Inovalon's Operating Policies and Procedures in all ways and at all times with respect to any aspect of the data handled or services rendered in the undertaking of the position;
