Essential Functions
Provision of Outpatient Care, including:
Collecting patient histories and conducting physical examinations.

In the clinic setting, participate in the education of nursing and medical assistant staff regarding disease processes (e.g., diabetes, asthma) and how to implement disease management at the practice level.
Care management of higher risk patients, including adherence to prescribed medicine regime, attending medical monitoring clinic visits at recommended frequency, coordination of care of specialists and primary care.
Required Education
Master?s Degree in Nursing from an accredited nursing program.

Perform pump refills, reprogramming, and troubleshooting in coordination with prescribing providers.

Collaborate with physicians, pharmacists, and other healthcare team members to ensure continuity and quality of care.

Qualifications
Registered Nurse (RN) with an active, unencumbered license in the state of practice.
Additional licensure or compact status preferred
Minimum 2 years of nursing experience with at least 1 year in home infusion or pain management.

Establish and maintain open and positive communications with facility staff and administration. Provide verbal and/or written instruction or feedback regarding medications and other pertinent caregiver information. Meet with Administrator or designated facility care coordinator equivalent prior to the start of your patient care visits each day and prior to leaving facility each day.
Be knowledgeable of and adhere to Eventus WholeHealth standards, policies and procedures. Be aware of and adhere to all legal and regulatory agencies' rules and guidelines and professional ethical standards. Comply with all regulatory agencies governing health care delivery.
Maintain patient confidentiality at all times including appropriate use of Cell phone, emails text messaging, patient charts and EMR. Agrees to abide by and be knowledgeable of HIPAA rules and regulations

The CHCGD organization currently has six site locations and services can be provided as office-based services and telehealth services as well.
Principal Duties and Responsibilities
· Provides psychiatric evaluations and medication management services
· Provides diagnosis and medication education to patients and their families
· Provides Behavioral Health consultations to Primary Care Providers and other member of the Behavioral Health team as needed
· Facilitates the hospitalization of patients served who require inpatient care

Job Overview:
We are looking for a dedicated Licensed Psychiatric Mental Health NursePractitioner (PMHNP) to join our team as an independent contractor. This flexible position offers a minimum commitment of 24 hours per week with in-person availability for at least 16 hours per week (mix of days with at least 2 afternoon/evenings). While 24 hours is the baseline, we welcome the possibility of additional hours based on availability.
In this role, you will play a pivotal part in establishing and expanding psychiatric services, working closely with counselors as part of an integrated care team. The position is structured as a 1099 contract, offering a combination of in-person and telehealth ambulatory work.

Responsibilities:
Provide high-quality care to patients as scheduled.
Collaborate closely with the designated physician to ensure effective patient outcomes.
Maintain a professional and patient-centered approach at all times.
Manages the healthcare of patients with compassion, cultural sensitivity, and preserves dignity and rights.
Prescribes medications and non-pharmacological therapies based on the patient's diagnoses, health history, and age.
Encourages positive health behaviors through education and counseling.
Applies standardized care guidelines in clinical practice.
Performs designated procedures.
Ensures compliance.
Qualifications and skills:
Required Board License - PMHNP.
Ability to commit to at least 24 hours of availability per week.
Willingness to work more hours as needed
Need to be open with 1099
Excellent written and verbal communication skills.
Organized, detail-oriented, self-motivated and a self-starter.
Ability to function as an effective member of an integrated healthcare team.
Two years relevant clinical health experience preferred.
