Your Qualifications:
Board Certified by the ANCC as a PMHNP

QUALIFICATIONS
* Education and Licensing: Graduate of a school of professional nursing accredited by an accrediting institution recognized by the U.S. Department of Education. A valid and unrestricted NursePractitioner license and is not barred from practicing in any of the 50 States, the District of Columbia, or a Commonwealth, territory, or possession of the United States.
* Required at least 1 year of experience working as a NursePractitioner.

About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member's current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member's overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers:

* Administer and order point of care and lab-based testing as needed

Role OverviewAs a Primary Care Advanced Practitioner, you will work collaboratively within a small, close-knit care team. You will provide direct patient care, manage chronic conditions, and promote wellness through education and preventive measures.

Responsibilities:- Provide comprehensive primary care services in an outpatient setting.
- Conduct routine office visits and address acute medical concerns.
- Perform annual wellness visits and health risk assessments.
- Educate patients on their conditions, treatment plans, and preventive care strategies.
- Coordinate care with specialists, diagnostic facilities, and other healthcare providers.
- Utilize telemedicine as appropriate for patient follow-up and consultation.
- Participate in care team discussions, ensuring best practices in patient management.
- Maintain accurate, thorough documentation of patient encounters.
- Engage in population health initiatives, including routine screenings and preventive care measures.
- Other duties as assigned.
Qualifications:- Certified NursePractitioner with at least 5 years of experience in primary care (internal medicine, family medicine, or geriatrics preferred).
- Experience with Medicare patients and senior-focused care.
- Strong understanding of medical documentation, coding (ICD-10, HCC), and risk adjustment.
- Active, unrestricted state license and DEA registration.
- Medicare enrollment in good standing or eligibility to enroll.
- Passion for primary care and patient-centered medicine.
- Tech savvy and a quick learner!
- Experience with Athena preferred.
- Strong teamwork and collaboration skills.
- Ability to adapt to a dynamic clinical environment and embrace technology in patient care.
