We are seeking an experienced PsychiatricNursePractitioner to join our team and provide high-quality, compassionate care to our patients. Collaborate with physician(s) and IDT in discharge planning from acute or long-term care settings.
In consultation with the PACE Physician and/or Community-Based PACE Physician, refer participants to a psychiatrist as indicated.
In collaboration with the PACE Physician and/or Community-Based PACE Physician, oversee the care of participants in the nursing home, making regular visits as dictated by WelbeHealth standards and participant needs.
Be a consultant to IDT, which includes communicating participant changes, collaborating on care planning arrangements, and coordinating 24-hour care delivery.

EDUCATION AND QUALIFICATION REQUIREMENTS
Master's degree in nursing, with a Psychiatric/Mental Health emphasis from an accredited college or university.
Certified as a Psychiatric Mental Health NursePractitioner.
Active Psychiatric Mental Health NursePractitioner State License without restrictions.
State CDS Certification and DEA License.
NPI license.
Valid State Driver's License with a clean DMV record.
Copy of recent Vehicle Insurance required.
Minimum of two (2) years of Psychiatric Mental Health experience working with the frail or elderly.

You also interpret and integrate data and order laboratory studies, x-rays, and other diagnostic and therapeutic procedures as indicated. With great attention to detail, you review lab and radiology results in consultation with the supervising physician. You submit patient health care plans and goals for periodic review and evaluation by the physician. To accomplish each patient's treatment plan, you also provide prescriptions for medications. When you are on-call, you take calls for pharmacies and assist the MD.
Using your excellent communication and people skills, you counsel patients and their families on health promotion, diagnosis, prognosis, and management options. You recommend community resources to meet patient and family needs. When it comes to medications and treatment instructions, you are sure to instruct patients and authorized family members in a way that is easy to understand.
