Recommendations for prescription of required medications and/or Durable Medical Equipment (DME) to facilitate wound healing and/or wound prevention within her/his scope of practice, license and as per the collaborative agreement with the supervising physician. Risk assessment for Members with Diabetes, as per the American Diabetes Association (ADA) guidelines. Patient education and management. Education of the Primary Care Physician (PCP), Case Manager (CM), or referring Physician on current wound management practices. Provide patient updates to PCP, CM, and Physicians. Communicate in a clear and understandable manner using appropriate verbal and non-verbal styles. Actively participate in departmental activities to improve training methods and achieve company goals. Assist with one-on-one training and coaching sessions as assigned. Attend required meetings and participate as a committee member as requested. Assist with special projects as needed. Maintain strict confidentiality. Other duties as assigned. Qualifications Candidates must hold a current state license as a Nurse Practitioner or Physician Assistant. Experience in direct patient care, particularly in the homecare setting, is highly valued. Strong clinical acumen, excellent communication skills, and a passion for innovative wound treatment are essential.
