Job Description This position will provide comprehensive psychiatric evaluation and medication management services to an especially diverse population within a primary care setting. As a member of the behavioral health team at Community Health Centers of Greater Dayton (CHCGD), this position will work collaboratively with the Psychiatrist, Behavioral Health Director, Behavioral Health Consultants, Primary Care Physicians, and other administrative and operational staff to ensure continuity of care for patients meeting criteria for mental health, substance use, and other related psychiatric illnesses. This position will serve the child, adolescent, and adult populations. The CHCGD organization currently has six site locations and services can be provided as office-based services and telehealth services as well.
