Key Responsibilities

RESPONSIBILITIES:
Assess patient wounds and develop a comprehensive treatment plan.
Debridement of wounds by sharp, enzymatic or autolytic methods.
Application of topical wound care and associated wound management products.
Recommendation of associated services to improve wound healing outcomes. This includes, but is not limited to, labs, radiologic tests, vascular tests, referrals, home health care, Negative Pressure Wound Therapy device, and orthotics.
Interpretation of diagnostic tests and recommendation of appropriate follow up.
Recommendations for prescription of required medications and/or Durable Medical Equipment (DME) to facilitate wound healing and/or wound prevention within her/his scope of practice, license and as per the collaborative agreement with the supervising physician.
Risk assessment for Members with Diabetes, as per the American Diabetes Association (ADA) guidelines.
Patient education and management.
Education of the Primary Care Physician (PCP), Case Manager (CM), or referring Physician on current wound management practices.
Provide patient updates to PCP, CM, and Physicians.
Communicate in a clear and understandable manner using appropriate verbal and non-verbal styles.
Actively participate in departmental activities to improve training methods and achieve company goals.
Assist with one-on-one training and coaching sessions as assigned.
Attend required meetings and participate as a committee member as requested.
Assist with special projects as needed.
Maintain strict confidentiality.
Other duties as assigned.
Qualifications
Candidates must hold a current state license as a NursePractitioner or Physician Assistant. Experience in direct patient care, particularly in the homecare setting, is highly valued. Strong clinical acumen, excellent communication skills, and a passion for innovative wound treatment are essential.
Requires manual dexterity, sitting, standing, stooping, reaching, kneeling, crouching, bending, walking, lifting to 50 lbs. Close vision and ability to adjust focus.

Consult with/or refer clients with abnormal findings such as Anal Cancer when indicated
Perform or order laboratory tests and interpret the results as part of the HRA procedure. Report communicable diseases in a timely manner to the appropriate health department as per health center protocol.
Follows established treatment guidelines as indicated by health center protocols
Conduct health teaching and counseling during the medical encounter as appropriate, enabling the client to make informed choices about health and treatment options

Total annual compensation range: $150,000-$200,000+, based on experience, caseload, and availability.

What You'll Bring

The Acute Care NursePractitioner (ACNP) manages patients on their specialty service/practice under the direction and indirection supervision of their supervising physician, who may or may not be the service line Medical Director. This role is a leadership position and the individual is responsible for effective time management, resource utilization, cost effective delivery of patient care and demonstration of positive role modeling. Qualifications:
Masters Degree in Nursing
Current California RN license and NursePractitioner Certificate
Specialty Board Certification as an ACNP or equivalent in specialty area
Current California Furnishing License and DEA Number
